We report a forty-six-year-old man with a past history of brain abscess managed by surgical drainage and recurrent ischemic strokes. After treatment of brain abscess, he had been on medication for symptomatic epilepsy, but had ceased medication by his judgment. He was taken to a hospital in an ambulance for an epileptic seizure. In the hospital he suffered from drug-induced renal dysfunction caused by the intravenous anti-epileptic drug, and right hemiparesis due to ischemic stroke occurred on the third hospitalization day. He was transferred to our hospital to get a treatment for renal failur…

PURPOSE: We report a rare case of brainstem abscess with hemorrhage mimicking diffuse intrinsic pontine glioma (DIPG).
METHODS: A 7-month-old baby girl presented with lethargy and poor oral feeding. She had the mild fever for 1 month. Brain computed tomography revealed the hypodense lesion in the pons. Brain magnetic resonance images (MRI) showed around 1.4-cm-sized rim-enhanced mass with perilesional edema and internal hemorrhage in the pons. The cerebral blood volume was increased in the rim-enhanced area. The provisional diagnosis was DIPG, but the mass did not show the expansile mass with

Dematiaceous fungi are responsible for a wide variety of infectious syndromes. They are often found in soil and generally distributed worldwide. This suggests that most if not all individuals are exposed to them, presumably from inhalation or trauma. In recent years, these fungi have been increasingly recognised as important pathogens. The spectrum of diseases they are associated with has also broadened and includes superficial and deep local infections, allergic disease, pneumonia, brain abscess and disseminated infection. For some infections in immunocompetent individuals, such as allergic f…

A cerebellar abscess due to Streptococcus intermedius is an uncommon clinical entity that may present with features resembling a neoplastic lesion, making early recognition challenging. S. intermedius, a member of the Streptococcus anginosus group, has a marked ability to induce tissue destruction and trigger a pronounced inflammatory response, ultimately leading to the formation of purulent collections with peripheral enhancement on neuroimaging. Although gram-positive cocci are recognized causes of brain abscesses, involvement of the cerebellum by this organism is rare. Awareness of this aty…

The case report highlights the potential intracranial complications associated with otitis media, particularly cholesteatoma and mastoiditis, including meningitis, brain abscesses, epi- and subdural empyema, and sigmoid sinus thrombophlebitis. Pyogenic ventriculitis (PV), an inflammation of the ependymal lining of cerebral ventricles, is a rare but serious complication often secondary to factors such as meningitis, cerebral abscesses, or neurosurgery. The case presented involves a 52-year-old male with a history of head trauma who developed PV secondary to cholesteatoma. Despite initial challe…

A 40-year-old man underwent ascending aorta replacement for an acute type A aortic dissection. After the operations, methicillin-resistant Staphylococcus aureus was identified in sputum and blood cultures. Although anti-methicillin-resistant Staphylococcus aureus drugs were administered, most of the intermittent blood cultures remained positive. The focus of methicillin-resistant Staphylococcus aureus infection was not evident in the early stages, and no specific symptoms such as abscess or endocarditis were observed. However, abscesses in the brain, mediastinum and spleen were found 3 years a

BACKGROUND: Chromoblastomycosis and phaeohyphomycotic abscesses are infections of the skin and subcutaneous tissues caused by dematiaceous fungi; more rarely, phaeohyphomycotic brain abscesses can occur. The epidemiology and clinical outcomes of chromoblastomycosis and phaeohyphomycotic abscesses are not well-understood in the United States.
METHODOLOGY/ PRINCIPAL FINDINGS: We used data from the Healthcare Cost and Utilization Project's National Inpatient Sample to obtain yearly national estimates of chromoblastomycosis and phaeohyphomycotic abscess-associated hospitalizations. We examined ag…

Primary lymphoma can occur in the brain and is restricted to the central nervous system (CNS). Secondary lymphoma involves the CNS after affecting other organs in the body. The prognosis is worse for secondary CNS lymphoma. Early diagnosis and treatment are mandatory. We report a case of secondary CNS lymphoma that was misdiagnosed as a cerebral abscess because of its unusual features.

Actinomycosis is a bacterial infection that affects chiefly the cervico-oro-facial area, the abdomen and lungs. From these sites it may sometimes spread to the central nervous system. Extremely rare, and controversial, is the possibility of a primary infection of the central nervous system. We report an odd case of apparently primary brain abscess caused by Actinomyces israeli and review the literature.

Submucous resection of the nasal septum (SMR) is a relatively simple and frequently performed operation. Yet, its intracranial complications, although rare, can be very serious and include anosmia, visual disturbances, cavernous sinus thrombosis, meningitis, cerebrospinal fluid rhinorrhea, pneumocephalus, subarachnoid hemorrhage, subdural empyema, and brain abscess. This article reviews intracranial complications of SMR, and the pathogenesis and prevention of such complications are discussed.

Intramedullary spinal cord abscesses rarely cause impaired consciousness without intracranial infection. We report the case of a 65-year-old woman who presented with neck pain and right upper limb weakness. She developed rapidly progressive impaired consciousness within three days of admission. Initial magnetic resonance imaging revealed a right-sided intramedullary lesion at C4-C5 with surrounding edema extending to the brainstem. Brain imaging showed no abnormalities. An initial diagnosis of a spinal cord tumor was made. Follow-up imaging demonstrated lesion expansion with ring enhancement.

Neurobrucellosis is a rare complication of brucella infection which presents as meningitis, meningoencephalitis, subdural empyema, brain abscess, myelitis, and radiculo- neuritis. We report the first case of neurobrucellosis presenting as an infected cerebellopontine cistern epidermoid cyst in a young immunocompetent male who presented with fever and acute raised intracranial pressure. MRI brain showed an extra-axial mass in the right cerebellopontine angle cistern with peripheral rim enhancement and diffusion restriction. Emergency surgery unveiled a well-encapsulated lesion containing thick …

Extra-intestinal amebiasis is secondary to invasive intestinal infections and usually results in an amebic liver abscess. Other organs, including lungs, brain, skin, spleen and kidneys, may be involved. Diagnosis of the cerebral infection is of the utmost importance and is most often made by detection of the organism at the time of brain biopsy or at autopsy. We report the first case of Entamoeba histolytica encephalitis diagnosed by PCR of the cerebrospinal fluid. The patient was treated successfully with metronidazole. PCR is an increasingly useful tool for the diagnosis of central nervous s

In a 10-year-old boy presenting with focal seizures and 6 months later with acute right-sided hemiparesis, the diagnosis of a cerebral abscess due to Actinomyces israelii was established. The immunocompetent child suffered from a complex congenital heart disease with pulmonary arteriovenous shunts and pulmonary hypertension causing mild cyanosis. His parents had been reluctant to agree to neuroimaging investigations resulting in a delayed diagnosis. Despite the long interval between first symptoms and commencement of treatment including neurosurgical excision of the abscess followed by a 4-wee

A 66-year-old woman with a diagnosis of asymptomatic Rathke's cleft cyst had been observed for almost 2 years and was admitted to our hospital in June 1992 because of high fever and loss of consciousness. Ophthalmological examination revealed right Horner's sign and ophthalmoplegia. A tentative diagnosis of bacterial meningitis was made based on the results of a lumbar puncture. T1-weight MRI showed a marked increase in the size of the suprasellar cystic mass lesion compared to the MRI images obtained 2 years earlier. Hence, the preoperative diagnosis was pituitary abscess. Both transsphenoida
