Postpartum anemia II: prevention and treatment.

This review focuses on the prevention and treatment of anemia in women who have just given childbirth (postpartum anemia). The problem of anemia both prepartum and postpartum is far more prevalent in developing countries than in the Western societies. The conditions for mother and child in the postpartum, nursing, and lactation period should be as favorable as possible. Many young mothers have a troublesome life due to iron deficiency and iron deficiency anemia (IDA) causing a plethora of symptoms including fatigue, physical disability, cognitive problems, and psychiatric disorders. Routine sc

Epidemiological association in US veterans between Gulf War illness and exposures to anticholinesterases.

To investigate complaints of Gulf War veterans, epidemiologic, case-control and animal modeling studies were performed. Looking for OPIDP variants, our epidemiologic project studied 249 Naval Reserve construction battalion (CB24) men. Extensive surveys were drawn for symptoms and exposures. An existing test (PAI) was used for neuropsychologic. Using FACTOR, LOGISTIC and FREQ in 6.07 SAS, symptom clusters were sought with high eigenvalues from orthogonally rotated two-stage factor analysis. After factor loadings and Kaiser measure for sampling adequacy (0.82), three major and three minor sympto

Exploring the ATN classification system using brain morphology.

BACKGROUND: The NIA-AA proposed amyloid-tau-neurodegeneration (ATN) as a classification system for AD biomarkers. The amyloid cascade hypothesis (ACH) implies a sequence across ATN groups that patients might undergo during transition from healthy towards AD: A-T-N-➔A+T-N-➔A+T+N-➔A+T+N+. Here we assess the evidence for monotonic brain volume decline for this particular (amyloid-conversion first, tau-conversion second, N-conversion last) and alternative progressions using voxel-based morphometry (VBM) in a large cross-sectional MRI cohort. METHODS: We used baseline data of the DELCODE cohort of

Measuring intrinsic capacity among Canadian adults: a population-based study using the Canadian Longitudinal Study on Aging (CLSA).

BACKGROUND: Intrinsic capacity (IC) is the composite of an individual's physical and mental capacities, capturing underlying reserve and heterogeneity in the aging process. Despite growing recognition of the importance of IC, inconsistencies in measuring IC persist. The objective of this study was to develop a measure of IC in Canadian adults and to assess its psychometric properties. METHODS: Data from the Canadian Longitudinal Study on Aging (n = 30,097; 50.9% female; 96.0% White) were used. Candidate measures of IC were identified through key documentation and expert consultation. A total

Precision in Complexity: A Protocol-Driven Quantitative Anatomic Strategy for Giant Olfactory Groove Meningioma Resection in a High-Risk Geriatric Patient.

Background/Objectives: Managing large midline olfactory groove meningiomas is especially difficult in elderly patients who have limited physiological reserves. Here we describe a unique and dangerous geriatric case where we used new quantifiable anatomical measurements and developed a structured multidisciplinary preoperative and postoperative protocol to assist in all aspects of surgery. Case Presentation: A 68-year-old male with fronto-lobe syndrome and disability (astasia-abasia; Tinetti Balance Score of 4/16 and Gait Score of 0/12) as well as cognitive dysfunction (MoCA score of 12/30) and

Modified 5-Item Frailty Index Is Associated with Postoperative Delirium but Not Survival in Patients Undergoing Surgery for Oral Squamous Cell Carcinoma.

Background/Objectives: Frailty is increasingly recognized as a clinically relevant marker of reduced physiological reserve in surgical oncology. The modified 5-item frailty index (mFI-5) is simple and practical, but its clinical significance in oral squamous cell carcinoma (OSCC) remains incompletely defined. This study evaluated the association between mFI-5-defined frailty and postoperative complications, especially postoperative delirium, as well as survival outcomes in patients undergoing surgery for OSCC. Methods: We retrospectively analyzed 127 patients who underwent surgical resection f

Timing of evidence-based psychotherapy for posttraumatic stress disorder initiation among Iraq and Afghanistan war veterans in the Veterans Health Administration.

OBJECTIVE: Cognitive processing therapy (CPT) and prolonged exposure therapy (PE) were widely disseminated to treat posttraumatic stress disorder (PTSD) in the Veterans Health Administration (VHA). However, few Iraq and Afghanistan war veterans (Operation Enduring Freedom [OEF], Operation Iraqi Freedom [OIF], Operation New Dawn [OND]) diagnosed with PTSD have received CPT/PE and many initiate CPT/PE after substantial delay. Veterans who do not initiate CPT/PE or initiate CPT/PE after delay may have poorer treatment outcomes. This study aimed to identify predictors of CPT/PE initiation and timi

High-dose intravenously administered iron versus orally administered iron in blood donors with iron deficiency: study protocol for a randomised, controlled trial.

BACKGROUND: About 2-3 % of the population participates in blood donation programmes. Each whole blood donation or ten apheresis donations cause a loss of 200-250 mg of iron. As a result, one of the most common risks of regular blood donors is iron deficiency. Although this has been known for decades, in most countries, iron status is currently not assessed or treated in this population. Premenopausal women are particularly affected, as they have lower iron reserves and higher daily requirements. Besides anaemia, iron deficiency may lead to fatigue and impaired cognitive and physical performanc

Adversity, psychological strengths, and well-being during collective trauma: Results from a nationally representative, probability-based U.S. sample.

OBJECTIVE: Guided by the resilience portfolio model, we examined associations between resilience profiles (defined by adversity exposure and psychological strengths) and well-being-related outcomes across 2 years during the COVID-19 pandemic. METHOD: A longitudinal, representative, probability-based sample of U.S. residents (N = 4,248) completed four surveys between March 2020 and May 2022. K-means clustering identified distinct profiles based on lifetime adversity exposure and psychological strengths, operationalized as four internal assets (i.e., positive affect, awe, cognitive reappraisal,

Challenges in opioid therapy implementation: national survey of palliative care consultation services.

BACKGROUND: Palliative care consultation services in hospitals can improve symptom control in patients with advanced illness by recommending or prescribing WHO step III opioids. However, effective treatment depends on the attending ward team which implements these therapies. While deviations from the opioid therapy recommended by the palliative care service and also treatment errors are often an issue in everyday life, there is no current data on the extent of the problem. This study explored the experiences of palliative care consultation services with the implementation of opioid recommendat

Alzheimer's disease.

Alzheimer's disease (AD) is a chronic neurodegenerative disease with well-defined pathophysiological mechanisms, mostly affecting medial temporal lobe and associative neocortical structures. Neuritic plaques and neurofibrillary tangles represent the pathological hallmarks of AD, and are respectively related to the accumulation of the amyloid-beta peptide (Aβ) in brain tissues, and to cytoskeletal changes that arise from the hyperphosphorylation of microtubule-associated Tau protein in neurons. According to the amyloid hypothesis of AD, the overproduction of Aβ is a consequence of the disruptio

Shared molecular mechanisms between colorectal cancer and cardiovascular disease drive perioperative neurological vulnerability.

BACKGROUND: Colorectal cancer (CRC) and cardiovascular disease (CVD) share overlapping molecular pathogenic axes-including chronic systemic inflammation, immune dysregulation, metabolic reprogramming, and cellular senescence-that collectively amplify perioperative neurological vulnerability in elderly patients. Postoperative delirium (POD), the most prevalent acute neuropsychiatric complication following major oncological surgery, emerges at the intersection of these shared pathogenic pathways: tumor-driven neuroinflammation, anesthesia-induced cerebral hypoperfusion, cardio-metabolic derangem

Delphi panel to obtain clinical consensus about using long-acting injectable antipsychotics to treat first-episode and early-phase schizophrenia: Treatment goals and approaches to functional recovery.

Background Schizophrenia is mostly a chronic disorder whose symptoms include psychosis, negative symptoms and cognitive dysfunction. Poor adherence is common and related relapse can impair outcomes. Long-acting injectable antipsychotics (LAIs) may promote treatment adherence and decrease the likelihood of relapse and rehospitalization. Using LAIs in first-episode psychosis (FEP) and early-phase (EP) schizophrenia patients could benefit them, yet LAIs have traditionally been reserved for chronic patients. Methods A three-step modified Delphi panel process was used to obtain expert consensus on

Diagnosis and Management of Hyponatremia: A Review.

IMPORTANCE: Hyponatremia is the most common electrolyte disorder and it affects approximately 5% of adults and 35% of hospitalized patients. Hyponatremia is defined by a serum sodium level of less than 135 mEq/L and most commonly results from water retention. Even mild hyponatremia is associated with increased hospital stay and mortality. OBSERVATIONS: Symptoms and signs of hyponatremia range from mild and nonspecific (such as weakness or nausea) to severe and life-threatening (such as seizures or coma). Symptom severity depends on the rapidity of development, duration, and severity of hypona

Planned Antihypertensive Deprescribing in Long-Term Care Residents: A Systematic Review of Randomized Evidence.

BACKGROUND: Evidence supporting antihypertensive treatment in older adults is derived largely from populations that differ from long-term care residents, in whom frailty, dementia, multimorbidity, polypharmacy, and competing clinical priorities may alter the balance between preventive benefit and treatment burden. OBJECTIVES: The aim was to synthesize randomized evidence on planned antihypertensive deprescribing in institutional long-term care and determine whether clinically and methodologically comparable effects support a common treatment-effect estimate. METHODS: Randomized or cluster-ra

Preoperative nutritional status, frailty, and postoperative delirium in patients undergoing pancreatic cancer surgery: a prospective observational study.

BACKGROUND: Postoperative delirium (POD) is a common neuropsychiatric complication after major abdominal surgery and is associated with adverse clinical outcomes. Patients undergoing pancreatic cancer surgery are particularly vulnerable to nutritional impairment and physiological decline, both of which may increase susceptibility to POD. Frailty reflects reduced physiological reserve and may be associated with both poor nutritional status and postoperative cognitive vulnerability. However, whether frailty helps explain the association between preoperative nutritional status and POD remains unc
