Topical corticosteroid phobia in parents of pediatric patients with atopic dermatitis: a multicentre survey.

BACKGROUND: Families of children affected with atopic dermatitis (AD) often report fear and anxiety regarding treatment with topical corticosteroids (TCS), which may lead to reduced compliance. The objective of our study was to measure, through a standardized questionnaire, fear of TCS in families of pediatric patients with AD and to identify items associated with fear. METHODS: Families of pediatric patients with AD were enrolled in 9 Italian centers of pediatric dermatology. Enrolled parents were invited to fill in a questionnaire including questions on sociodemographic and clinical charact

Adverse events of Dupilumab in adults with moderate-to-severe atopic dermatitis: A meta-analysis.

BACKGROUND: Dupilumab, a fully human monoclonal antibody against interleukin-4 receptor alpha, inhibits the signals of interleukin-4 and interleukin-13, and has also shown significant efficacy in patients with moderate-to-severe atopic dermatitis (AD), while the effect of it on adverse events remains controversial. OBJECTIVE: To assess the influence of dupilumab on adverse events in adults with moderate-to-severe AD. METHOD: Randomised controlled trials (RCTs) that compared dupilumab with a placebo for patients with moderate-to-severe AD were searched in the MEDLINE, EMBASE, Web of Science a

The validity of register data to identify children with atopic dermatitis, asthma or allergic rhinoconjunctivitis.

BACKGROUND: The incidence of atopic dermatitis, wheezing, asthma and allergic rhinoconjunctivitis has been increasing. Register-based studies are essential for research in subpopulations with specific diseases and facilitate epidemiological studies to identify causes and evaluate interventions. Algorithms have been developed to identify children with atopic dermatitis, asthma or allergic rhinoconjunctivitis using register information on disease-specific dispensed prescribed medication and hospital contacts, but the validity of the algorithms has not been evaluated. This study validated the alg

Melatonin Supplementation for Children With Atopic Dermatitis and Sleep Disturbance: A Randomized Clinical Trial.

IMPORTANCE: Sleep disturbance is common in children with atopic dermatitis (AD), but effective clinical management for this problem is lacking. Reduced levels of nocturnal melatonin were found to be associated with sleep disturbance and increased disease severity in children with AD. Melatonin also has sleep-inducing and anti-inflammatory properties and therefore might be useful for the management of AD. OBJECTIVE: To evaluate the effectiveness of melatonin supplementation for improving the sleep disturbance and severity of disease in children with AD. DESIGN, SETTING, AND PARTICIPANTS: This

Contact sensitization in Dutch children and adolescents with and without atopic dermatitis - a retrospective analysis.

BACKGROUND: Allergic contact dermatitis is known to occur in children with and without atopic dermatitis, but more data are needed on contact sensitization profiles in these two groups. OBJECTIVES: To identify frequent allergens in children with and without atopic dermatitis suspected of having allergic contact dermatitis. METHODS: A retrospective analysis of children aged 0-17 years patch tested between 1996 and 2013 was performed. RESULTS: Of all 1012 children tested because of suspected contact dermatitis, 46% developed one or more positive reactions, the proportions for children with (n

Circulating CLA+ T cells in atopic dermatitis and their possible role as peripheral biomarkers.

Cutaneous lymphocyte-associated antigen (CLA+ ) T cells are specialized for skin homing and represent the main T-cell population in atopic dermatitis (AD) lesions. CLA+ is expressed on the surface of circulating CD45RO+ memory T cells and most skin-infiltrating T cells. Mechanistic studies and thus treatment advancements are limited by the need of large number of skin biopsies. Circulating CLA+ T cells may be a reliable surrogate marker of the inflammatory events occurring in the skin, and thus, the evaluation of CLA+ T cells in the blood may eliminate the need for skin biopsies. Preliminary w

The patterns and clinical relevance of contact allergen sensitization in a pediatric population with atopic dermatitis.

BACKGROUND/AIM: Data about contact allergen sensitization (CAS) in children with atopic dermatitis (AD) are limited. The purpose of this study was to identify the frequency and patterns of CAS in children with AD by using a ready-to-use patch test system. MATERIALS AND METHODS: After receiving the history of CAS in the patients, the severity of AD and IgE-mediated allergen sensitization were determined. RESULTS: Of 134 children with AD, 33.8% (n = 45) had at least 1 positive reaction. The most frequent positive reaction was to nickel sulfate (NS) (37.8%, 17/45), followed by methylchloroisoth

DIAGNOSIS AND PALLIATIVE MANAGEMENT OF ATOPIC DERMATITIS IN A MALAYAN FLYING FOX (PTEROPUS VAMPYRUS).

A 7-yr-old male Malayan flying fox (Pteropus vampyrus) presented with chronic, recurrent bilateral conjunctivitis and palpebral chemosis. Variable, temporary response to topical ocular antibiotic-corticosteroid therapy occurred. Histopathology of a conjunctival biopsy was consistent with allergic conjunctivitis. The bat was treated systemically with antihistamines and topically with an ocular antibiotic-corticosteroid combination for acute episodes of conjunctivitis. Pruritus, severe alopecia of the head and neck, and moist dermatitis later developed in conjunction with bilateral conjunctiviti

Patterns of aeroallergen sensitization predicting risk for asthma in preschool children with atopic dermatitis.

OBJECTIVE: Atopic dermatitis (AD) is a chronic inflammatory skin disorder mostly affecting young children. Although several studies aimed to identify the risk factors for asthma in AD children, many aspects still need to be clarified. The aim of this study was to investigate the possible risk factors for asthma at school age in 99 children with early-onset and IgE-mediated AD. METHODS: All children performed clinical evaluation and total and specific IgE assay for a panel of inhalant and food allergens at two different times (t1 and t2) during preschool, and asthma diagnosis was assessed at o

IL-33 and ST2 in atopic dermatitis: expression profiles and modulation by triggering factors.

In the acute phase of atopic dermatitis (AD), T-helper type 2 (Th2) cytokines characterize the inflammatory response in the skin. IL-33 is a new tissue-derived cytokine, which is mainly expressed by cells of barrier tissues, and is known to activate Th2 lymphocytes, mast cells, and eosinophils. IL-33 signals through a receptor complex consisting of IL-33-specific receptor ST2 and a co-receptor IL-1RAcP. As IL-33 is known to promote Th2-type immunity, we examined expression profiles of IL-33 and its receptor components in human AD skin, in the murine model of AD, and in various cell models. We

Reversal of atopic dermatitis with narrow-band UVB phototherapy and biomarkers for therapeutic response.

BACKGROUND: Atopic dermatitis (AD) is a common inflammatory skin disease exhibiting a predominantly T(H)2/"T22" immune activation and a defective epidermal barrier. Narrow-band UVB (NB-UVB) is considered an efficient treatment for moderate-to-severe AD. In patients with psoriasis, NB-UVB has been found to suppress T(H)1/T(H)17 polarization, with subsequent reversal of epidermal hyperplasia. The immunomodulatory effects of this treatment are largely unknown in patients with AD. OBJECTIVE: We sought to evaluate the effects of NB-UVB on immune and barrier abnormalities in patients with AD, aimin

Effect of a new synbiotic mixture on atopic dermatitis in children: a randomized-controlled trial.

OBJECTIVE: Atopic dermatitis (AD) is the most common chronic relapsing skin disease seen in infancy and childhood. The intestinal microbiota play an important role in immune development and may play a role in the development of allergic disorders. Manipulation of the intestinal microbiota by synbiotics may therefore offer an approach to the prevention or treatment of AD and allergic diseases. We studied the clinical and immunologic effects of a new symbiotic (a mixture of seven probiotic strains of bacteria and Fructooligosaccharide) in infants and children with AD. METHODS: In a randomized,
