Treatment typically involves open reduction, often with the use of hinged external fixators. The role of triceps lengthening or primary collateral ligament reconstruction remains a topic of debate.

Polytrauma cases in mass disasters present several challenges to the orthopaedic surgeon. Delayed referral, multisystem involvement and the requirement to manage coexisting injuries by interhospital transfer often make infection an inevitable risk. 28 patients with polytrauma were studied after being referred after being recovered from the debris of their homes in the Kashmir earthquake. All patients were referred more than 24h after sustaining their injuries. The lower limb fractures were fixed by external fixators in all these cases before interhospital transfer for the management of their c…

Fractures of the distal radius often occur as a result of low-energy trauma and are frequently seen in osteoporotic patients. Many biomechanical studies and clinical case series have been carried out to investigate the effects on the fractured bones of different fixation methods such as cast immobilization, percutaneous pins, external fixators, or open reduction with internal fixation. In addition, the use of different bone cements as stand-alone solutions or as an adjunct to the aforementioned fixation methods is described as one possible way of increasing the performance of the fixation by r…

Hemorrhage control in patients with pelvic ring disruptions remains problematic. To decrease bleeding, efforts have been made to acutely reduce and stabilize the pelvis. The goals of rapid pelvic reduction and stabilization are restoration of normal pelvic volume, protection of the early clot, and improved patient comfort. The use of military antishock trousers, hip spica casts, external fixators, antishock pelvic clamps, early open reduction/internal fixation, and open packing have been reported in the literature. A simple temporary technique of reduction is reported using a circumferential b…

This article focuses on the surgical indications and techniques of antibiotic nailing of the femur. Spanning external fixation is an essential component of damage control orthopaedics. Fractures of the femur with significant soft tissue injury also often require prolonged external fixation, which increases the risk of infection. Antibiotic nails can sterilize the medullary canal contaminated by external fixator pins and prophylax against nosocomial infection, while the pin tracts heal to prepare the canal environment for a future staged metal nailing. Antibiotic nails may also be used as a bri

Hemorrhage is a major cause of mortality in pelvic fractures. Bleeding can be controlled in hypotensive patients by direct ligation, angiographic embolization, pelvic packing, and acute external fixation. Acute application of an external fixator can reduce pelvic volume and reduce bleeding fractures to effect tamponade. This therapy assumes that the pelvis represents a closed space, which clearly is not true anatomically. However, the premise may hold functionally. This study explored the relationship between pressure and volume in the intact and disrupted pelvic retroperitoneum. In cadaveric

Distal femur fractures are challenging fractures to treat, with nonunion rates as high as 22%. Precontoured locking plates have mitigated some earlier causes of failure, while introducing new challenges. The recognition of troublesome injury patterns and appropriate preoperative planning can avoid common pitfalls. Adjunctive techniques, including the use of a radiolucent triangle, an external fixator, unicortical plates, and crossing K-wires, can assist with fracture reduction and maintenance. It is important to understand the common pitfalls involved with distal femur plating and to consider

The technique of intrafocal pinning was originally described by Kapandji. This technique, along with the addition of supplemental external or internal bridging fixation, can be performed quickly with minimal soft tissue damage. This technique is indicated for the treatment of unstable extra-articular or minimally displaced intra-articular fractures of the distal radius. Studies demonstrate that young patients with displaced extra-articular distal radius fractures and minimal comminution can be treated by percutaneous intrafocal pin fixation alone, whereas, patients over 55 years of age and you

This paper presents the outcome of a data review of patients treated with the IOS external fixation system at the Royal Stoke University Hospital: a fixation designed to meet four requirements for external fixation proposed in this paper. Demographic data and outcome were collected and assessed. From 69 initial patients, 64 patients (55 males and 9 females) had an average age of 35.9 years. The mean time to union was 127 days. There were no incidences of malunion, or refracture post fixation removal attributable to the treatment method. In addition, in this cohort, there was no incidence of pi

BACKGROUND: Intramedullary rods and external fixators are reported to be more widely used than plating in osteosynthesis of femoral shaft fractures in children. In our institution plating is the routine, and we are reporting our experience in a retrospective study.
METHODS: Two groups of patients were treated by AO plating for femoral shaft fractures. The first included 36 polytraumatized children and the second involved 10 cases of old malunited fractures. Average follow-up for the patients was 5 years.
RESULTS: All children achieved union after one operation except one. There were no major…

Records of 52 polytraumatized patients with closed pelvic fractures were reviewed retrospectively to determine if spica cast application decreased mortality due to exsanguination. Twelve patients had supplemental stabilization with external fixators or internal fixation of the pelvis. Their results indicated that the spica cast is effective in decreasing or controlling bleeding since no patient died of exsanguination. Several patients did die, however, but the cause of death in these patients was usually adult respiratory distress syndrome with or without other sepsis. Injury severity score, a…

Injury to the popliteal artery is the most common cause of amputation in injured extremities. A six-year experience (1974-1979) at Cook Coun … onstruction, repair of concomitant popliteal vein injuries, initial bone fixation of fractures with external fixators, and the performance of intraoperative arteriography were all important steps taken to ensure a 100% limb salvage.

Distal radius fractures are common fractures that can cause significant disability. As techniques and implants have improved, better results can be expected from internal and external fixation of complex wrist fractures. With meticulous technique, articular alignment can be secured and small articular fragments can be replaced in anatomic locations through limited open or arthroscopic techniques. Bone graft should be used liberally in comminuted articular fractures. Such procedures are demanding, however. Familiarity with the techniques described in this article will enhance the surgeon's abil

This randomized controlled trial is aimed at evaluating whether external fixation of the urinary catheter to the body surface represents a low-value nursing intervention for patients undergoing transurethral resection of the prostate (TURP). A total of 208 patients who received indwelling urinary catheters after TURP in a tertiary hospital in Qingdao, China between June 2024 and May 2025 were randomly assigned to one of two groups: a nonexternal fixation group (n = 103) and an external body surface fixation group (n = 105). A between-group comparison of outcomes included postoperative hematuri

The aim of distal humerus fracture treatment is articular surface reduction and stable fixation for early mobilisation and rehabilitation. This is usually performed by open reduction and internal fixation with plates. In the elderly osteoporotic patient this treatment is difficult to achieve due to fixation failure in fragile bone. We present our experience with treatment by closed reduction and external fixation with a non-bridging ring fixator in distal humerus fractures in elderly patients. There were ten females, aged 70-89 (average 78.4). Fracture types (AO/ASIF) included three supracondy
