[Carpal tunnel compression syndrome and third finger in trigger secondary to an encapsulated fibrolipoma of the wrist: a case report].

INTRODUCTION: the association of carpal tunnel syndrome with stenosing tenosynovitis of the hand is very rare, even more, if it is generated by a fibrolipoma at the carpal tunnel. The imaging study useful to detect this type of hand injuries are X-ray screening for carpal tunnel, computed tomography and magnetic resonance imaging. But these are not commonly used for the study of protocolized carpal tunnel syndrome and much less trigger finger. OBJECTIVE: the aim of this work is to report a case of a middle-aged female with carpal tunnel syndrome characteristic symptoms, associated with the th

Distal interphalangeal Joint Arthrodesis using only Kirschner Wires in small distal Phalanges.

PURPOSE: To report the clinical and radiographic results of arthrodesis of relatively small-sized distal interphalangeal joints (DIPJs) using only K-wire fixation. PATIENTS AND METHODS: Between January 2000 and December 2018 28 arthrodesis in 21 patients (9 males and 12 females with an average age of 52.1 years) with relatively small-sized DIPJs were performed using only K-wires. Data on patient's characteristics, such as age, sex, affected finger, and the number and size of the used k-wires were collected from the medical database. The narrowest diameter of the cortex and medulla of the dist

[High resolution ultrasound diagnosis of the annular tendon system of the hand].

High-resolution ultrasonography has gained wide acceptance in the evaluation of tendons and tendon sheaths of the finger flexors. The annular pulleys are depicted as a hypoechoic band surrounding the flexor tendons. In patients with trigger finger or polyarthritis, the tendons as well as the pulleys are of interest to the investigator. Ultrasonography allows direct or indirect diagnosis of partial and total disruptions of the pulleys. Loss of (parts of) the pulleys can be seen as a direct sign and bowstringing of the tendons during forced flexion as an indirect sign. We provide a review of the

[Lipoma of the fingers].

Benign tumours of the hand are a common disease, usually corresponding to nodes. Other etiologies are less common. Lipomas of the hand are a rare feature, especially in the fingers. We present four cases of digital lipomas and a review of surgery of lipomas over the past ten years, and of the literature.

Proximal interphalangeal joint injuries and their initial treatment.

Many PIP joint injuries which appear to be innocuous are actually very severe injuries to this small hinged joint and carry a high percentage of disability for the hand as well as the individual digit. A flexion contracture of the PIP joint of one digit may significantly reduce the functional capacity of the entire hand. To prevent such physical impairment, the type of PIP joint injury must be defined and the proper treatment for the particular injury must be initiated. Second, it is paramount that prolonged immobilization of this joint be prevented. It has been found, through clinical experie

Thenar Flap: A Workhorse Flap for Fingertip Injuries.

INTRODUCTION: Fingertip injuries are the most common type of upper limb trauma, frequently involving volar oblique or transverse defects with exposed bone or tendon. The thenar flap, first described in 1926, remains a reliable and aesthetically favorable option for reconstruction, particularly for index, middle, and ring finger injuries. Despite concerns about joint stiffness and donor site morbidity, refinements in surgical technique have improved outcomes. METHODS: A retrospective study was conducted at a tertiary care center from January 2023 to January 2025. Twenty-five patients with trau

[Radiographic study of the hand in 50 cases of primary articular chondrocalcinosis. Comparison with a control series of 100 subjects].

The authors analysed radiograms of the hands of 50 patients suffering from primary articular chondrocalcinosis (ACC) and compared them with those of 100 control subjects of similar sex and age. There were 40 women and 10 men, with an average age of 69.6 years (plus or minus 9.82). A chalky incrustation of the semilunar pyramidal interspace was observed in 40 percent of cases. The frequency of this incrustation was next in order of importance to incrustation of the triangular ligament which occurred in 52 percent of cases. The difference from the controls was highly significant (p less than 0.0

Single-Portal Antegrade Endoscopic Trigger Finger Release: Cadaveric and Clinical Outcomes.

BACKGROUND: This study aimed to examine the relationship between anatomical surface landmarks in fresh frozen cadavers as related to in vivo endoscopic trigger finger release (ETFR) and present clinical outcomes after a single-portal antegrade ETFR technique. METHODS: Endoscopic trigger finger release was performed on 40 cadaveric digits. Each digit was dissected and the following measurements were recorded: distance from palmar digital crease and A1 pulley, length of the A1 pulley, percentage of A1 pulley released, and injury to vulnerable anatomy. A retrospective chart review was performed

[Carpometacarpal fractures and fracture dislocations of rays 2-5].

BACKGROUND: Fractures and fracture dislocations of carpometacarpal joints 2-5 may be easily overlooked. This can be explained by often subtl … and radial or ulnar location of the injury. Following operative therapy, early mobilization of all finger joints should be performed.

Eosinophilic fasciitis in a child mimicking a myopathy.

A 14-year-old boy was suspected of having a myopathy with joint contractures. He presented with progressive painless joint contractures of his right wrist and fingers, and reduced muscle strength of his right arm, without obvious skin changes. Laboratory investigation showed a normal CK, hypergammaglobulinemia and eosinophilia. Ultrasonography revealed thickened fasciae in the forearm. A full thickness biopsy confirmed the diagnosis of eosinophilic fasciitis. The contractures diminished quickly with prednisolone and methotrexate. The ultrasound scans 2 years after diagnosis showed improvement,

Compression of Intercalary Bone Graft Used in Fusion of the Proximal Interphalangeal Joint after Failed Arthroplasty.

Fusion of the proximal interphalangeal joint (PIPJ) after failed arthroplasty may be challenging in view of bone loss at the ends of the proximal phalanx and the middle phalanx. This might require the use of a structural bone graft to fill the defect and avoid shortening of the digit. Although several methods have been described for primary PIPJ fusion, none of these methods addresses bone loss at this joint. We use a tricortical iliac crest bone graft fashioned to provide the required angulation at the fusion site. A dorsal plate is used to fix the fusion mass and obliquely placed screws prov

Consequences of Untreated Pediatric Trigger Thumb: Case Report.

Pediatric trigger thumbs present a conundrum for hand surgeons. Surgery for trigger thumbs has inherent risks, such as infection, nerve injury, and the risks of anesthesia, but will reliably solve the problem. But is surgical intervention necessary? Would these cases resolve spontaneously, eventually, without intervention? If not, what are the long-term consequences of the inability to fully extend the thumb interphalangeal joint? We present a pediatric trigger thumb that was symptomatic for 22 years, with complete resolution of symptoms after surgical intervention. This report illustrates at

Reconstruction of digital deformities in rheumatoid arthritis.

Digital deformities result from rheumatoid synovitis. These deformities are easier to treat in the early stage, when the deformity is passively correctable. Treatment options become limited as the disease progresses and the deformity becomes fixed. Surgical treatment of digital deformities is last in the priority of surgical procedures for the rheumatoid hand and wrist. It is therefore important to understand the patient's needs and expectations for improvement and attempt to match them with the surgical options that can predictably improve the patient's function. A close collaboration with th

[An unusual case of localised tophaceous gout in the left index finger].

The present article describes the biopsy-based (proximal interphalangeal joint II, HE staining) diagnosis of tophaceous gout. Eosinophilic/basophilic deposit surrounded by fibrohistiocytic cells and some giant cells of foreign type could be detected on light microscopy. No birefringent material could be observed. Differential diagnosis included caseating granulomatous infectious diseases, which are typically characterized by zonally organised granulomas with central necrosis, epitheloid cell reaction, as well as Touton-type giant cells.
