Gut microbiome dysbiosis in neonatal sepsis.

Sepsis is a highly heterogeneous, life-threatening organ dysfunction primarily caused by a dysregulated immune response to counter bacterial, viral, or fungal infections, resulting in haemodynamic changes and significant morbidity and mortality across all ages. In recent times, it has become one of the foremost causes of morbidity and mortality among newborns globally. The neonates, particularly the preterm neonates, due to their immature immune systems and non-canonical microbial community acquisition in the gastrointestinal tract and other body habitats, are adversely affected compared to th

A comparison of interview data and medical records for previous medical conditions and surgery.

Although interview information is usually the sole source of data in case-control studies, the accuracy of such data is infrequently assessed. We compared interview data on selected medical conditions and surgical procedures with medical records of subjects with chronic lymphocytic leukemia. We examined agreement by type of respondent (self or surrogate), age, sex, race, and type of hospital. The strength of agreement between the two data sources (as measured by kappa statistics) was substantial kappa greater than 0.6) for splenectomy, appendectomy, asthma, and systemic lupus erythematosus; mo

Associations of surgical breast cancer treatment at regional or community hospitals with patient-reported experience measures.

PURPOSE: High-volume hospitals deliver improved outcomes for patients with breast cancer, supporting referrals for breast cancer surgical care to high-volume regional facilities. This retrospective cohort study investigated how multidisciplinary cancer care trajectories and patient care experiences varied by surgical facility classification (regional vs. community as a proxy for high- vs. low-volume) and whether effects differed by rural-residence. METHODS: We used Surveillance, Epidemiology, and End Results-Consumer Assessment of Healthcare Providers and Systems (SEER-CAHPS) data for Medicar

Doxycycline versus azithromycin as combination therapy in the empirical treatment of community-acquired pneumonia in an intensive care unit - A retrospective propensity score-matched cohort study.

INTRODUCTION: Community-acquired pneumonia constitutes a significant burden to healthcare systems. While international guidelines recommend beta-lactam plus macrolide in treating severe community-acquired pneumonia, doxycycline instead of macrolide provides antimicrobial activity to macrolide-resistant organisms that are highly prevalent in East Asia and avoids its side effects. Yet, doxycycline's immunomodulatory effect is not as well studied. We investigated the comparative efficacy of doxycycline with azithromycin in the treatment of severe community-acquired pneumonia. METHODS: From 1 Jan

"I felt so alone in the process:" Experiences of Indigenous women with maternal medical travel in the Canadian Arctic and opportunities for improvement.

PROBLEM: Indigenous women face unique challenges during pregnancy/birth-related medical travel due to systemic barriers, cultural differences, and geographic isolation. BACKGROUND: In Canada, Indigenous women frequently travel substantial distances for pregnancy care and birth. Many pregnant women in Northwest Territories (NWT) are mandated to travel to urban centres with secondary/tertiary hospitals between 36-38-weeks gestation. QUESTION/HYPOTHESIS/AIM: What are the experiences of Indigenous women in NWT regarding pregnancy and birth-related medical travel? How do cultural and community-sp

A Successful Endovascular Treatment of Massive Hematuria Caused by Aorto-Ureteric Fistula: A Report of a Rare Case.

Fistula formation between the urinary tract and the arterial system is very rare, and usually involves the ureter and the adjacent iliac vessels. Communication of the ureter with the aorta has been described a few times worldwide, and most of them had a fatal outcome. In our case, a 79-year-old man had a history of total cystectomy for malignancy and diversion of both ureters to a single site in the right hypogastrium with the left one crossing over the aorta. He was admitted elsewhere several times for intermittent hematuria, and four months ago the diagnosis of communication of the left uret

Symptomatic communicating hydrocephalus in a contemporary cohort of high grade glioma patients.

INTRODUCTION: High grade glioma (HGG) treatment has seen a paradigm shift with intensified regimes, but hospitalisation burden is partially attributed to side effects of therapy. Symptomatic communicating hydrocephalus (HC) is a potential complication. Aim of this study was to investigate its incidence, risk factors and treatment in a contemporary cohort. METHODS: We performed a retrospective review of HGG patients. Patients with symptomatic communicating HC were identified and demographic, tumour and treatment variables extracted from hospital notes. Descriptive statistics were performed and

Are there more healthcare workers in our community? A phenomenological exploration of factors influencing local hospital and primary care worker re-engagement in rural Australia.

INTRODUCTION: The local healthcare workforce is critical to rural communities, which face more complex health conditions than urban counterparts. Workforce shortages exacerbate strain on rural healthcare systems and create service access barriers. To date, retention and recruitment strategies have struggled to resolve staffing issues. This study investigated an overlooked local resource: hospital and primary healthcare workers previously employed or working at less than full-time capacity in rural Queensland, Australia. We explored barriers and enablers to increasing their working hours or ret

The acute abdomen: the role of computer-assisted diagnosis.

CAD has been used in over 20,000 patients in an Accident and Emergency Department. Structured documentation has replaced normal case notes and a patient record is created which includes investigations and management. A copy of the documentation acts as an immediate communication to the general practitioner. The information is held on a computer and is readily accessed to produce feedback and to allow audit of any number of parameters. The results show that an improvement in diagnostic accuracy and a reduction in admissions and unnecessary laparotomies continues over 15 years. Problems have occ

Verbal autopsy as a tool for identifying children dying of sickle cell disease: a validation study conducted in Kilifi district, Kenya.

BACKGROUND: Sickle cell disease (SCD) is common in many parts of sub-Saharan Africa (SSA), where it is associated with high early mortality. In the absence of newborn screening, most deaths among children with SCD go unrecognized and unrecorded. As a result, SCD does not receive the attention it deserves as a leading cause of death among children in SSA. In the current study, we explored the potential utility of verbal autopsy (VA) as a tool for attributing underlying cause of death (COD) in children to SCD. METHODS: We used the 2007 WHO Sample Vital Registration with Verbal Autopsy (SAVVY) V

Facilitators, barriers, and guidance to successful implementation of multidisciplinary transitional care interventions: A qualitative systematic review using the consolidated framework for implementation research.

BACKGROUND: Multidisciplinary transitional care interventions aim to improve the coordination and continuity of healthcare during hospitalization and after discharge for patients with complex care needs related to physical, nutritional, or psychosocial status. Implementing such interventions is complex as they involve many stakeholders across multiple settings. Numerous studies have evaluated patients', family members', and healthcare professionals' experiences with multidisciplinary transitional care interventions, which can provide insight into facilitators and barriers to their implementati

Wound care and the NHS 10-year health plan: aligning clinical innovation with system value.

The NHS Fit for the Future 10 Year Health Plan published in July 2025, represents the most ambitious structural redesign of the health service for a generation. For wound care, this creates both significant challenges and considerable opportunities. This article examines how wound care products and services can be aligned with the strategic priorities of the 10-year plan, including three systemic shifts, in addition to value-based procurement reform and drug tariff changes. These changes will affect how wound care products already listed on drug tariff are assessed through a renewal process. T

The Role of Family or Community Nurse in Dealing with Frail and Chronic Patients in Italy: A Scoping Review.

Even though Family and Community Nurses (FCNs) were introduced into the Italian healthcare system in 2000, to date, there is a substantial knowledge gap regarding the implementation of these professional figures within the Primary Care (PC) system. This scoping review aims to provide a comprehensive picture of the role of FCNs in managing older adults and the elderly with chronic conditions within the Italian PC system. A search on Medline, Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, Embase, and Scopus was conducted including studies published until 7 April 2023

Leadership for climate-resilient healthcare: insights from an ATACH technical meeting.

INTRODUCTION: Climate change is increasingly recognised as both a public health emergency and a systemic stressor on healthcare systems. While many healthcare organisations have articulated ambitious climate and sustainability commitments, progress is frequently constrained by gaps in leadership capacity, governance and decision-making frameworks. Understanding what forms of leadership are required to move healthcare systems from climate ambition to operational readiness is a critical priority. APPROACH: This article draws on insights from two global technical meetings convened by the WHO's A

How Clinical Research Can Contribute to Strengthening Health Systems in Low Resource Countries.

Clinical research on neglected tropical diseases is a challenge in low-resource countries, and the contribution of clinical and operational research to health system strengthening is poorly documented. Developing new, simple, safe, and effective treatments may improve the effectiveness of health systems, and conducting research directly in health structures may have an additional impact. This study describes the process of conducting clinical trials in the Democratic Republic of Congo (DRC) in compliance with international standards, and the role of the trials in strengthening health system fu
