[Reoperation in necrotizing acute pancreatitis: evaluation of physiopathology and surgical treatment].

We reviewed our experience of PAN cases operated for complications after a first laparotomy over the period 1992-1994. Over 29 PAN cases, 7 (24%) had been submitted to a second laparotomy or more. Total mortality rate of PAN was 10.3%, while mortality rate of relaparotomy was 14.2%. Haemorrhage and intra-abdominal sepsis were the main cause of relaparotomy (42.8% of the re-operations in both cases). Abdominal wall abscesses (14%) were treated locally; enteric or pancreatic fistulas (34%) were successful treated by drugs, such as somatostatin and octreotide, and / or by total parenteral nutriti

A case of hepatocolic fistula after percutaneous drainage for a gas-containing pyogenic liver abscess.

We describe a rare case of gas-containing pyogenic liver abscess which penetrated the adjacent colon, forming a hepatocolic fistula, after percutaneous transhepatic abscess drainage (PTAD) had been performed. To the best of our knowledge, this is the first report of hepatocolic fistula associated with a gas-forming liver abscess in a diabetic patient, with radiological and surgical confirmation of the fistula.

Enterovesical fistula following radiotherapy for gynecologic cancer.

Enterovesical fistula is a rare complication of pelvic radiotherapy. Recurrent disease is the cause of fistulization in most patients. We identified 14 patients who developed enterovesical fistula in the absence of tumor recurrence. These women were at high risk for radiation morbidity due to prior surgery, pelvic inflammatory disease, adjuvant hyperbaric oxygen, or locally high doses of radiotherapy caused by suboptimal geometry and technique. All patients underwent radiographic imaging including barium enema, intravenous pyelogram, and upper gastrointestinal study with small bowel follow-thr

[Cecovesical fistula of inflammatory origin simulating neoplasm of the cecum].

We describe a case of a caecovesical fistula in a 60-year-old woman caused by an inflammatory tumour of a probably appendicular origin which mimicked cancer of the caecum. The patient, had a history of idiopathic peripheral polineuropathy and chronic constipation. Presented urinary tract infection and fecaluria, without symptoms of appendicitis. At operation, a solid mass including the caecum and the fundus of the bladder was found, and the appendix was not identified. In view of the neoplastic appearance and the impossibility of separating the caecum from the bladder, "en bloc" right hemicole

Choledochoduodenal fistula from a penetrating duodenal ulcer. A case report.

Choledochoduodenal fistula is an uncommon complication of duodenal ulcer disease. The role of medical therapy is unclear and surgical management has been recommended for the condition. A patient who was treated with ranitidine 150 mg twice daily for 8 weeks for a giant duodenal ulcer with a choledochoduodenal fistula is reported. Both the ulcer and the fistula completely healed on medical therapy. Medical therapy may now become the treatment of choice for this condition due to the availability of potent acid-reducing agents.

Cloacal extrophy.

Extrophic lesions of the ventral abdominal wall are represented by a range of anomalies of which the most severe is cloacal extrophy. By 1986, only 190 cases were reported in the world literature and it is estimated that only 15 cases are seen in the United States of America per year. In Nigeria today, with improvement in antenatal and perinatal services, children who one or two decades ago would have been stillborns or products of miscarriages are now born alive. This paper reports a case of cloacal extrophy in a Nigerian neonate and reviews the current management of the condition.

Colouterine fistula and pyometrium treated with percutaneous drainage: a case report.

Percutaneous drainage of intraabdominal abscesses has become a routine procedure for preoperative confirmation of diagnosis and patient stabilization, and often provides definitive therapy. Diverticulitis is a frequent cause of pericolonic abscess but rarely results in colouterine fistula and pyometrium. We describe the first reported percutaneous drainage of such a case of pyometrium.

Invasive amoebiasis: an unusual presentation.

A 63 year old Asian woman who presented with three week's abdominal pain was found to have a hard right iliac fossa mass and rectal ulceration. Profuse rectal bleeding necessitated a laparotomy. An inflammatory paracaecal mass with fistulae involving appendix, small bowel, and bladder was excised with exteriorisation of the bowel ends. Microscopy showed invasive amoebae. Re-anastomosis was successfully done after treatment with metronidazole and diloxanide. There are no previous reports of a paracaecal amoeboma with fistulae to either the appendix, or urinary bladder.

Spontaneous healing of a gastro-colic fistula due to a benign gastric ulcer.

Gastro-colic fistula is an uncommon complication of a benign gastric ulcer (Laufer et al., 1976). The commonest causes of a gastro-colic fistula are carcinoma of the stomach, carcinoma of the colon and previous gastric surgery for peptic ulcer disease (Allison, 1973; Cody et al., 1975) and surgery is the treatment of choice. We report a case of gastro-colic fistula due to a benign gastric ulcer, which healed spontaneously without treatment. Only one previous case of spontaneous healing of a gastro-colic fistula has been described (Rivera, 1972) and this patient subsequently had surgery to excl

[Renocolonic fistula complicated by a renal stone: report of two cases].

A renocolonic fistula, complicated by a renal stone, was diagnosed in two women, 53 and 63 years old. The first patient had had a previous history of surgery when she underwent exploratory laparotomy for a right perinephric abscess, but the attempt to remove the right kidney was abandoned due to complications and a drainage tube was left. Six years after the first admission, an increased purulent discharge brought her in for further evaluation, fistulography then disclosed the presence of a renocolonic fistula. The second patient had a staghorn stone in the non-functioning left kidney. Retrogr

[Double pylorus and pyloroduodenal fistulas: two case reports and discussion].

Congenital double pylorus is extremely rare; only 2 cases have been found in the English and French literature. Acquired double pylorus occurs more often; up to 1982, 66 cases had been reported. During 6 810 consecutive barium meal examinations over a period of 2 years, we diagnosed the condition in 5 patients. Two came to operation, at which the diagnosis was confirmed. These 2 cases are described. Acquired double pylorus is in reality a short pyloroduodenal fistula situated next to the pylorus, usually on the lesser curvature side. In the majority of cases it results from a pyloric ulcer pen

[Biliary ileus caused by incarceration of a large gallbladder calculus in the duodenal bulb in a young man].

A case of gallstone ileus is presented, secondary to a biliary-enteric fistula with obstruction of the duodenal bulb, in a 23 year-old man. The patient underwent exploratory laparotomy because his clinical condition was deteriorating steadily. An intraoperative diagnosis of gallstone ileus was formulated; a cholecystectomy and a suture of the duodenal opening were carried out.

Duodenoureteral fistula.

A rare case of duodenoureteral fistula, manifested by pneumaturia, dysuria, and hematuria, followed a shotgun injury to the right upper abdominal quadrant five years previously. The kidney and affected ureter were removed and the duodenal opening closed successfully.
