Fracture dislocation of the proximal humerus with ipsilateral shaft fracture: a report of two cases.

We report two cases of ipsilateral proximal humerus fracture dislocation and shaft fracture in young adults. Fractures were treated with prosthetic replacement for the proximal humerus and open reduction and internal fixation for the shaft fracture due to the extent and complexity of the injury.

[Injuries in the Lisfranc joint (author's transl)].

132 injuries of the Lisfranc joint were reviewed in order to show methods of diagnosis and treatment. Injuries of this joint are rare. Almost one third are open fractures and/or dislocations, mostly comminuted lesions, associated with severe soft tissue trauma and so decreasing blood supply of the foot. Most dislocations and fractures can be treated conservatively by closed reductions and following fixation by means of Kirschner wires. In cases of minimal or moderate displacement closed reduction and plaster cast proved to be sufficient. Some cases of severely dislocated fractures or fracture

Screw fixation for certain fractures of the phalanges and metacarpals.

Twenty-one patients with fractures of the bones of the hand were treated by open reduction and internal fixation with compression screws. The firm fixation allowed range-of-motion exercises in most patients to be begun on the third postoperative day. On follow-up mild restriction in digital motion was seen in only one patient, and significant disturbance in joint function was not encountered. In spite of these advantages, the method is indicated in only a small percentage of such fractures. Usually traditional methods of open reduction and internal fixation are preferred when an open procedure

Interposition of extensor digitorum tendon in distal radius fracture mimicking a compartment syndrome.

A 14-year-old boy sustained a dorsally displaced Salter-Harris Type II fracture of the distal radius. A single attempt at closed reduction failed. After this closed reduction, the patient was unable to passively or actively extend or flex his fingers. He had increasing pain and compartment pressures were abnormally elevated. Dorsal fasciotomy was done, and during the open reduction, entrapment of the dorsal extensor retinaculum and extensor digitorium communis within the fracture was found. After removal of these structures, fracture reduction was easily accomplished and his fingers had a full

Treatment of nasofrontal ethmoidal complex fractures.

Successful reduction of severely telescoped nasofrontal ethmoidal fractures is difficult to achieve. In an effort to improve the results, a technique involving open reduction and external fixation was performed on three patients. These cases, along with ten other nasofrontal ethmoidal fractures treated between 1971 and 1980, are presented, with special emphasis in methods of reduction, stabilization, and results. The method of open reduction using an osteoplastic frontal sinus flap with external fixation gave the best overall results.

Nonunion of long bone fractures in children: a review of 30 cases.

Nonunions after diaphyseal fractures of long bones in children are rare. Thirty diaphyseal nonunions in 30 children are reported. The sites of nonunion were tibia (15), femur (5), ulna (4), humerus (3), radius (2), and fibula (1). Nonunions in children tend to occur after high-energy trauma, particularly when the fracture is compound, there is soft tissue loss, and infection develops. Open reduction and internal fixation may contribute to nonunion, particularly when the fixation is inadequate or holds the fracture apart. Repeated manipulation of the fracture after open reduction may also contr

Decision-Making and Management of Proximal Humerus Nonunions.

Proximal humerus fracture nonunions are heterogenous group of posttraumatic sequelae in both the operatively and nonoperatively treated proximal humerus fracture. The management of these fractures is largely based on the residual morphology. Understanding the relationship of the nonunited and malunited fragments, anatomic location of the fracture, and viability of the residual bone stock will allow for better surgical planning. Patient optimization with nonoperative care, open reduction internal fixation, intramedullary nailing, and shoulder arthroplasty, all have a role in the treatment of pr

Traumatic Fifth Toe Proximal Interphalangeal Joint Dislocation in a Pediatric Patient: Management with Open Reduction Internal Fixation and Kirschner Wire Fixation.

Traumatic dislocations of the proximal interphalangeal joint (PIPJ) of the lesser toes are rare injuries in the pediatric population, with limited literature available to guide management decisions. We report the case of a 5-year-old male who presented with irreducible dorsolateral dislocation of the right fifth toe PIPJ following trauma during play activities. Clinical examination revealed swelling, tenderness, and restricted movement of the affected digit. Plain radiographs confirmed the dislocation without associated fractures. Initial attempts at closed reduction under procedural sedation

Primary arthrodesis versus open reduction internal fixation for acute Lisfranc injuries: a systematic review and meta-analysis.

INTRODUCTION: The presence of a Lisfranc injury alone is considered a surgical indication in most patients. Indications for primary arthrodesis (PA) versus open reduction internal fixation (ORIF), however, is a topic of debate among surgeons. Conflicting data exists as to which treatment modality leads to improved patient-reported outcome measures (PROMs), reoperations, and complications. METHODS: Databases queried included PubMed, OVID Medline, Embase, SCOPUS, Cochrane Central Register of Clinical Trials, and clinicaltrials.gov from their dates of inception to 3/21/2024. Use of a Suture Button Device to Correct Syndesmotic Malreduction Caused by a Trans-Syndesmotic Screw.

Ankle syndesmosis malreduction is a common problem that leads to significantly worse functional outcomes. This report describes the replacement of a rigid syndesmotic screw with a flexible suture button fixation device to correct fibular translation and tibiotalar malalignment related to screw contact with other tibial implants. Suture button devices, although more expensive, can result in less postoperative malreduction and lead to better functional outcomes. As seen in these cases, careful attention should be given to syndesmotic or tibiotalar reduction after screw placement around other tib

Chronic Ulnohumeral Subluxation After Surgical Treatment of Lateral Condylar Fracture Dislocation of the Elbow in a Child: A Case Report.

CASE: A 10-year-old boy presented with chronic ulnohumeral subluxation 6 months after an open reduction of a Milch type II lateral condyle fracture subluxation performed at another hospital. The patient had persistent elbow pain and limited range of motion (ROM) between 20° and 50°. After the open reduction for chronic ulnohumeral subluxation and corrective osteotomy for articular surface reconstruction, he had an uneventful recovery with improved ROM of the affected elbow and no pain. CONCLUSIONS: Intra-articular corrective osteotomy for articular surface reconstruction may be feasible to im

Complications of Nasal Bone Fractures.

The aim of this study was to perform a systematic review of the treatment of nasal bone fractures. The search terms ("nasal bone fracture" AND complication) and ("nasal bone fracture" AND [anosmia OR olfaction OR olfactory nerve OR smell]) and (anosmia AND ["nasal preparation" OR "nasal antiseptics"]) were used to search PubMed and SCOPUS. Of the 500 titles, 40 full papers were reviewed. One paper was excluded, and 3 mined papers were added. Ultimately, 12 papers were analyzed. The overall deformity rate was 10.4% ± 4.8%. No significant differences were found between patients who underwent clo

Proximal femoral fractures in children.

Proximal femoral fractures in children can be divided into three groups: femoral neck fractures, apophyseal trochanteric separations and subtrochanteric fractures. Femoral neck fractures are extremely rare. They are classified according to Delbet and Colonna into four types. The more lateral is the line of the fracture, the better is the prognosis, and the less sequelae occur. The treatment of femoral neck fractures in children is in nearly all cases surgical, only in some pertrochanteric and in the minority of non-complete basicervical fractures conservative treatment can be considered. The s

Fractures of the distal clavicle: a case for fixation.

We present a series of 43 Neer type 2 fractures of the distal one-third of the clavicle. These displaced fractures are associated with a high incidence of non-union. In those patients treated non-operatively there was a higher incidence of local complications, residual shoulder dysfunction and non-union than in those treated by open reduction and internal fixation. We recommend that type 2 fractures should be treated by open reduction and internal fixation.

Operative management of Hoffa fractures--a prospective review of 18 patients.

OBJECTIVES: Hoffa fractures represent coronal fractures of the femoral condyles. Isolated Hoffa fractures are rare and current management of these fractures is based on a few descriptions in literature. The goal of the study was to prospectively analyse the clinico-radiological and functional outcome following open surgical treatment. PATIENTS AND METHODS: A total of 18 isolated Hoffa fractures were identified during the study period spanning 29 months. All fractures were treated by open reduction through a medial parapatellar approach. Internal fixation was performed with screws in the anter

[Research progress in the treatment of calcaneal fractures].

Calcaneal fracture is a kind of common injury of foot. Calcaneal fracture could be treated with conservative therapy, open reduction and internal fixation (ORIF), minimal invasive poking reduction and Kirschner wire fixation, arthrodesis or external fixation and other treatment methods. Open reduction and internal fixation as the main treatment method of calcaneus fracture, especially for intra-articular calcaneal fractures, as Sanders type II and III fractures, could gain satisfactory effect. But whether bone grafting should be taken is controversial in Sanders type IV fractures, because soft

Cross-Sectional Study to Assess the Functional Outcome of Neglected Bimalleolar Fracture.

INTRODUCTION: Ankle fracture is the most common intra-articular fracture of a weight-bearing joint and accounts for 9% of all fractures. Ankle fractures are classified into 3 subgroups: unimalleolar, bimalleolar, and trimalleolar fractures. Accurate reduction and stable internal fixation is necessary in bimalleolar fractures; otherwise, it may lead to posttraumatic painful restriction of movements or osteoarthritis. The purpose of this study is to assess the functional outcome and results of treatment of neglected bimalleolar fracture. MATERIALS AND METHODS: Seventeen neglected bimalleolar fr

Fractures of the base of the thumb metacarpal.

The thumb trapeziometacarpal joint is a saddle joint that is subject to high compressive forces during prehensile hand function. Fractures to the base of the thumb metacarpal occur commonly following axial load to a partially flexed thumb. Although reduction is easily performed, severe deforming forces act to displace the fractures into a varus and shortened position. Most extra-articular fractures can be treated with closed reduction and cast immobilization. Angulation up to 30 degrees can be tolerated because of the substantial compensatory motion at the thumb carpometacarpal joint. In Benne

A case of traumatic spino-pelvic dissociation.

A case of traumatic spino-pelvic dissociation is reported. The unusual fracture pattern (grade III-B open left sacroiliac joint fracture dislocation, Denis type I longitudinal right sacral fracture, transverse sacral fracture at the S3-4 level, and a left comminuted pelvic wing fracture) led to a complete dislocation of the spine from the pelvis, transection of the left S1 and S2 nerve roots, and intrusion of the cranial portion of the spine into the pelvis. The patient underwent open reduction internal fixation of the spine and the pelvis in combination with general surgical management. Two y

Blood flow changes to the femoral head after acetabular fracture or dislocation in the acute injury and perioperative periods.

OBJECTIVES: Acute blood flow to the femoral head has been postulated to be affected negatively by traumatic acetabular fracture or dislocation. To the best of our knowledge, a prospective study that has examined acute changes in blood flow to the femoral head with respect to the timing of reduction and the effect of open reduction and internal fixation after acetabular fracture or dislocations has not been performed. DESIGN AND SETTING: From June 1994 to February 1996, fifty-four consecutive patients with hip dislocations with or without fractures of the acetabulum were entered into this inve
