Successful Pavlik treatment in late-diagnosed developmental dysplasia of the hip.

PURPOSE: The objective of this study was to evaluate success and complication rates of the Pavlik harness in late-diagnosed hip dislocation (developmental dysplasia of the hip, DDH). We also set out to assess the additional value of an abduction brace for three to six months, when successful reduction using the Pavlik harness was achieved. METHODS: We included 31 patients (31 hips, 28 female, right/left = 4/27) with late-diagnosed hip dislocation treated with a Pavlik harness between 1995 and 2008. The average age at the start treatment was 27 weeks (21-57). None were lost to follow-up; the m

Cosmetic and cognitive outcomes of positional plagiocephaly treatment.

PURPOSE: Positional plagiocephaly is an acquired deformation of an intrinsically normal infant skull by sustained or excessive extrinsic forces. Non-surgical techniques include counter-positioning, supervised prone time and orthotic molding for more refractory cases. Long-term effects of positional plagiocephaly on development remain undefined, and this study evaluated cosmetic and cognitive outcomes of plagiocephaly management. METHOD: Surveys were administered to parents of patients treated for positional plagiocephaly through the Children's Hospital of Eastern Ontario. Categorical response

Splint: the efficacy of orthotic management in rest to prevent equinus in children with cerebral palsy, a randomised controlled trial.

BACKGROUND: Range of motion deficits of the lower extremity occur in about the half of the children with spastic cerebral palsy (CP). Over time, these impairments can cause joint deformities and deviations in the children's gait pattern, leading to limitations in moblity. Preventing a loss of range of motion is important in order to reduce secondary activity limitations and joint deformities. Sustained muscle stretch, imposed by orthotic management in rest, might be an effective method of preventing a decrease in range of motion. However, no controlled study has been performed. METHODS: A sin

Musculoskeletal conditions of the foot and ankle: assessments and treatment options.

Musculoskeletal conditions of the foot and ankle are an important public health challenge due to their increasing incidence combined with their substantial negative impact on patients' quality of life. Non-pharmacological treatments serve as the first line of treatment and are frequently used for patients with musculoskeletal conditions of the foot and ankle. This review provides a summary of the assessments and non-invasive treatment options based upon available evidence. Recent studies show that individuals with foot and ankle pain have multiple co-existing impairments in alignment, motion,

Effects of wrist-extension orthosis on shoulder and scapular muscle activities during simulated assembly tasks.

The purpose of this study was to observe changes in electromyographic activity appeared in shoulder and scapular muscles when one wears no wrist extension orthosis, short wrist extension orthosis and long wrist extension orthosis when performing two model assembling operations. The study subjects were 16 adults who use right hand and did not have any functional disorder on their wrist or the upper limbs without any orthopedic surgery. The study subjects were asked to do the two model assembling operations without wrist extension orthosis, with a short wrist extension orthosis and with a long w

Design and development of anthropometric device for the standardization of sizes of knee-ankle-foot orthoses.

An anthropometric device (AD) was designed and developed to collect data on foot and knee of locomotor disabled people. The aim was to standardize the sizes of knee-ankle-foot orthoses (KAFOs) in a standard modular form so that they can be mass produced to cater for fitting to a large number of locomotor disabled people. The anthropometric data collected on large numbers of locomotor disabled people were processed, with the help of a computer programme, to arrive at standard sizes for three modules, i.e. a foot plate (seven sizes), knee pieces (six sizes) and a lateral upright in a universal s

Triathlon participation for the physically challenged athlete: medical considerations.

An overview of the evolution of challenged athlete participation in sports is presented, and specifically as it relates to triathlon participation. Currently there is little literature relative to sports participation for the physically challenged athlete. There is credibility for sports participation for the physically challenged relative to physical, mental, and social issues. Positive aspects of competitive sports for the challenged athlete are apparent to any health professional who has had the opportunity to witness first hand the growth in self-confidence, enhanced self-image, and all ar

Long-term outcome of infants with positional occipital plagiocephaly.

BACKGROUND: Despite much interest in positional (deformational) plagiocephaly, the natural history is unclear. The purpose of this study was to determine cosmetic and developmental outcomes at a minimum of 5 years of age in children diagnosed in infancy with positional plagiocephaly (PP) and the impact of cranial orthotic use. METHODS: A questionnaire survey was sent to parents of children diagnosed with PP in infancy and now aged more than 5 years. A retrospective review of the child's clinic chart was performed of consenting families, and prospective follow-up was done when families agreed

Effectiveness of nonpharmacological and nonsurgical interventions for patients with rheumatoid arthritis: an overview of systematic reviews.

Conclusions based on systematic reviews of randomized controlled trials are considered to provide the highest level of evidence about the effectiveness of an intervention. This overview summarizes the available evidence from systematic reviews on the effects of nonpharmacological and nonsurgical interventions for rheumatoid arthritis (RA). Systematic reviews of studies of patients with RA (aged >18 years) published between 2000 and 2007 were identified by comprehensive literature searches. Methodological quality was independently assessed by 2 authors, and the quality of evidence was summarize

Stage I and II posterior tibial tendon dysfunction treated by a structured nonoperative management protocol: an orthosis and exercise program.

BACKGROUND: Posterior tibial tendon dysfunction (PTTD) is a relatively common problem of middle-aged adults that usually is treated operatively. The purpose of this study was to identify strength deficits with early stage PTTD and to assess the efficacy of a focused nonoperative treatment protocol. METHODS: Forty-seven consecutive patients with stage I or II posterior tibial tendon dysfunction were treated by a structured nonoperative protocol. Criteria for inclusion were the presence of a palpable and painful posterior tibial tendon, with or without swelling and 2) movement of the tendon wit

[Diagnosis and treatment of disorders of the foot in children].

During the skeletal development of the axis and foot of the child physiological specialties can occur. Those are thick fatty sole, increased valgus in the ankle joint, tibial varus as a toddler with first varus, later on valgus in the knee, increased laxity of the ligaments, increased anteversion of the femoral neck with "kneeing in" and "toeing in", and with knees first in flexion, later on hyperextension. The knowledge of those specialties is important to be able to diagnose and treat deformities of the foot during childhood. Included are club foot, flexible flatfoot, vertical talus, pes add

A prospective study of the effect of foot orthoses composition and fabrication on comfort and the incidence of overuse injuries.

BACKGROUND: Foot orthoses are widely prescribed both to treat existing pathological conditions and to prevent overuse injuries, but little is known about the effect of their material composition and fabrication technique on patient comfort and the incidence of overuse injuries. MATERIALS AND METHODS: The acceptance rates and comfort scores of soft custom, soft prefabricated, semirigid biomechanical, and semirigid prefabricated orthoses and their effect on the incidence of stress fractures, ankle sprains, and foot problems were studied in a prospective, randomized, single-blinded clinical tria

Controversies in cervical discectomy and fusion: practice patterns among Canadian surgeons.

OBJECTIVE: Optimal fusion technique and peri-operative management of patients undergoing anterior cervical discectomy (ACD) is unclear. We document current practice patterns among Canadian spinal surgeons regarding the surgical management of single level degenerative cervical spondylosis. METHODS: We conducted a web-based survey of neurosurgeons and spinal orthopedic surgeons in Canada. We asked questions pertaining to the management of single level cervical degenerative disc disease causing radiculopathy and/or myelopathy, including frequency of fusion following single-level discectomy, pref

Fracture prevalence in Duchenne muscular dystrophy.

The objective of this study was to determine the prevalence, circumstances, and outcome of fractures in males with Duchenne muscular dystrophy (DMD) attending neuromuscular clinics. Three hundred and seventy-eight males (median age 12 years, range 1 to 25 years) attending four neuromuscular centres were studied by case-note review supplemented by GP letter or by interview at the time of clinic attendance. Seventy-nine (20.9%) of these patients had experienced fractures. Forty-one percent of fractures were in patients aged 8 to 11 years and 48% in independently ambulant patients. Falling was th

[Conservative treatment of neurogenic foot deformities].

The lack of muscle control in neurological disorders often leads to deformities of the foot. The equinus (a contracture of the triceps surae muscle), the abduction and clubfoot deformity are the most common ones. The present paper describes the deformities and stresses the functional relevance for the patient. Therapeutic principles of the conservative orthopaedic means are described with special emphasis on the need of an adequate height of the aide and the foot position. Casting and the application of Botulinun toxine are described having a major side effect of muscle weakening. A combinatio

Pediatric flatfoot: evaluation and management.

Although the exact incidence of flatfoot in children is unknown, it is a common finding. All children have only a minimal arch at birth, and more than 30% of neonates have a calcaneovalgus deformity of both feet. This condition is not painful and generally resolves without treatment; very rarely is corrective casting necessary. Most children who present to an orthopaedist for evaluation of flatfoot will have a flexible flatfoot that does not require treatment. Nevertheless, the examining physician must rule out other conditions that do require treatment, such as congenital vertical talus, tars
