BACKGROUND: Problem of closure of skull base defects after removal of craniobasal lesions, especially, craniofacial, is one of the most challenging in neurosurgery. Persistent skull base defect produces extremely high risk of cerebrospinal fluid leaks and consecutive infectious complications. Local pedicled grafts are the preferred material for plasty. In this study, the authors present original technique of using a pedicled buccal fat pad (BFP) graft. Anatomy and functions of BFP are discussed in details as well as surgical technique illustrated by 2 case reports.

BACKGROUND: Current international guidelines mandate routine skull base coverage for the nodal clinical target volume (CTVn) in nasopharyngeal carcinoma (NPC), however, this universal approach may lead to unnecessary radiation exposure. This study aimed to validate the feasibility of our optimized CTVn delineation strategy that challenges this conventional paradigm.
METHODS: This retrospective study included non-metastatic NPC patients who received radical IMRT at our institution between 2014 and 2018. We employed an individualized CTVn delineation strategy: the cranial border was primarily s

BACKGROUND: Endoscopic transnasal surgery (ETS) has become an established treatment for skull base lesions; however, its lateral reach to the pterygopalatine fossa (PPF) and infratemporal fossa (ITF) remains limited due to restricted maneuverability within the nasal corridor. To address this limitation, the endoscopic modified medial maxillectomy (EMMM) and the direct approach to the anterior and lateral part of the maxillary sinus (DALMA) provides lateral access through the maxillary sinus. This study evaluated the technical feasibility and outcomes of ETS combining EMMM and DALMA.

BACKGROUND: Chordomas are rare, aggressive tumors of notochordal origin, commonly affecting the spine and skull base. Skull base chordomas (SBCs) comprise approximately 39% of cases, with an incidence of less than 1 per million annually in the United States. Prognosis remains poor due to resistance to chemotherapy, often requiring extensive surgical resection and adjuvant radiotherapy. Current classification methods based on chromosomal deletions are invasive and costly, presenting a need for alternative diagnostic tools. Radiomics allows for noninvasive SBC diagnosis and treatment planning.

AIM: In this study, we aimed to provide our initial experience with a novel combined approach resection of chondrosarcomas of the skull base and petrous apex (ChPA) and posterior cranial fossa by employing middle cranial fossa and retrosigmoid (RS) approaches with an augmented reality and virtual reality (AR/VR) system.
CASE PRESENTATION: A 66-year-old female patient was referred to our department owing to the growth of a left petrous apex lesion noted in computed tomography (CT) and magnetic resonance imaging (MRI) scans 10 years earlier. Her symptoms included headache, imbalance, and left-s

Reconstruction of the middle cranial fossa following extensive dural resection remains challenging, particularly in ensuring watertight closure and minimizing complications. We present a consecutive case series of five patients undergoing skull base surgery via a minipretemporal approach, in whom direct dural closure was not feasible due to resection-related dural loss or intraoperative dehiscence. A vascularized "superfascial" flap was used for reconstruction. This flap combines periosteum, galea, and superficial temporalis fascia in a single vascularized layer, preserving branches of the sup

Skull-base chordoma is a rare, aggressive bone cancer with a high recurrence rate. Despite advances in genomic studies, its molecular characteristics and effective therapies remain unknown. Here, we conduct integrative genomics, transcriptomics, proteomics, and phosphoproteomics analyses of 187 skull-base chordoma tumors. In our study, chromosome instability is identified as a prognostic predictor and potential therapeutic target. Multi-omics data reveals downstream effects of chromosome instability, with RPRD1B as a putative target for radiotherapy-resistant patients. Chromosome 1q gain, asso

ObjectiveProximal internal carotid artery (ICA) ligation with high-flow bypass is an important vascular reconstructive technique for giant ICA aneurysms or skull base tumors involving the ICA to control intraoperative bleeding. Retrograde or collateral blood flow occasionally remains postoperatively and requires complete internal trapping. Although transgraft internal trapping is one of the options, there are few reports on transgraft treatment. The purpose of this study was to report our experience of transgraft internal trapping and evaluate the safety and efficacy of this procedure.MethodsW

BACKGROUND: The expanded endoscopic endonasal approach (EEA) is currently well accepted for a variety of ventral skull base tumors. Such an approach often results in a transdural defect and intraoperative cerebrospinal fluid (CSF) leak, and adequate reconstruction is necessary to prevent postoperative CSF leak and its complications. Reconstruction is usually performed using a variety of materials along with the nasoseptal flap.
OBJECTIVE: This work's aim is to describe a new reconstructive technique called the "soft gasket seal" (SGS) in detail and compare it with the standard reconstructive

A 52-year-old man was admitted to our hospital because of two episodes of bacterial meningitis within a 6-month period. CSF examination showed neutrophilic pleocytosis with marked elevation of protein and hypoglycorrhachia, but the inflammatory reaction was mild and blood and CSF cultures were negative. At the time of the second admission, intermittent watery nasal discharge caused by CSF rhinorrhea was evident. CT and MR imaging revealed a tiny clival bone defect, and transnasal endoscopic repair was performed successfully. The pathological diagnosis was chordoma based on immunohistochemical

INTRODUCTION: Intraoperative identification of the cranial nerves is a useful technique in removal of skull base tumors through the endoscopic endonasal approach. Searching through the scientific literature found one pilot study on the use of triggered electromyography (t-EMG) for identification of the VIth nerve in endonasal endoscopic surgery of skull base tumors (D. San-Juan, et al, 2014).
AIM: The study objective was to prevent iatrogenic injuries to the cranial nerves without reducing the completeness of tumor tissue resection.
MATERIAL AND METHODS: In 2014, 5 patients were operated on

BACKGROUND: The endoscopic modified Lothrop procedure (EMLP) is well established for resistant frontal sinus disease and anterior skull base (ASB) exposure. However, this technique may be unnecessarily aggressive by removing avoidable sinonasal structures in select cases. Previously, in a cadaveric study, we proposed a modification of the EMLP, termed the modified subtotal-Lothrop procedure (MSLP), to access the ASB and to address complex frontal sinus disease, for which access to the bilateral frontal sinus posterior table is required. This study provides a step-by-step description of this te

The clivus is a bone of the central skull base formed from the basisphenoid and the basiocciput. During normal ageing, the clival marrow converts from haematopoietic with low-to-intermediate T1 signal to fatty high T1 signal. Low T1 signal in adults is a marker of disease. This review illustrates the main important diseases of the clivus. Chordoma, fibrous dysplasia, myeloma, and metastasis arise within the clivus. Chondrosarcoma, nasopharyngeal carcinoma, invasive pituitary macroadenoma, cholesteatoma, and mucocele can be recognized from attention to adjacent structures. Although some disease

BACKGROUND: Successful reconstruction of large anterior skull base (ASB) defects after craniofacial resection of malignant skull base tumors is paramount for preventing cerebrospinal fluid (CSF) fistulas. The vascularized pedicled pericranial flap (PCF) has been the gold standard for repairing ASB defects after transbasal transcranial approaches. However, flap necrosis and delayed CSF leaks can occur after adjuvant radiation therapy. We describe a "double flap" reconstruction technique in which the PCF is augmented inferiorly by a secondary vascularized pedicled nasoseptal flap (NSF) that is h

Tuberculum sellae meningiomas frequently extend into the optic canals. Radical tumor resection including the involved dural attachment, underlying hyperostotic bone, and intracanalicular tumor in the optic canal offers the best chance of a Simpson Grade I resection to minimize recurrence. Decompression of the optic canal with removal of the intracanalicular tumor also improves visual outcome since this portion of the tumor is usually the cause of asymmetrical visual loss. The purely endoscopic endonasal extended transsphenoidal approach offers a direct midline trajectory and immediate access t
