Biliary tract was investigated in 105 patients after endoscopic papillosphincterotomy for choledocholithiasis and papillostenosis. The follow-up revealed papillary restenosis in 8,6%, which occurred during first 2 years after the procedure. Short cut of the duodenal papilla and intramural part of the common bile duct and anamnestic papillostenosis and septic cholangitis proved to be the main reasons of the papillary restenosis.

Patients with mechanical obstruction of the pancreatic duct, which can be caused by chronic pancreatitis, suffer from recurrent attacks of pain and inflammation of the pancreas. We report a novel approach using an endoscopic ultrasound- (EUS-) assisted rendezvous technique, which allows drainage of the pancreatic duct in patients in whom primary management by transpapillary drainage during an endoscopic retrograde cholangiopancreatography (ERCP) procedure has failed. Transgastric puncture of the pancreatic duct was performed using a 19-gauge needle under EUS guidance, and a 0.035-inch guide wi

BACKGROUND/PURPOSE: Little is known about whether the severity of pancreatitis depends upon persistent stone impaction or stone passage into the duodenum, and the role of endoscopic sphincterotomy (ES) has remained controversial.
METHODS: This study reviewed our experience of 183 patients with gallstone pancreatitis, with special attention paid to the relationship between the severity of pancreatitis, the severity of coexisting biliary pathology, and the outcome.
RESULTS: Sixteen patients (9%) had severe pancreatitis (SP) and the remaining 167 (91%) had mild pancreatitis (MP). All of the SP …

Trauma to the major papilla and transient supraphysiologic biliary tract pressure during ERCP may produce transient serum liver test elevation. Further investigation of these abnormal serum tests may be costly, potentially hazardous, and unnecessary. Transient rises in post ERCP serum liver tests may be a common epiphenomenon that requires only careful clinical observation. Our aim was to study serum liver test results collected before and after ERCP in asymptomatic (or minimally symptomatic) patients and determine the natural clinical history of these patients, without further intervention. Previous upper gastrointestinal surgery with the construction of a Roux-en-Y jejunal loop may prevent endoscopic access to the second part of the duodenum. We report a technique of percutaneous transhepatic balloon sphincteroplasty to facilitate the removal of common bile duct (CBD) stones. A 67-year-old woman presented with a 1-week history of right upper quadrant abdominal pain and fever, deranged liver function tests, and dilated intrahepatic ducts. The patient had previously had a total gastrectomy with Roux-en-Y reconstruction for a high-grade B-cell lymphoma of the stomach. Peroral endos

Extracorporeal shock wave lithotripsy was performed on 4 high risk patients with large common bile duct stones by a Dornier MPL 9000 lithotripter and X-ray positioning. Every patient was treated twice using 2000 shock wave discharges and 21 kV average generator voltage in a single session. Average treatment time was 51 minutes. Desintegration of 3 patients stone was successful and fragments were removed by Dormia basket extraction and by biliary lavage. One out of the four patients stone failed to be desintegrated this patient underwent an operation. Apart from mild fleabite-like petechiae and

Endoscopy is frequently used to treat biliary abnormalities; however, controlling the catheter is difficult when tortuous structures or specific intrahepatic ducts must be negotiated. Intraductal manipulation with conventional angiographic guidance is difficult, and combined transhepatic approaches are painful, associated with risk, and cumbersome. The authors describe a multidisciplinary approach to complex endoscopic procedures in which the interventional radiologist controls catheter and guide wire placement. Of 344 procedures attempted over a 3-year period, 304 were accomplished with trans

A 91-year-old Chinese man with a 30-year history of diabetes and 5-year history of hypertension presented in January 2026 with acute epigastric pain and obstructive jaundice. Unenhanced CT showed dilated bile ducts with multiple linear hyperattenuating structures mimicking biliary stones. ERCP after sphincterotomy yielded abundant blood clots but no calculi, with persistent active bleeding; cholangioscopy revealed an irregular friable bleeding lesion at the left hepatic duct/hilar confluence. Fully covered metal and plastic stents were placed, with marked reduction of bleeding. Follow-up CT tw…

Choledochotomy and choledochoduodenostomy were commonly used technique in the early twentieth century to extract bile duct stones. Endoscopic retrograde cholangiopancreatography and sphincterotomy revolutionised the scenario and is currently the preferred first-line option for managing choledocholithiasis. However, in certain circumstances, where endoscopic retrograde cholangiopancreatography fails, choledochotomy and choledochoduodenostomy are the only available options. We present the case of an 86-year-old female with a background of multiple previous presentations with biliary sepsis and a…

Cholangitis is a GI emergency requiring prompt recognition and treatment. The purpose of this document from the American Society for Gastroi … cutaneous drainage and biliary decompression within 48 hours. Additionally, the panel suggests that sphincterotomy and stone removal be combined with drainage rather than decompression alone, unless patients are too unstable to tolerate more extensive endoscopic treatment.

The results of treating 29 patients with Mirizzi syndrome are analyzed: 17 patients underwent traditional surgery, 11 patients--laparoscopic procedures, and endoscopic sphincterotomy with nasobiliary drainage was performed in a type I patient. Laparoscopic operations were performed in patients with types I and II syndrome, patients with types III and IV had only open surgery. In 2 cases operations were complicated by iatrogenic injury to the hepaticocholedochus, in 4 cases--by residual bile duct stones. Pronounced inflammatory alterations and scarring of the hepaticocholedochus, III and IV typ…

Endoscopic Retrograde Cholangiopancreatography (ERCP) is commonly performed in patients after liver transplantation. The most common indications for ERCP include treatment of bile leaks and anastomotic and nonanastomotic biliary strictures. In this report we describe an unusual complication of ERCP in a liver transplant recipient with a bile leak two months after orthotopic liver transplantation (OLT). After confirming a bile leak, a hydrophilic guide wire was placed in the intrahepatic duct, an endoscopic sphincterotomy was performed, and a biliary plastic stent was successfully placed over t…

We present 13 patients who were seen at Glasgow hospitals over a 33-year period. Only five (38 per cent) were children. Of the eight adults, … five had internal drainage, one had partial excision and one had cholecystectomy and (years later) sphincterotomy. We believe that excision of the cyst is the treatment of choice.

Forty-one patients presented to our hospital between 1959 and 1983 with periampullary carcinoma. Twenty-six (63 per cent) underwent radical surgery, eight (20 per cent) local excision of the tumour, six (15 per cent) had bypass procedures and one was treated by endoscopic sphincterotomy (2 per cent). Potentially curative resection was performed in 83 per cent of the 41 patients. The operative mortalities for radical, local and bypass surgery were 7.7 per cent, 25 per cent and 16.6 per cent respectively. The degree of tumour differentiation significantly affected survival while local spread did…

Despite the careful observance of standard precautions during endoscopic sphincterotomy, serious complications are sometimes unavoidable and these may require various forms of treatment. Surgical intervention is necessary for fulminating pancreatitis, acute arterial hemorrhage, and retroperitoneal abscess. The endoscopic insertion of a drainage tube into the common bile duct can serve to overcome biliary obstruction due to cholangitis secondary to stone impaction, Dormia basket impaction or blood clot. The obstruction can be removed electively either endoscopically or surgically in a symptom-f…

A 50-year-old female presented with acute epigastric abdominal pain, diarrhea, nausea, and vomiting for two days. Laboratory data showed hyp … bile duct dilation. These findings are consistent with a diagnosis of Lemmel's syndrome. A biliary sphincterotomy was performed to relieve the obstruction and one temporary plastic biliary stent was placed into the common bile duct. The duodenal diverticulitis was treated with antibiotics for 10 days and the patient made a good recovery.
