Bilateral patellar tendon rupture following indirect trauma in a young patient due to suspected chronic tendon degeneration caused by repetitive microtrauma: A case report.

INTRODUCTION: Bilateral patellar tendon rupture is an uncommon injury, particularly in young, healthy individuals without established risk factors for tendinopathy. Most reported cases involve systemic disease or medication-related tendon compromise. We present a rare case of simultaneous bilateral patellar tendon rupture in a healthy, athletic male following an indirect mechanism of injury. CASE PRESENTATION: A 33-year-old man sustained acute bilateral knee injuries while playing recreational rugby. He reported an audible "pop" in both knees, immediate pain, and inability to extend either le

Gliding Tissue Reconstruction Using a Dorsal Digital Adipofascial Flap in Complex Extensor Injury.

BACKGROUND: Postoperative scar adhesions between tendons and phalanx bones cause persistent disability in complex injuries involving tendons and bones of the hand. Although gliding tissue reconstruction is effective in preventing peritendinous adhesion formation and a dorsal digital adipofascial flap is a reliable method to prevent scar adhesion between tendon and bone after extensor tendon repair, no comparative clinical reports exist. This study aimed to determine the usefulness of a gliding tissue reconstruction method by comparing postoperative range of motion between patients who underwen

Mechanical, histological, and functional properties remain inferior in conservatively treated Achilles tendons in rodents: Long term evaluation.

Conservative treatment (non-operative) of Achilles tendon ruptures is suggested to produce equivalent capacity for return to function; however, long term results and the role of return to activity (RTA) for this treatment paradigm remain unclear. Therefore, the objective of this study was to evaluate the long term response of conservatively treated Achilles tendons in rodents with varied RTA. Sprague Dawley rats (n=32) received unilateral blunt transection of the Achilles tendon followed by randomization into groups that returned to activity after 1-week (RTA1) or 3-weeks (RTA3) of limb castin

[Significance of the preoperative status in the evaluation of results of the reconstruction of extensor tendons].

Since 1977, a new evaluation scheme that is simple to use and which provides more objective information than earlier plans has been employed for the assessment of the results of extensor tendon reconstruction. The results in the four groups were evaluated and given a prognosis in accordance with the newly developed scheme. This permitted not only a better differentiation of the value of the therapeutic procedures, but also allowed one to make prognostic predictions. The prognosis is also of particular importance during preoperative talks with the patient.

The Utility of Plain Radiography in Flexor Tendon and Digital Nerve Lacerations of the Hand.

PURPOSE: Physical examination during hand lacerations may sufficiently identify flexor tendon and digital nerve injuries, but radiographs are often acquired to rule out fracture, dislocation, or foreign body. This study examines the impact of plain radiography in the management of patients with isolated sharp injuries of the hand. METHODS: We retrospectively reviewed consecutive adult patients with flexor tendon and/or digital nerve lacerations who presented during 2019-2021 to a tertiary referral urgent care hand clinic. Charts were reviewed to determine the presence of bony injury or foreig

Simultaneous Quadriceps Tendon Rupture, Contralateral Patellar Tendon Rupture, and Lateral Meniscus Tear Following a Single Fall: A Case Report.

Simultaneous ruptures of the quadriceps tendon and contralateral patellar tendon are exceedingly rare, particularly in patients without serious comorbidities or other known predisposing factors. We present the case of an obese 31-year-old man without metabolic syndrome or any other known predisposing risk factors who suffered a right quadriceps tendon rupture and simultaneous left patellar tendon rupture, and tear of the anterior horn of the left lateral meniscus. These injuries occurred while walking downstairs and missing a step. He presented with bilateral knee pain, ecchymosis, and the ina

Restoration of the first dorsal interosseous muscle.

Motor dysfunction of the 1st dorsal interosseous (DIO) muscle is typically observed in low and high ulnar nerve palsy. This causes weak thumb-index pinch, which can be disabling for the patient. Various reconstructive techniques have been described; however, the choice often depends on the surgeon's experience, the presence of associated neurovascular and musculotendinous injuries, as well as the requirements of the palliative surgery schedule. Nerve transfers can be proposed when patients present early in the course of the disease. Tendon transfers are often a last resort when late presentati

Partial-thickness tears of the gluteus medius: rationale and technique for trans-tendinous endoscopic repair.

Tears in the gluteus medius and minimus tendons, often misdiagnosed as trochanteric bursitis, have recently emerged as an important cause of recalcitrant greater trochanter pain syndrome. Advances in endoscopic surgery of the hip have created opportunities to better evaluate and treat pathology in the peritrochanteric compartment. We reviewed the literature on trochanteric pain syndrome and gluteus medius tendon injuries. Existing techniques for endoscopic and open gluteus tendon repair and potential challenges in restoration of abductor function were analyzed. Partial-thickness undersurface t…

Rehabilitation following repair of a torn latissimus dorsi tendon.

BACKGROUND AND PURPOSE: This report describes the rehabilitation of a patient following surgical repair of a torn latissimus dorsi tendon. The scientific rationale for the treatment progression is discussed. CASE DESCRIPTION: A 35-year-old man with a ruptured latissimus dorsi tendon 6 weeks following surgical repair was referred for physical therapy to recover range of motion and strength sufficient for return to work as a police officer on the SWAT team. A review of tendon healing in animal studies is presented and related to the development of the plan of care for this patient. OUTCOMES: L

Dislocation of the tibialis posterior tendon: diagnosis and treatment.

Traumatic dislocation of the tibialis tendon occurred from minor ankle sprains in a 37-year-old male and a 53-year-old female. Both complained of local pain at the medial malleolus, and both walked with a limp. The diagnosis was suspected by clinical examination, in one case with 2 months' delay, and verified by ultrasound, computed tomography, and magnetic resonance imaging. The male patient was initially treated for an "uncomplicated ankle sprain." For various reasons surgery was delayed 4 months. During this interval the male patient complained of pain and severe dysfunction, requiring anal

Surgery under fire: war injuries and the consequences of delayed care in Ethiopia's Tigray region.

BACKGROUND: The armed conflict in Ethiopia, particularly in the Tigray region, has caused a substantial burden of war-related injuries in the context of a fragile and disrupted health system. Restricted access to timely surgical care has resulted in delayed presentation and may contribute to secondary complications, contractures, peripheral nerve dysfunction, and long-term disability. However, the consequences of delayed surgical treatment for conflict-related extremity trauma in active war settings remain insufficiently described. This study aimed to characterize conflict-related injuries in

[Relative Motion Splinting: Indications and Results].

Relative motion splinting is a simple way to reduce tension on extensor and flexor tendons after an injury or surgery. The affected finger is placed in increased extension (relative motion extension splint, RMES) or flexion (relative motion flexion splint, RMFS) relative to the neighbouring fingers, allowing for active tendon gliding within a protected range of motion at the site of repair or injury. While originally introduced for the treatment of extensor tendon injuries, the concept of relative motion splinting is now being used for a growing number of indications. It is characterised by a …

Muscle-tendon-related abnormalities detected by ultrasonography are common in symptomatic hip dysplasia.

INTRODUCTION: Hip dysplasia is characterized by reduced acetabular coverage of the femoral head leading to an increased mechanical load on the hip joint and the acting hip muscles. Potentially, the muscles and tendons functioning close to the hip joint may present with overuse-related ultrasonography findings. The primary aim was to report the prevalence of muscle-tendon-related abnormalities detected by ultrasonography in 100 patients with symptomatic hip dysplasia. The secondary aim was to investigate correlations between muscle-tendon-related abnormalities detected by ultrasonography and cl

Twelve-Month Maturation Timeline of Quadriceps Tendon Autograft Signal Intensity Ratio After ACL Reconstruction Surgery Using Magnetic Resonance Imaging.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are among the most common sports-related injuries, accounting for about half of all knee injuries, and most athletes opt to undergo ACL reconstruction (ACLR). The quadriceps tendon (QT) ACLR has only recently been gaining popularity, and therefore few studies exist for evaluating the healing, or ligamentization, timeline of the QT autograft. PURPOSE: To evaluate the signal intensity (SI) ratio of the tendon QT autograft after ACLR during the first postoperative year. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A prospective

Conservative management of an isolated near-complete distal biceps femoris tendon rupture in an elite football player.

This case report presents an adolescent elite football player who sustained an isolated, near-complete rupture of both the long and short heads of the distal biceps femoris tendon, with approximately 20 mm tendon retraction and no associated bony avulsion or posterolateral knee injury. The injury occurred during a forceful cross-kick and was initially challenging to characterise because of conflicting ultrasound and MRI findings. Given the preserved clinical function, limited retraction and evidence of residual tendon continuity, the medical team opted for conservative management, without brac
