Lateral circumflex femoral artery system: warehouse for functional composite free-tissue reconstruction of the lower leg.

BACKGROUND: Microsurgical free-flap surgery has progressed from simply providing wound coverage to restoring a high level of function. The concepts and practice of using compound, composite, and chimeric flaps have recently further enhanced the versatility of free flaps in reconstructive surgery. A lateral circumflex femoral arterial (LCFA) system can provide a potential single composite free-tissue transfer for restoration of functional and structural integrity. METHODS: Between 1997 and 2003, we used 44 free flaps to restore functional and structural defects in the lower limbs. The versatil

Effect of calcaneal osteotomy and lateral column lengthening on the plantar fascia: a biomechanical investigation.

Medial calcaneal displacement osteotomy or lateral column lengthening fusion has been advocated to augment tendon transfer in planovalgus foot deformity associated with chronic posterior tibial tendon insufficiency. It is hypothesized that plantar fascia tightening occurs with these procedures, helping to restore a more normal longitudinal arch. To investigate this further, nine fresh-frozen cadaver below-knee specimens were used. A flatfoot model was created by sectioning of the posterior tibial tendon, spring ligament, talonavicular capsule, and deltoid ligament. A liquid-metal strain gauge,…

Anterior transfer of the toe flexors for equinovarus deformity due to hemiplegia.

We report the results of transfer of the long toe flexors and lengthening of the calcaneal tendon in 33 patients with equinovarus deformity requiring orthoses after a stroke. Review of 29 patients more than two years after surgery showed that 21 were able to walk without an orthosis. Equinovarus deformity had recurred in six patients and hammer toe in 11, but walking ability without bracing was still better in seven of these. Results are improved by the release of the short toe flexors.

Modified arthroscopic latarjet procedure with coracoid exteriorization for treatment of anterior glenohumeral instability.

The Latarjet procedure for treating anterior glenohumeral instability includes transfer of the coracoid and biceps tendon to the anterior glenoid. A modified method for the arthroscopic procedure was developed to facilitate the procedure and minimize the risk of injury to the brachial plexus. The detached coracoid was exteriorized through the anteroinferior portal for drilling and shaping. A Coracoid Drill Guide (Arthrex, Naples, FL) was used to help cut the coracoid to the desired size and make 2 drill holes in the coracoid for fixation to the glenoid. The Coracoid Transfer Instrument (Aciera

Transposition of local muscles to restore elbow flexion in brachial plexus palsy.

Between 1994 and 2001, triceps to biceps transfers were done in 10 men and a transfer of the forearm flexors and extensors (Steindler procedure) in nine. All had suffered from a post-traumatic lesion of their brachial plexus, resulting in loss of elbow flexion. Their mean age at the time of the original accident was 27 years (range 16-50 years) and at the time of muscle transfer 35 years (range 22-56 years), with a mean observation period of 20 months (range 6-51 months). In 16 patients, a neurosurgical procedure had been performed after the trauma, and in 22 patients other reconstructive oper

Mitochondria-Rich Extracellular Vesicles From Bone Marrow Stem Cells Mitigate Muscle Degeneration in Rotator Cuff Tears in a Rat Model Through Macrophage M2 Phenotype Conversion.

PURPOSE: To investigate the protective effects of extracellular vesicles derived from bone marrow stem cells (BMSC-EVs) on muscle degeneration in a rat model of rotator cuff tendon and suprascapular nerve (SSN) transection (termed the RCT-SSN model), focusing on mitochondrial transfer. METHODS: The EVs were identified and characterized. The RCT-SSN model was established by transecting the supraspinatus, infraspinatus tendons, and suprascapular nerve. Ninety-six rats were divided into 4 groups (n = 24 each): sham surgery, RCT-SSN treated with BMSC-EVs, RCT-SSN treated with EVs from rhodamine 6

Soft tissue rebalancing procedures with and without internal rotation osteotomy for shoulder deformity in children with persistent obstetric brachial plexus palsy.

INTRODUCTION: Children with obstetric brachial plexus palsy (OBPP) frequently have problems related to their shoulder. The aim of the investigation was to determine our results in treating shoulder deformity with tendon transfers and soft tissue releases with and without internal rotational osteotomy. We also evaluated the relationships between neurological status, age and selected clinical parameters. MATERIALS AND METHODS: We reviewed data of 25 patients (12 girls) after latissimus dorsi and teres major tendon transfers to the rotator cuff. Internal rotation osteotomy was performed in ten c…

Arthroscopic Management of Massive Irreparable Rotator Cuff Tears: Whole Rotator Cable Reconstruction Using Proximal Biceps Tendon Autograft.

Massive irreparable rotator cuff tears (MIRCTs) are not uncommon in clinical practice, significantly impacting shoulder function and daily activities. Extensive tear size, tendon contracture, and fat infiltration within the rotator cuff pose significant challenges for both patients and clinicians. This type of tear is a key area of interest and a challenge in research and treatments. Current treatment options include conservative management, debridement, partial repair, superior capsule reconstruction (SCR), tendon transfers, and reverse total shoulder arthroplasty (RTSA). However, clinical ou…

Free transplantation of autogenous palmaris longus tendon in the repair of cicatricial ectropion of lower eyelid.

Treatment of cicatricial lower eyelid ectropion is often difficult and requires surgical intervention. Numerous techniques have been developed over the years to treat the cicatricial lower eyelid ectropion. This article describes an effective surgical technique using the free transplantation of autogenous palmaris longus tendon in the repair of cicatricial lower eyelid ectropion. The operation was performed with the patient under local anaesthesia. After the contracture of the lower eyelid skin or conjunctiva had been thoroughly released, a palmaris longus tendon graft was obtained through sma

The Clinical Outcome of Anatomical Reattachment of the TFCC to the Ulnar Fovea using an ECU Half-Slip and an Interference Screw.

We developed an original reattachment technique using a half-slip of the extensor carpi ulnaris (ECU) tendon with a very small titanium interference screw for chronic foveal avulsion of the TFCC. The clinical outcome of 66 wrists with foveal detachment of the TFCC treated by this procedure was examined.A distally based ECU half-slip was harvested, inserted into the TFCC, sutured to the remnant of the TFCC, and pulled out through a 2.5-mm bone tunnel at the centre of the fovea. The ECU half-slip was subsequently anchored to the ulnar fovea with a small titanium interference screw. We evaluated

The role of proximal pulleys in preventing tendon bowstringing: pulley rupture and tendon bowstringing.

PURPOSE: The aim of this study was to investigate factors that contribute to tendon bowstringing at the proximal phalanx. We hypothesised that: (1) a partial rupture of the A2 pulley leads to significant bowstringing, (2) the location of the A2 rupture, starting proximally or distally, influences bowstringing, (3) an additional A3 pulley rupture causes a significant increase in bowstringing following a complete A2 pulley rupture and (4) the skin and tendon sheath may prevent bowstringing in A2 and A3 pulley ruptures. METHODS: Index, middle and ring fingers of eight freshly frozen cadaver arms

Correction and prevention of deformity in type II tibialis posterior dysfunction.

UNLABELLED: Cobb described a method of reconstruction in Johnson and Strom Type II tibialis posterior dysfunction (TPD) using a split tibialis anterior musculotendinous graft. We assessed patient function and satisfaction after a modified Cobb reconstruction in a group of patients with a narrow spectrum of dysfunction, examined a modification of the Johnson and Strom classification to emphasize severity of deformity, and assessed the ability of the technique to prevent subsequent fixed deformity. We prospectively followed 32 patients managed by this technique and a translational os calcis oste

Arthroscopic assisted lower trapezius transfer for the treatment of irreparable posterosuperior rotator cuff tears with nonfunctional teres minor: are the results sustained after more than 7.5 years?

BACKGROUND: Lower trapezius transfer (LTT) has been demonstrated to restore external rotation (ER) in patients with brachial plexus palsies. In certain cases of cuff tear arthropathy, patients exhibit preserved forward elevation but lack active ER. This study evaluates the clinical outcomes of LTT, augmented with a semitendinosus tendon autograft and anchored to the infraspinatus insertion using an arthroscopically assisted technique and, to determine whether pain relief and functional improvement are maintained at minimum 7.5-year follow-up compared to the 11-month follow-up. METHODS: Betwee

Surgical Options for the Restoration of Elbow Extension After Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are devastating and lead to marked functional disability. Injuries involving the proximal radial nerve, posterior cord, or lower cervical nerve roots can lead to triceps palsy and inability to actively extend the elbow. Although not as functionally important as elbow flexion, active elbow extension is required for some activities of daily living, and discussion of reconstructive options with the patient is warranted. Various nerve transfers have been successfully used and may be available, depending on the extent of injury and other surgeries planned or performe

Reconstruction of a C7-T1 brachial plexus lower root injury by transferring multiple nerves and a free gracilis muscle: Case report.

Lower-type brachial plexus injuries (BPI) are uncommon, and traditional reconstruction with tendon transfer procedures generally produce mediocre results. However, the advent of nerve transfers has rejuvenated the reconstructive options for peripheral nerve and spinal cord injuries. In this paper, we report the case 32 year-old patient with a C7-T1 avulsion BPI in whom multiple motor and sensory nerve transfers, combined with a free gracilis muscle flap transfer, were used to restore upper-limb functional defects. Five months after injury, several nerves were transferred (posterior division of…

Improving the extensor lag and range of motion following free vascularized joint transfer to the proximal interphalangeal joint: Part 2. A clinical series.

BACKGROUND: Free vascularized joint transfer for reconstructing the posttraumatic proximal interphalangeal joint has enjoyed limited popularity because of the low range of motion typically achieved after transfer. One of the commonest complaints is the significant extensor lag. Part 2 of this two-part study is focused on the clinical outcomes following a more anatomical approach to extensor tendon reconstruction. METHODS: Nine patients (eight male and one female), with a mean age of 31.7 years, underwent free vascularized joint transfer for posttraumatic proximal interphalangeal joint injurie

Secondary reconstruction of the extensor tendons and overlying soft tissue deficiency.

Reverse-flow pedicled radial forearm tendocutaneous flap transfers were performed to reconstruct simultaneous skin and tendon defects in the dorsum of the hand in six patients. All flaps survived and four of six patients obtained good functional results. To obtain satisfactory functional outcome, early surgical repair is critical after trauma. The only disadvantage of this operation is unacceptable donor site morbidity.
