Laryngotracheal injury following cricothyroidotomy.

Airway complications following elective cricothyroidotomy were reviewed in 48 adult cardiothoracic surgery patients. A 52% incidence of airway complications was found and manifested by failed or delayed decannulation, extensive subglottic granulation tissue, stenosis, vocal cord paralysis, and aspiration pneumonia. The most common cause for decannulation difficulty was subglottic stenosis (50%). Several risk factors were specifically identified, including a period of cricothyroid cannulation exceeding 30 days, the presence of diabetes, and advanced age. These findings suggest that airway seque

[Asphyxic pulmonary edema caused by upper airway obstruction in adults].

A case of pulmonary oedema following acute pharyngo-laryngeal obstruction is reported. The case is compared with the results of the literature. The haemodynamic data suggested a non-cardiogenic effect, with low pulmonary arterial occlusion pressure of 1 mmHg and right-to-left shunting. Positive end-expiratory pressure was used with success. The mechanisms underlying such oedema are not well known, and may include disturbances in the balance of transpulmonary forces or hypoxia. However the outcome is usually favourable.

Obstructive sleep apnea treated with uvulopalatopharyngoplasty: a systematic follow-up study.

We evaluated ten consecutive cases of obstructive sleep apnea treated with uvulopalatopharyngoplasty (UPPP). The preoperative all-night polysomnograms were compared to those done one to three months after surgery. Excessive daytime sleepiness, the main complaint of every patient, partially improved in three and was completely abolished in seven. The lowest oxygen saturation levels were below 80% in nine cases before UPPP and in only three afterward. The number of apneic episodes per hour of sleep, or apnea index, decreased in every patient and reached normal levels in five. Improvement of othe

Severe upper airway obstruction and difficult intubation in cicatricial pemphigoid.

Upper airway obstruction in cicatricial pemphigoid is rarely seen by the anaesthetist. A case of severe dyspnoea due to progressive laryngeal obstruction is presented. Tracheal intubation during induction of general anaesthesia for permanent tracheostomy was extremely difficult and was successfully performed by introducing a size 12 French suction catheter through the stenotic laryngeal orifice. Adequate ventilation was achieved by using the oxygen flush valve to deliver oxygen intermittently through the catheter.

The anatomy of the fasciae of the face and neck with particular reference to the spread and treatment of intraoral infections (Ludwig's) that have progressed into adjacent fascial spaces.

Descriptions of the fasciae of the lower half of the face and of the adjacent cervical fasciae have long been puzzling and descriptively much too complex. For this reason, medical students, young medical and dental practitioners, and at times even senior surgeons frequently do not understand the anatomy of the cervicofacial fasciae, which plays such an important role in the spread and subsequent final localization of primary intraoral infections. This article attempts to simplify the descriptions of these fasciae, in particular, their sites of origin and insertion.

Sleep apnea syndromes.

The sleep apnea syndromes have been recognized clinically in the United States only within the past ten years. The true extent of the problem is not known, but it seems certain that these syndromes are much more common than was generally assumed five years ago. Every clinician should be aware of the signs and symptoms of sleep apnea because of the rapid and prompt response to therapeutic measures. Sleep apnea syndromes, whether obstructive or central, can result in systemic or pulmonary hypertension, arterial blood gas abnormalities, life-threatening cardiac arrhythmias, chronic respiratory fa

Paralysis of the larynx and pharynx.

Widespread neurologic dysfunction of the pharynx and larynx is catastrophic. Aspiration pneumonitis, resulting from laryngeal and pharyngeal incompetence, is incompatible with life. In such cases, surgical intervention plays a key role and may be clearly life-saving. A system of surgical management has evolved based upon an understanding of functional priorities, leading to an organized plan of treatment and rehabilitation.

The treatment of subglottic stenosis in children by prolonged dilatation.

Laryngeal stenosis can present with recurrent lower respiratory tract infections, hoarseness, stridor or decreased exercise tolerance. Asphyxia and death may follow. One accepted method of treatment is to perform a tracheostomy, dilate the stricture and insert an obturator for 6 to 9 mo. Ten children with a subglottic stenosis were treated by this technique and 7 completed the treatment. Results in 5 of these children were good, 1 was improved and 1 restenosed. These results compare favorably with those reported by others. This form of treatment is indicated for unyielding but dilatable strict

[Mode of action and results of long-term assisted ventilation at home of chronic respiratory insufficient patients (author's transl)].

It is generally agreed that one assisted ventilation session has direct effects in chronic respiratory insufficiencies (C.R.I.) of chronic obstructive pulmonary diseases: hyperventilation where PaCO2 decreases and PaO2 increases, diminution of ventilatory work, reduction of cardiac output. Less is known, however, about the action on V and Q distribution and mechanics of breathing. The return to control values occurs quite rapidly after the end of the session. Long-term effects are much more questionable. Many publications in English have concluded that ona long-term basis, patients do not bene
