Non-leukemic myeloid sarcomas involving only the central nervous system: a case report and literature review.

A previously healthy 39-year-old male presented with a five-month history of dizziness and progressive numbness and weakness of the lower limbs for more than two months. Contrast-enhanced MRI revealed multiple enhancing soft tissue masses in the spinal canal, while PET/CT indicated numerous hypermetabolic lesions in the intracranial and spinal regions. Initial pathological findings suggested inflammatory lesions, and the diagnosis remained uncertain after consultations at multiple hospitals. The patient was admitted to our hospital on June 15, 2019. Subsequent examinations after admission rule

A Case Report of Widely Disseminated Tuberculosis in Immunocompetent Adult Male.

INTRODUCTION: Disseminated tuberculosis (TB) is rare, affects any organ system, and presents mainly in immunocompromised populations. Typical presentation is non-specific, posing a challenge for diagnosis. CASE REPORT: This case presents an immunocompetent male presenting with severe headaches with meningeal signs. Lab and lumbar puncture results suggested bacterial meningitis, yet initial cerebral spinal fluid cultures and meningitis/encephalitis polymerase chain reaction were negative. A chest radiograph (CXR) provided the only evidence suggesting TB, leading to further tests showing dissem

Spinal cord infection: myelitis and abscess formation.

PURPOSE: Our purpose was to describe the MR findings and evolution of spinal cord abscess and to define those MR features that allow differentiation of cord infection from other intramedullary abnormalities. METHODS: We retrospectively reviewed the MR studies of all patients in whom intramedullary spinal cord abscess was proved either by blood or cerebrospinal fluid culture or by serologic examination at our institution between January 1988 and January 1996. The study group included four adults and two children, 7 to 74 years old (mean age, 38 years). RESULTS: Initial MR studies showed intra

[Clinical and pathological manifestations in 129 patients with tuberculous meningitis].

OBJECTIVE: To investigate the diagnosis of tuberculous meningitis. METHOD: One hundred twenty-nine cases of tuberculous meningitis proved by autopsy, from the year 1952 through 1994, were retrospectively reviewed. RESULTS: Sixty-four cases were misdiagnosed before death, and the misdiagnosis rate was 49.6%. Among these misdiagnosed cases, 34 were misdiagnosed as other brain diseases, and the other 30 cases were misdiagnosed as severe pulmonary tuberculosis, severe pneumonia, gastric cancer, rheumatic heart disease, cor pulmonale and leukemia, etc. The main clinical manifestations included me

Etiological Insights and the Role of Individual Factors in Infectious Spondylodiscitis.

Objectives: Spondylodiscitis can be caused by various microorganisms and has shown a continuous rise in incidence and mortality. The purpose of our study was to analyze the demographic and laboratory data, as well as comorbidities of patients that were surgically treated for spondylodiscitis in our hospital. The causative pathogens involved in the etiology of spinal infections were also assessed. Methods: The study included 92 patients who underwent clinical, radiological, and microbiological analyses including bacterial isolation. According to their culture results, patients were divided into

Acute vertebral body compression fractures: discrimination between benign and malignant causes using apparent diffusion coefficients.

Diffusion weighted MRI was performed on patients with acute vertebral body compression. The usefulness of the apparent diffusion coefficient (ADC) in differentiating between benign and malignant fractures was evaluated. A total of 49 acute vertebral body compression fractures were found in 32 patients. 25 fractures in 18 patients were due to osteoporosis, 18 fractures in 12 patients were histologically proven to be due to malignancy, and 6 fractures in 2 patients were due to tuberculosis. Signal intensities on T(1) weighted, short tau inversion recovery (STIR) and diffusion weighted images wer

Unusual causes of spinal foraminal widening.

Spinal neural foraminal widening is usually caused by benign lesions, most commonly neurofibromas. Rare lesions can also cause spinal neural foraminal widening. Computed tomography and/or MRI are the modalities of choice for studying the spinal foraminal widening. The present pictorial review describes six rare lesions, namely a lateral thoracic meningocele, a malignant fibrous histiocytoma, a tuberculous abscess, an osteoblastoma, a chondrosarcoma and a malignant tumour of the lung which caused spinal neural foraminal widening.

Pott's disease and hypercalcemia in a patient with rheumatoid arthritis receiving methotrexate monotherapy.

Methotrexate (MTX) may have adverse effects on multiple organs and system. A few cases of pulmonary tuberculosis in-patients with rheumatoid arthritis (RA) while receiving MTX monotherapy has been reported in the literature. We submit a case of vertebral tuberculosis with hypercalcemia in a patient receiving MTX monotherapy. Patient with RA taking MTX for 15 years developed pancytopenia, skin necrosis, tuberculous spondylodiscitis and hypercalcemia. The present case showed adverse effects of MTX therapy may occur even after years of continuous treatment. Due to pancytopenia in older patients,

[Cervical vertebral tuberculosis simulating a peritonsillar abscess].

We report the case of a 28-year-old woman initially diagnosed with a left peritonsillar abscess, which was drained, resulting in clinical relief. Twelve days later, a bulge was observed in the posterior pharyngeal wall. CT and MRI showed a tumour with destruction of atlas lateral mass, with a soft tissue component in prevertebral, retropharyngeal, left carotid and paraspinal spaces. Biopsy and microbiological study confirmed the presence of Mycobacterium tuberculosis. Therapy was initiated with isoniazid, pyrazinamide, rifampicin and ethambutol, an occipitocervical-C1-C2 arthrodesis was perfor
