Assessing the subjective visual vertical, SVV, in a static upright position is an easy clinical test in which a deviation of some 10 degrees from true vertical indicates an acute loss of unilateral (otolithic) vestibular function on the side to which the SVV is tilted. Because this deviation of the SVV is compensated during the following months, patients with chronic unilateral vestibular loss do no longer differ from normal subjects. This study presents an experimental set-up that allows for clear detection of compensated chronic loss of unilateral otolithic function by testing the SVV. 21 no

OBJECTIVE: Patients with dizziness may present with symptoms of tilting, swaying, rocking, floating or with disequilibrium. This may be suggestive of an isolated otolithic dysfunction yet, there is little emphasis on this emerging clinical entity. To characterize and describe the prevalence of isolated otolith dysfunction in a local tertiary hospital and correlate them with clinical diagnosis.
METHODOLOGY: Retrospective medical chart review of patients who presented with dizziness to the specialist outpatient Otolaryngology clinic, who required vestibular laboratory investigation.
RESULTS: O

Sixty-four patients with the diagnosis of Menière's disease were tested at the House Ear Clinic with an active head-rotation test system, the Vestibular Autorotation Test (VAT). The VAT is a portable, computerized test that measures the horizontal vestibulo-ocular reflex (VOR) with the use of high-frequency (2 to 6 Hz) active head movements to obtain gain and phase. The purpose of this study was to characterize the horizontal VOR at high frequencies in patients with Menière's disease. At frequencies from 5 to 6 Hz, all patients demonstrated horizontal phase greater than 180 degrees and 85% sho

OBJECTIVES: Lindsay-Hemenway syndrome (LHS) is characterized by an acute unilateral vestibular loss followed by ipsilateral posterior semicircular canal (PSC) benign paroxysmal positional vertigo (BPPV). Despite its clinical relevance, comparative data between LHS, isolated acute unilateral vestibulopathy (AUVP), and idiopathic PSC BPPV are uncommon. This work aims to compare vestibular function and clinical outcomes in patients diagnosed with LHS, isolated AUVP, and idiopathic PSC BPPV.
METHODS: A multicenter longitudinal study was conducted between 2018 and 2025, enrolling 98 patients divid

Microvascular decompression procedures were carried out to relieve right hemifacial spasm in a 54-year-old man. His facial spasm was satisfactorily cured but he complained of severe vertigo, and slight facial palsy was also shown after the operation. Electronystagmograph disclosed spontaneous horizontal and rotatory nystagmus toward the non-operated (left) side. The postoperative caloric test showed profound dysfunction on the operated side. His hearing was well maintained postoperatively. These findings indicated, in addition to facial nerve injury, that the peripheral portion of the vestibul

PURPOSE: The primary goals are to investigate the rate of vestibular deficit (VD) during the acute attack and to follow up on vestibular recovery (VR) after the resolution of BPPV in patients with rare forms of BPPV that cause downbeat nystagmus. Second, to determine the effects of VD on dizziness severity in patients with rare BPPV forms and to compare all the results with those of typical posterior canal BPPV.
METHODS: This prospective follow-up study included 23 patients (56.86 ± 12.21 years) with rare BPPV forms (5 anterior canal, 18 apogeotropic posterior canal) and 23 patients (50.75 ±

Vestibular compensation after unilateral vestibular deafferentation is modulated by certain individual characteristics, such as pre-operative visual neurosensory preference or vestibular pattern. Physical activity (PA) allows the implementation of new sensorimotor and behavioral strategies leading to an improvement of balance control. This study aimed to evaluate the effect of the level of PA before surgery on balance compensatory mechanisms in patients after vestibular schwannoma (VS) surgery. Thirty patients with VS, 15 considered as regularly physically active and 15 as sedentary participat

CONTEXT: Concussion causes physiological disruptions, including disruptions to the vestibular and visual systems, which can cause dizziness, imbalance, and blurry vision. The vestibular ocular reflex functions to maintain a stable visual field, which can be measured using the gaze stability test (GST).
DESIGN: This preliminary study used retrospective chart review to examine changes in GST performance and asymmetry in a sample of 117 youth athletes with concussion (mean age = 14.51, SD = 2.08) before (T1) and after (T2) they completed a vestibular therapy program that included in-office treat

Vestibular dysfunction in humans is associated with anxiety and cognitive disorders. However, various animal studies of the effects of vestibular loss have yielded conflicting results, from reduced anxiety to increased anxiety, depending on the particular model of vestibular dysfunction and the anxiety test used. In this study we revisited the question of whether rats with surgical bilateral vestibular deafferentation (BVD) exhibit changes in anxiety-related behaviour by testing them in the open field maze (OFM), elevated plus maze (EPM) and elevated T maze (ETM) in the presence of a non-sedat

Vertigo, dizziness, and imbalance are among the most frequent presenting symptoms in primary care and in the emergency department (ED). The clinical diagnosis is based on a structured patient history and bedside examination of the vestibular system and the ocular motor system, looking especially for central ocular motor signs. Recent guidelines, such as the GRACE-3 guideline on acute vertigo and dizziness, the increasing availability of bedside diagnostic algorithms, and the use of quantitative vestibular and ocular motor testing in both the ED-setting and in the outpatient center have facilit

The spectrum of diagnoses of patients with dizziness as the leading symptom who consult a neurologist does not differ greatly from the spectrum of those who consult ear nose and throat (ENT) specialists or general practitioners (GP). The most frequent forms are benign paroxysmal positioning vertigo (BPPV), phobic postural vertigo, central vertigo disorders, Menière's disease, vestibular neuritis and bilateral vestibulopathy. However, the first and most important question that is posed to neurologists is whether it is a central or peripheral syndrome. In more than 90 % of cases this differentia

Ménière's disease, characterized by episodic vertigo, sensorineural hearing loss, and tinnitus, is well-documented in humans but rarely reported in veterinary medicine. This case report describes a 15-year-old male mixed-breed dog with recurrent vestibular episodes lasting 30 min to several hours. Clinical signs included ataxia, head tilt, and nystagmus. MRI and diagnostic workups ruled out common structural causes of vestibular signs and ear diseases. Audiometric testing revealed bilateral sensorineural hearing loss with preserved high-frequency hearing in the right ear, a pattern resembling

BACKGROUND: Caspase-3 is one of the most downstream enzymes activated in the apoptotic pathway. In caspase-3 deficient mice, loss of cochlear hair cells and spiral ganglion cells coincide closely with hearing loss. In contrast with the auditory system, details of the vestibular phenotype have not been characterized. Here we report the vestibular phenotype and inner ear anatomy in the caspase-3 deficient (Casp3(-/-)) mouse strain.
RESULTS: Average ABR thresholds of Casp3(-/-) mice were significantly elevated (P < 0.05) compared to Casp3(+/-) mice and Casp3(+/+) mice at 3 months of age. In DPOA
