A case of fixed cutaneous sporotrichosis that developed on the extensor aspect of the left wrist of an 83-year-old woman was treated once a week with 200 mg of oral fluconazole. This dermatological lesion healed within 4 months, leaving a scar. No side-effects were seen. At the time of writing, 7 months after the end of treatment, there has been no recurrence. Reports on the efficacy of once-weekly fluconazole administration in cases of sporotrichosis have appeared, but it would be valuable to study more such cases.

The authors investigate a case of agraphia induced by a left parieto-occipital tumor (glioma) in a right-handed 67 year-old patient. After three successive neuropsychological examinations it still proves immensely difficult to determine whether defects of spontaneous writing, dictation and copy are due to apraxia, alexia or motor disturbances. The authors discuss whether the clinical findings indicate a "pure agraphia" syndrome or "amnesic agraphia". In the elderly patient with tremor, the differential diagnosis is usually between essential tremor (ET) and Parkinson disease (PD). A careful history and examination are the keys to the diagnosis. Essential tremor is a bilateral action tremor of the upper limbs whereas PD begins unilaterally and is a rest tremor. A handwriting sample can usually distinguish PD from ET as the former is small (micrographic) but atremulous whereas writing in ET is tremulous but normal sized. In ET, there are no signs aside from tremor but in PD, the tremor is accompanied by bradykinesia and rigidity.

Catheterisation for the relief of urinary retention was used by the ancient Chinese and Egyptians and is described in the writings of the Indian surgeon Susrata, who flourished in Benares sometime after Christ. Although many of these cases were the result of urethral stricture from trauma or gonorrhoea, we can assume that, in older men, the cause was often benign prostatic hypertrophy (BPH).

In April 1852 in Cape Town, South Africa, Dr David Livingstone, then 39 years of age and yet to commence the explorations of Africa which would bring him to public attention, underwent the surgical procedure of uvulectomy. This article reviews the history of Livingstone's uvulectomy and its indications and outcome, as documented by his own writings. The practice of uvulectomy in indigenous African societies and some contemporary ideas about uvulectomy are also considered in order to try to ascertain why Livingstone elected to undergo this procedure.

The transfer of Western medicine to Russia increased significantly in the Eighteenth century. Foreign doctors were employed, their writings translated, their education standards copied. But who regarded that knowledge as superior and why? Taking the Moscow Plague of 1771 as a case study, this article examines the crucial role foreign and Russian medical practitioners played during the epidemic. It argues that especially those ideas and practices that were useful for social control filtered into politics and public discourse, but failed to convince the majority of the population.

Naming disorders, as a symptom, is always found in all aphasic patient. The patient although has a clear understanding of what he is trying to name or what he is told to write, but needs longer time or can not articulate it. Sometimes he substitutes with other words or even produces jargon words. Assessment and intervention are complicate and delicated the most important functions of the clinician. The goal of word-retrieval activities in treatment is to improve the patient's cognitive performance and to teach method to circumvent the vocabularly when needed.

Jean Dominique Bauby, former editor of Elle, suffered a stroke to his brain stem that left him with locked-in syndrome. Subsequently, through blinking his left eye, he writes his memoir of this experience, The Diving Bell and the Butterfly. This paper explores the meaning of embodiment, especially as one's body bears upon one's personal identity. It explores the various challenges and threats to selfhood that result from Bauby's experience and recounts how Bauby rises to the challenge through his memory and imagination.

Health visitors are ideally placed in primary care to offer effective dietary and life-style advice as part of a team approach to coronary heart disease prevention, write Alan Jones et al. Results from a dietary and life-style intervention trial programme suggest that dietitians, who are limited in numbers, should be used as a resource to provide expert dietary advice and training to the more numerous health visitors. In this way CHD risk factor identification can be developed into effective intervention.
