A broad overview of intraocular lens implantation is presented for the optometrist. Included in this overview is a review of the most recent literature comparing complication rates for lens implants versus standard cataract surgery. A proportion of the paper addresses itself to the pre and post-operative optometric management of the pseudophakic patient. This paper is presented to give the optometrist a basic understanding of the expanding field of intraocular lens implantation as well as a reference list which will serve as a starting point for further review.

This paper discusses the four available types of plastic aphakic spectacle lenses, which include (1) nonaspheric nonlineticular, (2)hardly aspheric lenticular and moderately aspheric lenticular, (3) mildly aspheric nonlenticular, and (4) highly aspheric nonlenticular in much larger eye size but adjusted closer to the eye to produce an enlarged visual field and suppress the "jack-in-the-box" phenomenon. Controlled aspiration of aqueous before excising the recipient cornea resulted in maintenance of an intact vitreous face during aphakic penetrating corneal transplants in eight of 11 eyes. Vitreous corneal contact or subsequent graft opacification has not occurred in the study group during the observation period.

In series of 39 corneal grafts, aphakic bullous keratopathy comprised 13 (33%) of the cases. By scanning and transmission electron microscopy, endothelial degeneration and posterior collagenous proliferation were consistent features. AIM: To describe a surgical technique for sulcus fixation of a foldable single-piece acrylic intraocular lens (IOL) through a 3.0 mm clear cornea incision.
