[Double superior vena cava with drainage of the right superior vena cava into the left auricle. Presentation as a cerebral abscess in an adult].

A 44 year old man presented with a cerebral abscess, the location of which suggested a septicaemic origin. Although the patient was not cyanosed, a cardiological work-up was requested to exclude a right-to-left shunt. This showed a double abnormality of the systemic venous drainage: presence of an abnormal left superior vena cava draining into the coronary sinus and of a right superior vena cava draining into the left atrium. These two vena cava intercommunicated by anastomoses. Angiography in the right superior vena cava after occlusion by balloon catheter at its junction with the left atrium

Pituitary abscess, plexus-choroiditis and encephalitis by Helcococcus ovis in a calf.

We report here the first confirmed case of a pituitary abscess in a calf caused by Helcococcus ovis, an emerging pyogenic bacterium in veterinary medicine. An 8-month-old calf showed progressive neurological signs following the use of a nose ring. Gross and histopathological examination revealed a large abscess in the pituitary gland accompanied by leptomeningitis, encephalitis, and plexus choroiditis. Helcococcus ovis was isolated from the pituitary abscess and identified by MALDI-TOF mass spectrometry with high confidence. Coccoid gram-positive bacteria were observed on gram stain of formali

Fungus-like hyphochytrids associated with human disease.

We report two cases, with liver and brain abscess, respectively, where fungus-like organisms belonging to the Hyphochytriomycota were found at the site of inflammation together with Peptococcus in the first and Cysticercus cellulosae in the second case. This is the first time these groups of organisms have been reported in human material. The role of hyphochytrids in human pathology remained uncertain as they were found together with already known human pathogens.

Systemic zygomycosis diagnosed by fine needle aspiration and confirmed with enzyme immunoassay.

A diagnosis of systemic zygomycosis made by fine needle aspiration of a kidney abscess was confirmed with a serum enzyme immunoassay for Zygomycetes. The serum immunoassay may provide a method for rapid diagnosis and obviate major invasive diagnostic procedures in establishing the diagnosis of zygomycosis.

A Rare Case of Vancomycin-Induced Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Syndrome.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare but potentially life-threatening adverse drug reaction characterized by extensive skin rash in association with hematological abnormalities, including eosinophilia and atypical lymphocytosis, lymphadenopathy, fever, and extensive visceral organ involvement. Here, we presented a rare case of vancomycin-induced DRESS syndrome in a male who was treated with IV vancomycin for a brain abscess.

Neurological complications during treatment of liver abscess: think of metronidazole toxicity.

Related neurological adverse effects to metronidazole are rarely encountered in clinical practice despite its wide use as an antibacterial or antiparasitic agent. The neurotoxicity is not dose-dependent and is fully reversible with discontinuation of the drug. We describe a young man who was receiving metronidazole for an amoebic liver abscess and developed encephalopathy and seizures. Brain magnetic resonance imaging showed characteristic bilateral symmetrical cerebellar dentate hyperintensities.

Listeria monocytogenes brainstem infection (rhombencephalitis) mimicking ischemic stroke.

The frequency of Listeria monocytogenes (Lm) infection of the central nervous system is increasing. We report a patient recently treated with chemotherapeutic drugs for pulmonary adenocarcinoma who suddenly developed hemiparesis, was initially diagnosed with stroke, and was then found to be affected by Lm rhombencephalitis accompanied by a brain abscess. Lm meningoencephalitis mimicking ischemic stroke is rare but must be considered, especially in specific patients.

Rationale for management of frontal sinus fractures.

The optimal treatment of frontal sinus fractures remains controversial. Multiple treatment options and algorithms have been proposed by multiple specialties throughout the years; however, the optimal method of frontal sinus repair has yet to be discovered. Overwhelming complications such as meningitis, encephalitis or brain abscess are quite uncommon nowadays. Nevertheless, late development of invasive mucoceles is not a rarity and therefore long-term follow-up is mandatory.

Chronic Mycobacterium avium complex infection of the central nervous system in a nonimmunosuppressed woman.

We report the case of a woman who developed chronic meningitis and a brain abscess due to Mycobacterium avium, without immunosuppressed state. She was treated with antituberculous drugs but the infection progressed and she died 4 years after first becoming ill. The occurrence of CNS infection with M. avium in the absence of acquired immunosuppression is rare. Its diagnosis is difficult, delaying clinical recognition and appropriate therapy.

Brain imaging.

Imaging with radionuclides should be used in a complementary fashion with other neuroradiologic techniques. It is useful in the early detection and evaluation of intracranial neoplasm, cerebrovascular accident and abscess, and in postsurgical follow-up. Cisternography yields useful information about the functional status of cerebrospinal fluid pathways. Computerized axial tomography is a new technique of great promise that produces a cross-sectional image of the brain.

Incidence of mastoiditis complications in relation to prior community-based antibiotic treatment.

OBJECTIVES: To evaluate the association between prior community-based antibiotic treatment for acute otitis media (AOM) and the incidence an … s extracranial (subperiosteal abscess) or intracranial (epidural abscess, sigmoid sinus thrombosis, brain abscess, or meningitis) complications. RESULTS: Of 522 children, 110 (21%) received prior antibiotics and 412 (79%) did not. While the composite complication rate was higher in the prior treatment group (54.5% vs. 38.6%, unadjusted OR = 1.91, p = 0.003), p …

Application of metagenomic next-generation sequencing for the diagnosis of intracranial infection of Listeria monocytogenes.

BACKGROUND: Intracranial infection of Listeria monocytogenes (LM) can lead to various manifestations, including meningitis, meningoencephalitis, brainstem encephalitis, and brain abscess, which often have a poor prognosis. Metagenomic next-generation sequencing (mNGS) is a promising new tool for the diagnosis of intracranial infection of LM. We describe the typical clinical manifestations of LM intracranial infection and highlight its rarity and severity to help physicians better understand the disease characteristics. METHODS: Six cases of severe LM intracranial infection were diagnosed by m…

Imaging findings in intracranial aspergillosis.

RATIONALE AND OBJECTIVES: The authors' purpose was to elucidate the various computed tomographic (CT) and magnetic resonance (MR) imaging findings in intracranial aspergillosis. MATERIALS AND METHODS: Retrospective analysis of cranial imaging findings was performed in eight proved cases of central nervous system aspergillosis. The patients ranged in age from 17 to 75 years. Four patients were immunocompromised, and four were immunocompetent. CT was performed in all eight patients, and MR imaging in five. RESULTS: Six patients (75%) had multiple lesions seen on the imaging studies, with a tot
