Clinical identifiers for early-stage primary/idiopathic adhesive capsulitis: are we seeing the real picture?

BACKGROUND: Adhesive capsulitis is often difficult to diagnose in its early stage and to differentiate from other common shoulder disorders. OBJECTIVE: The aim of this study was to validate any or all of the 8 clinical identifiers of early-stage primary/idiopathic adhesive capsulitis established in an earlier Delphi study. DESIGN: This was a cross-sectional study. METHODS: Sixty-four patients diagnosed with early-stage adhesive capsulitis by a physical therapist or medical practitioner were included in the study. Eight active and 8 passive shoulder movements and visual analog scale pain sco

MRI and arthroscopy correlations of the shoulder: a case-based approach.

Over the past three decades, shoulder arthroscopy and MRI have seen significant advancements. Initially, distinguishing between normal and pathologic anatomy was one of the greatest challenges. Radiologists and arthroscopic shoulder surgeons have provided feedback to each other based on their experiences with selected shared patients, thereby accelerating the understanding of the arthroscopic and MRI appearance of both normal and pathologic anatomy. This process has led to the current refined understanding of both the basics and subtleties of MRI and arthroscopy correlations in the shoulder. U

Approach to the active patient with chronic anterior knee pain.

The diagnosis and management of chronic anterior knee pain in the active individual can be frustrating for both the patient and physician. Pain may be a result of a single traumatic event or, more commonly, repetitive overuse. "Anterior knee pain," "patellofemoral pain syndrome," and "chondromalacia" are terms that are often used interchangeably to describe multiple conditions that occur in the same anatomic region but that can have significantly different etiologies. Potential pain sources include connective or soft tissue irritation, intra-articular cartilage damage, mechanical irritation, n

Reliability of function-related tests in patients with shoulder pathologies.

STUDY DESIGN: Nonexperimental. OBJECTIVE: To investigate the intertester and intratester reliability of a battery of function-related tests in patients with shoulder pathologies and associated reduced range of motion. BACKGROUND: A battery of function-related tests has the potential to complement assessment of functional limitation in patients who have shoulder pathologies. METHODS AND MEASURES: Three function-related tests (hand to neck, hand to scapula, and hand to opposite scapula) were conducted on 46 patients with shoulder pathologies, and 46 age- and gender-matched control subjects. [In vitro effects of diclofenac and selective cyclooxygenase-2 inhibitors on prostaglandin release from inflamed bursa subacromialis tissue in patients with subacromial syndrome].

BACKGROUND: To compare the in vitro effects of selective COX-2 inhibitors (L-745,337, NS-398 and DFU) and of COX-unspecific diclofenac on release of PGE(2 )and 6-keto-PGF(1alpha) from inflamed bursa subacromialis tissue (IBST) obtained from a total of 35 patients with shoulder impingement syndrome (SIS). PATIENTS AND METHODS: Bursal specimens were incubated in the presence of drugs (0.01-1000 microM) for 20 min and 16 h. RESULTS: After 20 min 10 microM diclofenac significantly inhibited formation of PGE(2) and 6-keto-PGF(1alpha), whereas L-745,337 and NS-398 (10-1000 microM) induced signific

Common foot and ankle injuries in dance.

Classical ballet is a popular but physically demanding activity. Minor injuries become increasingly common as the dancer encounters the greater workload of professional dancing. The doctor must have a basic knowledge of ballet technique and an understanding of the mental approach of dancers to accurately diagnose and effectively treat their injuries. The practice of dancing on the toes, the exaggerated turn out of the feet, and the extreme flexibility of the hips and spine all lead to unusual injuries. With informed conservative treatment, most ballet injuries will heal, but surgery is occasio

Control of pain in the rheumatic disorders.

Pain may be attacked in the rheumatic diseases (1) centrally, with drugs ranging in efficiency from those which are potentially addictive and under the Dangerous Drugs Act (e.g., pethidine) and are therefore rarely used, down to simple analgesics such as paracetamol; (2) peripherally, by local action, whether it be by applications of heat or cold, by injections of local anaesthetics or anti-inflammatory agents, or by surgery; (3) peripherally, by anti-inflammatory non-analgesic agents taken systemically, such as the corticosteroids; and (4) peripherally, by anti-inflammatory-analgesic-antipyre

The role of systemic and nervous system factors in patients with shoulder pain: a perspective review.

BACKGROUND: Persistent shoulder pain is often driven by inflammatory conditions, including tendinopathy, bursitis, and frozen shoulder. Treatment remains uncertain, but targeting underlying mechanisms like inflammation, metabolic factors, and nervous system disturbances may be more effective. OBJECTIVE: This perspective review summarizes these underlying mechanisms' roles in patients with inflammatory-driven shoulder pain and potential effective treatments for these mechanisms. RESULTS: Literature links inflammatory-driven shoulder pain to low-grade inflammation, obesity, hypertension, diabe…

Projected AR Serious Game "Painting Discovery" for Shoulder Rehabilitation: Assessment With Technicians, Physiotherapists, and Patients.

OBJECTIVE: Motivation and adherence are crucial for effective rehabilitation, yet engagement remains a challenge in upper limb physiotherapy. Serious Games (SGs) have emerged as a promising tool to enhance patient motivation. This study evaluates Painting Discovery, a projected augmented reality (AR) SG for shoulder rehabilitation, assessing engagement, ergonomics, and its potential to differentiate motor performance between healthy and those with rheumatoid arthritis, bursitis, subacromial impingement, rotator cuff tear, or calcific tendinopathy. Additionally, it examines improvements in path…

Performance of ultrasound to monitor Achilles enthesitis in patients with ankylosing spondylitis during TNF-a antagonist therapy.

Enthesitis is considered as the primary anatomical lesion in ankylosing spondylitis (AS). We aimed to investigate the potential of ultrasound to detect early changes after TNF-a antagonist therapy of Achilles enthesitis of AS patients. One hundred AS patients with active disease, requiring TNF-a antagonist therapy, were included (etanercept n = 25, infliximab n = 25, adalimumab n = 25, non-biologic disease-modifying antirheumatic drugs (DMARDs) n = 25). Physical examination was performed to evaluate disease activity and detect Achilles enthesitis and/or retrocalcaneal bursitis. Ultrasound of t…

Effect of immediate full weight bearing on abductor repair and clinical function after THA through a modified Hardinge approach.

This study evaluated the effect of immediate full weight bearing on abductor repair and clinical function after total hip arthroplasty (THA) … rence was observed in Harris Hip Score, limp, thigh pain, high-grade heterotopic bone, trochanteric bursitis, or stem subsidence between groups. Restricting weight bearing postoperatively to protect the Hardinge abductor repair appears unnecessary, and recovery of activity is improved with immediate weight bearing.
