Myasthenia gravis: subgroup classification and therapeutic strategies.

Myasthenia gravis is an autoimmune disease that is characterised by muscle weakness and fatigue, is B-cell mediated, and is associated with antibodies directed against the acetylcholine receptor, muscle-specific kinase (MUSK), lipoprotein-related protein 4 (LRP4), or agrin in the postsynaptic membrane at the neuromuscular junction. Patients with myasthenia gravis should be classified into subgroups to help with therapeutic decisions and prognosis. Subgroups based on serum antibodies and clinical features include early-onset, late-onset, thymoma, MUSK, LRP4, antibody-negative, and ocular forms

New classification of Herlyn-Werner-Wunderlich syndrome.

BACKGROUND: Uterus didelphys and blind hemivagina associated with ipsilateral renal agenesis are collectively known as Herlyn-Werner-Wunderlich syndrome (HWWS). In the literature, the syndrome often appears as a single case report or as a small series. In our study, we reviewed the characteristics of all HWWS patients at Peking Union Medical College Hospital (PUMCH) and suggested a new classification for this syndrome because the clinical characteristics differed significantly between the completely and incompletely obstructed vaginal septum. This new classification allows for earlier diagnosi…

Classification of middle fossa floor dehiscence syndromes.

OBJECT: Middle fossa floor dehiscence (MFFD) can present as multiple syndromes depending on dehiscence location, tissue herniation, and dural integrity. The authors propose a classification system for MFFD with the potential to guide clinical decision making. METHODS: A retrospective analysis of the electronic medical records (years 1995-2012) of patients who had undergone temporal craniotomy for the surgical repair of an MFFD syndrome at a single institution was undertaken. Reviewed data included demographic, operative, presentation, and outcome details. Middle fossa floor dehiscence was cla…

Palmar arch anatomy: Ajmer Working Group classification.

INTRODUCTION: Forearm arteries are frequently used as workhorse site for cardiac catheterization, bypass grafting and haemodialysis access. There is paucity of data on palmar circulation in live human being and only cadaveric data are available till date. We, therefore, made an attempt to look at the various patterns of sufficient or insufficient palmar arch circulation and various anomalies of forearm arteries, to discuss their clinical implications. METHODS AND RESULTS: We obtained the forearm and hand arteriograms of patients (n = 302) through radial (n = 200) and ulnar routes (n = 102). M

Hemimandibular hyperplasia: classification and treatment algorithm revisited.

Hemimandibular hyperplasia (HH) is a developmental asymmetry characterized by three-dimensional enlargement of one half of the mandible. The hyperplastic side usually involves the condyle, condylar neck, ramus, and body, with the anomaly terminating abruptly at the symphysis. The malformation results in the clinical presentation of ipsilateral enlargement of the mandible and tilted occlusal plane, associated with a deviated chin to the contralateral side. Since the first case report of HH in the English literature in 1836, various terminology and classifications were used. In this study, the a…

Clinical and radiographic degenerative spondylolisthesis (CARDS) classification.

BACKGROUND CONTEXT: Lumbar degenerative spondylolisthesis (DS) is a common, acquired condition leading to disabling back and/or leg pain. Although surgery is common used to treat patients with severe symptoms, there are no universally accepted treatment guidelines. Wide variation in vertebral translation, disc collapse, sagittal alignment, and vertebral mobility suggests this is a heterogeneous disease. A classification scheme would be useful to differentiate homogenous subgroups that may benefit from different treatment strategies. PURPOSE: To develop and test the reliability of a simple, cl…

Oral lichenoid tissue reactions: diagnosis and classification.

The concept of lichenoid tissue reaction/interface dermatitis was introduced in dermatology to define a number of diverse inflammatory skin diseases linked together by the presence of common histopathological features. Similarly to the skin, the oral mucosa is affected by a variety of oral lichenoid lesions. Oral LTRs (OLTRs) include: oral lichen planus; oral lichenoid contact lesion; oral lichenoid drug reaction; oral lichenoid lesions of graft-versus-host disease; oral discoid lupus erythematosus; oral lesions of systemic lupus erythematosus; erythema multiforme; paraneoplastic pemphigus/par

[Indolent lymphomas: classification, clinical presentation and treatment]].

The malignant lymphomas comprise a heterogeneous group of neoplastic diseases of the lymphatic system. In the WHO classification 70 different entities are subdivided. Each type of lymphoma is defined as a disease with specific clinical characteristics, as well as morphological, immunophenotypic and genetic characteristics. The indolent lymphomas include follicular lymphoma, marginal zone lymphoma, Lymphoplasmacytic lymphoma, small cell lymphocytic lymhoma/chronic lymphocytic leukemia, prolymphocytic leukemia and hairy cell leukemia. Indolent lymphomas are characterized by slow growth and a chr…

Classification criteria: peripheral spondyloarthropathy and psoriatic arthritis.

Recent proposals for re-classification of spondyloarthropathies according to the predominance of peripheral and axial manifestations and for non-radiographic ankylosing spondylitis to be re-named axial spondyloarthritis are reviewed. It is argued that such proposals are less likely to advance knowledge in the study of spondyloarthopathies and that accurate classification criteria for defined diseases, for example psoriatic arthritis, remain as necessary now as they ever did. The CASPAR criteria remain the best performing classification criteria for psoriatic arthritis.

Wavelet packet entropy for heart murmurs classification.

Heart murmurs are the first signs of cardiac valve disorders. Several studies have been conducted in recent years to automatically differentiate normal heart sounds, from heart sounds with murmurs using various types of audio features. Entropy was successfully used as a feature to distinguish different heart sounds. In this paper, new entropy was introduced to analyze heart sounds and the feasibility of using this entropy in classification of five types of heart sounds and murmurs was shown. The entropy was previously introduced to analyze mammograms. Four common murmurs were considered includ…

[Bronchiolar disorders: clinical-radiological assessment and classification].

Bronchiolar disorders are generally difficult to diagnose. A detailed clinical history may point toward a specific diagnosis. Pertinent clinical questions include history of smoking, collagen vascular disease, inhalation injury, medication use and organ transplantation. It is important also to evaluate possible systemic and pulmonary signs of infection, evidence of air trapping, and high-pitched expiratory wheezing, which may suggest small airways involvement. Pulmonary function tests and plain chest radiography may demonstrate abnormalities; however, they rarely prove sufficiently specific to

Dissimilarity-based classification of anatomical tree structures.

A novel method for classification of abnormality in anatomical tree structures is presented. A tree is classified based on direct comparisons with other trees in a dissimilarity-based classification scheme. The pair-wise dissimilarity measure between two trees is based on a linear assignment between the branch feature vectors representing those trees. Hereby, localized information in the branches is collectively used in classification and variations in feature values across the tree are taken into account. An approximate anatomical correspondence between matched branches can be achieved by inc…

Chronic migraine, classification, differential diagnosis, and epidemiology.

Chronic migraine (CM) is the most disabling of the 4 types of primary chronic daily headache (CDH) of long duration, a syndrome defined by primary headaches 15 or more days per month for at least 3 months with attacks that last 4 hours or more per day on average. CDH of long duration includes CM, chronic tension-type headache, new daily persistent headache, and hemicrania continua. CM affects approximately 2% of the adult population in Western countries, imposing substantial burdens on individual sufferers and their families and, more broadly, upon society. Although this disorder is highly dis

Acquired immune and inflammatory myopathies: pathologic classification.

PURPOSE OF REVIEW: We discuss pathology-based characterization and classification of acquired immune and inflammatory myopathies (IIMs). RECENT FINDINGS: Several types of IIMs do not fit well into the typical IIM subclassifications: dermatomyositis, polymyositis and inclusion body myositis (IBM). Myopathologic features that can provide additional diagnostic clarification in IIM are types of muscle fiber pathology; immune changes (cellular and humoral); and tissues with distinctive involvement (connective tissue, vessels and muscle fibers). Pathologic classification categories include immune m…

Chasing Jenner's vaccine: revisiting cowpox virus classification.

Cowpox virus (CPXV) is described as the source of the first vaccine used to prevent the onset and spread of an infectious disease. It is one of the earliest described members of the genus Orthopoxvirus, which includes the viruses that cause smallpox and monkeypox in humans. Both the historic and current literature describe "cowpox" as a disease with a single etiologic agent. Genotypic data presented herein indicate that CPXV is not a single species, but a composite of several (up to 5) species that can infect cows, humans, and other animals. The practice of naming agents after the host in whic
