[Seborrheic keratoses studied using scanning electron microscopy].

Ten seborrheic keratosis of acanthotic type have been observed by S.E.M. The morphological study could confirm the existence of two processes in the origin of the horny cysts which are peculiar to this affection. a) an extrafollicular process causes the involvement of groups of keratinocytes which, by assuming a vortex disposition, produce a concentric keratinization; b) a follicular process which causes an accentuated keratinization of the cells surrounding the epithelial sheath. This process was associated to the presence of multiple hairs a single follicle. Moreover, with higher magnificati

Cutaneous nerve elimination following punch biopsy.

We report the first case of cutaneous nerve elimination following routine punch biopsy and primary closure. The appearance of a delicate whitish strand from a healed biopsy site, which produces lancinating pain, may represent an extruded cutaneous nerve. This case was treated successfully with simple transection of the nerve at its portal of exit.

Immunohistochemical study on keratin expression in certain cutaneous epithelial neoplasms. Basal cell carcinoma, pilomatricoma, and seborrheic keratosis.

We investigated immunohistochemically 20 basal cell carcinomas (BCC), five pilomatricomas, and nine seborrheic keratoses using anti-BCC keratin monoclonal antibody (BKN-1) and anti-hair keratin monoclonal antibodies (HKN-2, HKN-4- -7). The neoplastic cells in all the cases of BCC were always uniformly stained by BKN-1, HKN-2, and HKN-4, indicating that the BCC cells display a constant antigenicity of keratin, which may be different from that of the normal epidermis. Although no fluorescence by HKN-6 or HKN-7 was seen in any cases of BCC, HKN-5 partially but strongly stained the neoplastic nest

Simultaneous occurrence of junctional nevus and seborrheic keratosis.

A 45-year-old man presented with a pigmented lesion on his face. A clinical diagnosis of melanocytic nevus was made and the lesion was excised. Histopathologic examination showed a reticular seborrheic keratosis associated with a junctional nevus. We discuss the relation of nevocellular nevi with epidermal and adnexal tumors.

Sulfur revisited.

Sulfur is a time-honored therapeutic agent useful in a variety of dermatologic disorders. Its keratolytic action is due to formation of hydrogen sulfide through a reaction that depends upon direct interaction between sulfur particles and keratinocytes. The smaller the particle size, the greater the degree of such interaction and the greater the therapeutic efficacy. When applied topically, sulfur induces various histologic changes, including hyperkeratosis, acanthosis, and dilatation of dermal vasculature. One study showed that sulfur was comedogenic when applied onto human and rabbit skin, fi

Stereological study of desmosomes in basal cell carcinoma and seborrheic keratosis.

Desmosomes exhibit quantitative alterations in neoplastic diseases. We used direct and stereological measurements to quantify these structures in basal cell carcinomas and in seborrheic keratoses, in comparison with the basal stratum of normal epidermis. Tissue fragments were processed for electron microscopy. Random sampling procedure was used to obtain micrographs of cells from each of the three groups. The following parameters were calculated: volume of the 'average' cell (Vcell), relative surface area of plasma membrane occupied by desmosomes (Ss), mean desmosome diameter (delta) and mean

[Pityrosporum ovale in keratotic lesions of seborrheic areas].

The Pityrosporum ovale (PO) is demonstrated in various percentages of the keratotic lesions of the seborrheic areas depending upon the techniques. The accumulation of horny material most probably enhances the multiplication of the yeast and makes its demonstration easier. The PAS technique prevails upon the mycological ones (77 p. 100 versus 50-65 p. 100). PO proliferates only in the keratin layer; there are isolated spores, small clusters or even large stratified colonies. On the other side the prevalence of PO does not differ significantly whatever the ultimate diagnosis of seborrheic kerato

Therapy for acne vulgaris.

Although not curable or preventable, acne can benefit from adequate, appropriate therapy. Modern therapy for acne includes various topical agents highly effective against noninflammatory lesions, and oral or topical antibiotics effective against the inflammatory lesions. Long-term physical and psychologic scarring can be minimized by controlling acne in its active stages. Acne may flare during the first two to three weeks of intensive therapy. Unless undue side effects occur, any single agent or combination of agents should be tried for at least six weeks before being considered ineffective. T

The sign of Leser-Trélat in association with lymphocytic lymphoma.

The sign of Leser-Trélat was observed in a patient with multiple seborrheic keratoses and hemangiomata. The increase in these lesions of the skin preceded the development of a poorly differentiated lymphocytic lymphoma, a type of malignancy which has not yet been reported in association with this sign.

New treatment for seborrheic alopecia: the ligature of the arteries of the scalp.

Bilateral ligature of the superficial temporal arteries and of the posterior auricular arteries is proposed as a treatment for seborrheic alopecia. The arterial circulatory dynamics are, thus, replaced by capillary circulatory dynamics. Hypoxia is produced which inhibits enzymatic systems and lessens nocuous action of androgen and lipid factors on the pilosebaceous effectors. The histologic study shows that the production of sebum is greatly reduced and the condition of the hair follicle is strikingly improved.
