The pathogenesis of lymphatic filariasis has been a matter of debate for many decades. Here, Gerusa Dreyer and colleagues propose a dynamic model of bancroftian filariasis, integrating clinical, parasitological, surgical, therapeutic, ultrasonographic and histopathological data. This model has profound implications for filariasis control programs and the management of the individual patient.

Concomitant parasitism with Plasmodium spp. and Wuchereria bancrofti was examined in Orissa, India, to study the influence of one parasite infection on the other in human communities. A survey of 1815 nocturnal blood films in 11 villages indicated an overall prevalence of 9.6% for malaria and 8.5% for microfilaraemia. Only 0.3% of the population harboured both parasites. Filarial IgM antibody response to various antigenic fractions isolated from immune complexes by SDS-PAGE viz., IC-2, IC-4, IC-7, and IC-9, was studied by ELISA. The IgM antibody response to almost all antigenic fractions was seen more in microfilaraemic group compared to the clinical filariasis group. In April 1991 supervised mass prophylaxis of lymphatic filariasis with a single dose of ivermectin, 100 micrograms/kg, was carried out in a Polynesian village with a high infection rate of Wuchereria bancrofti in humans and active transmission by the vector mosquito, Aedes polynesiensis. We report the first case of inflammation of the retinal pigment epithelium and retinal vasculities presumably caused by microfilaria of Wucheria bancrofti and discuss its possible pathogenesis. Therapy with diethyl carbamazine citrate resulted in rapid resolution of the inflammation.

We compared the efficacy of a single dose of ivermectin with that of a standard course of diethylcarbamazine (DEC) for the control of microfilaremia in 60 patients with bancroftian filariasis who had developed recurrent microfilaremia after each of three or more prior treatments with DEC. Brugia pahangi adults grown in the peritoneal cavities of jirds (Meriones unguiculatus) were implanted into the peritoneal cavities of six inbred mouse strains to investigate this system as a screen for detecting filaricidal activity. The mice were given 15 adult B. pahangi and autopsied 35 days later. The recoveries of adult worms were 25%, 35%, 49%, 33%, 26% and 27% of the number implanted respectively for the MF1, TO, NIH, CBA, BALB/c and C3H/HE strains. Clinical data on 75 patients, 33 males (44%) and 42 females (56%), with histopathological diagnosis of bancroftian filariasis observed in Recife are presented. The disease predominated in young people. Lymph nodes, especially those of the inguinal region, were the most common site of lesion while the female breast represented the most frequent extra-nodal involvement. The index of clinical suspicion was low, neoplasias being the main preoperatory diagnosis.

Nine new zoonotic Brugia infections, 8 acquired in the United States and 1 in Brazil, are described. In each, a single worm was found in histological sections of lymph nodes or lymphoid tissues. A biochemical key was applied in order to study transmission of malaria and Bancroftian filariasis in Anopheles gambiae sensu stricto, An. arabiensis and An. merus in different localities in north-eastern Tanzania.
