Background and Objectives: Tibial malunions are defined as tibial fractures that have healed in a clinically unacceptable position, resulting in deformity such as shortening, lengthening, abnormal rotation, or angulation. These deformities can have adverse effects on patients, such as pain and gait disturbance, as well as long term development of post-traumatic arthritis. This paper seeks to highlight some of the options for surgical management of malunions and detail the strategies and approaches used to manage these complicated cases. Materials and Methods: An exhaustive search was conducted

CASE: We describe an irreducible anterolateral tibiotalar dislocation with an AO/OTA (Arbeitsgemeinschaft fur Osteosynthesefragen/Orthopaedic Trauma Association) B-type pilon fracture. The injury was initially treated with closed reduction, using a medializing force achieved with an external fixator to unhinge the talar body from the fibula, followed by temporary stabilization. Definitive fixation was performed once the soft tissues had recovered.
CONCLUSION: This unique irreducible pilon fracture dislocation pattern is important to recognize to prevent iatrogenic complications associated wit

Fracture healing is a complex physiologic process, relying on the crucial interplay between biological and mechanical factors. It is general … search groups are currently developing smart orthopaedic implants (plates, intramedullary nails and external fixators), able to provide information about the fracture healing process. These devices could significantly improve orthopaedic and trauma clinical practice in the future and, at the same time, reduce patients' exposure to X-rays. This study aims to define …

Midfoot Charcot joints are complex problems that are most commonly seen in patients with peripheral neuropathy secondary to diabetes. The goal of management is to prevent pedal collapse, which can lead to ulceration; infection; and in some cases, amputation. Principles of surgical management should be centered on respecting the soft tissue, obtaining correction, maintaining correction, and supplementing with orthobiologics to achieve healing. The authors present strategies, case examples, and tips and tricks to treat this complex condition with external and internal fixation.

Difficult femoral nonunion takes account of infective nonunion and aseptic gap nonunion. Limb length discrepancy and nonunion need to be tackled simultaneously. Conventionally Ilizarov ring fixator is in vogue but it has some limitations. To overcome these, monorail fixator is an effective alternative. Persistent good results can be obtained if we can get a perfect anatomical alignment and good regeneration.

We present our experience and results after using polydioxanone (PDS) foil in septal reconstruction. In a period of 2 years, 12 patients who were admitted in our department with severe septal deviation and breathing problems underwent septoplasty under general anaesthesia. The nasal septum was approached via an external approach. In all patients, after resecting and exposing the septum, the removable piece after being divided into straight pieces, was sutured onto an appropriate sized PDS foil and reimplanted together between the mucoperichondrium flaps. Sutures were placed to fixate the "new

BACKGROUND: Three-dimensional (3D) printing is a common manufacturing technique. It is being adopted to produce essential surgical equipment … Fused Deposition Modeling printers. All materials were thermoplastic. Printed equipment ranged from external fixators to surgical sets. The printing details were well reported. However, there was inconsistency in testing methods, resulting in data that is incomparable across the studies. 2 of 15 papers conducted mechanical testing for external fixators using Americ …

Currently, there are two methods of treating subtrochanteric femur fractures: intramedullary nailing and extramedullary plate fixation. However, it remains to be seen whether internal or external fixation is the primary treatment for subrotator femur fractures in adults. In this meta-analysis, we reviewed the effects of internal and external fixation on the treatment of subrotator and subrotator fractures. Until July 2023, the researchers have been researching the Cochrane Library, Embase, Web of Science and PubMed, with English as their only language. The aims of the study are as follows: (1)

Purpose. Pediatric femoral supracondylar fractures are difficult to reduce by either closed or open reduction. The abnormal muscle forces around the knee tend to significantly displace the distal short metaphyseal fragment. We describe a novel technique utilizing the combination of a temporarily intraoperative external fixation in order to achieve and maintain the reduction followed by internal fixation. Method. Three male patients younger than 16 years of age were operated in our department. The fractures were defined as pathological in 2 patients. In order to facilitate and maintain fracture

The aim of this study was to compare the results of treatment of segmental tibial defects in the pediatric age group using an Ilizarov external fixator versus a nonvascularized fibular bone graft. This study included 24 patients (age range from 5.5 to 15 years) with tibial bone defects: 13 patients were treated with bone transport (BT) and 11 patients were treated with a nonvascularized fibular graft (FG). The outcome parameters were bone results (union, deformity, infection, leg-length discrepancy) and functional results: external fixation index and external fixation time. In group A (BT), on

PURPOSE: Proximal tibiofibular joint (PTFJ) dislocations are under-investigated injuries. There is scant basic science or clinical evidence to direct management. The purpose of this study was twofold; first to investigate the pathomechanics of PTFJ dislocation on knee mechanics. The second purpose was to evaluate knee mechanics following reduction and fixation.
METHODS: Six cadaveric legs were tested on a mechanical platform. A 5 Nm external rotation force was applied to each knee and the external rotation and fibular translation was measured for several study conditions at 0°, 30°, and 90° o

The condition known as longitudinal epiphyseal bracket or delta phalanx is a rare congenital anomaly that affects the tubular bones of hand or foot. Metatarsal epiphyseal bracket restrains longitudinal growth, causing progressive deformity and resulting in a short, broad metatarsal and a medially deviated metatarsophalangeal joint and hallux magnus. Although there are some case series describing metatarsal lengthening with the unilateral fixator in the literature, we could not find any case that combines both metatarsal lengthening and deformity correction with the circular external fixator. Equinovarus deformity is associated with various disorders and diseases, including this case of recurrent hemarthrosis due to hemophilia. The patient demonstrated progressive deformation in the ankle and foot over the course of several years of evaluation. Until medical hemostatic control was possible, surgery was contraindicated. At age 9, therapeutic control of bleeding was attained, and surgical intervention was initiated. Due to concerns for skin and soft tissue compromise in this patient, a monolateral multiplanar-geared external fixator (M2 MultiPlanar MiniRail, Orthofix) was placed usin

OBJECTIVE: To evaluate a combined technique for treating distal femoral bone defects after debridement of osteomyelitis, using an external fixator together with a short supracondylar nail.
DESIGN: Retrospective study.
SETTING: Single tertiary referral center.
METHODS: Between 2003 and 2018, 23 patients with a mean age of 37.2 years (26-56) underwent surgery with the same technique to manage postdebridement defects in the distal femur due to osteomyelitis. This involved acute shortening and intramedullary fixation of the defect site, together with relengthening from a proximal osteotomy usin

PURPOSE: There are few clinical data evaluating the outcome of surgery for open distal radius fractures based on treatment method. Specifically, the major contributing factors to infection are largely unknown. The purpose of this study is to determine the effect of early versus delayed debridement and the choice of initial external versus internal fixation on infection rates and the need for secondary procedures.
METHODS: Thirty-two patients with open distal radius fractures were identified from a database. Ten debridements were early (<6 h after hospital admission), and 22 debridements were
