Fibular shaft allograft support of posterior joint depression in tibial plateau fractures.

Posterior depression of the lateral articular surface of the tibial plateau can be difficult to elevate and support with morselized bone graft and internal fixation. Progressive collapse after open reduction and internal fixation has been described and can lead to failure in treatment. A standard anterolateral approach to the tibia may not allow direct reduction and stabilization of posterolateral joint depression given the anatomic barriers of the fibular collateral ligament and the proximal tibiofibular articulation. Posterolateral approaches to the tibial plateau have been described and may

[Treatment of old femoral neck and nonunion with free vascularized fibular grafting].

OBJECTIVE: To evaluate the clinical result of the treatment of old femoral neck fracture and nonunion with free vascularized fibular grafting. METHODS: From November 2000 to December 2005, 29 cases with old femoral neck fracture and nonunion had been treated by free vascularized fibular grafting with an average follow-up of 28.5 months. RESULTS: All the fracture were healed without any severe complications. And the healing time was 4-6 months (5.6 months on average). During the follow-up, the hips of 28 cases got well-function, and the average Harris hip score was 88.2. One case came about w

Percutaneous cyst aspiration with injection of two different bioresorbable bone cements in treatment of simple bone cyst.

INTRODUCTION: Simple bone cysts (SBCs) are common in children and adolescents. The risk of refracture and the probability of spontaneous healing in SBCs are mainly dependent on the activity of the cyst and can be quantified with the Cyst-Index. Avoiding pathological fractures is the primary goal. Our study presents a comparison of two different bioresorbable bone graft substitutes (BGSs) in the minimally-invasive treatment of SBC in the active stage by percutaneous cyst aspiration and injection. METHODS: Between 2006 and 2017, 38 patients (aged two to 37 years; mean age 12.4 (sd 5.6)) were tr

Contemporary management of aseptic diaphyseal tibia non-unions - A systematic review.

INTRODUCTION: Tibia fractures are the most common long bone injuries encountered in the trauma population. The majority are treated successfully but non-union remains a common complication. A systematic review of current evidence regarding the management for aseptic diaphyseal tibial non-unions was undertaken. METHODS: A systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA), was conducted. RESULTS: A total of 632 publications were screened for inclusion. Full text review of 91 studies resulted in 26 publications being retained for final revie

[Open valgus alignment osteotomy of the proximal tibia with fixation by medial plate fixator].

High tibial osteotomy (HTO) is an established technique for the treatment of the symptomatic varus malaligned knee. Correction is usually achieved by closed wedge osteotomy from a lateral exposure. This procedure has a certain risk potential regarding peroneal nerve injuries, instability of the osteotomy and secondary loss of correction. We present four technical modifications of HTO which improve safety and reproducibility of this operation. 1) Open wedge osteotomy from a medial exposure avoids lateral muscle detachment, dissection of the peroneal nerve, proximal fibula osteotomy and leg shor

A Non-Lethal Osteogenesis Imperfecta Type II Mutation.

BACKGROUND: We discuss the ethical decision points in a case report that describes a novel COL1A1 mutation associated to Osteogenesis Imperfecta type II, but with a non-lethal outcome. CASE: A 33-year-old female underwent a 21-week ultrasound that revealed short bowed femurs and humeri with old fractures and bowed tibias and fibulas. Amniotic fluid testing revealed a novel COL1A1 mutation (c.1840G>A; p.Gly614Arg). OI Type II diagnosis was made. A previously reported mutation of the same gene but different locus (c.1840G>C; p.Gly614Arg) led to a lethal form of OI type II. The newborn was deliv

Common peroneal nerve injury during varicose vein operation.

Common peroneal nerve (CPN) injury produces considerable and serious disability. The nerve is most frequently damaged as a result of trauma (sharp or blunt, traction, fracture, laceration, and avulsion). Less often iatrogenic injury is the cause of damage (application of tight plaster, retraction injury, division during operation). Even rarer is the complete or partial division of CPN during varicose vein operations. In the UK, on average 34 patients every year begin legal action against their medical attendants in connection with the treatment of varicose veins, on a background of an estimate

Packing with high-porosity hydroxyapatite cubes alone for the treatment of simple bone cyst.

In 23 patients, simple bone cysts were treated by curettage followed by packing the cavity with cubes of high-porosity hydroxyapatite (HA). In 22 patients, HA was packed exclusively without autogeneic bone graft. Except in one patient who sustained pathologic fracture at the femoral diaphysis, HA was packed with a fibula strut graft. Each case had an uneventful postoperative course and was followed, on average, two years. Complete healing without cyst recurrence occurred in 18 cases (78%). In three cases--all rapidly growing children--a new cyst appeared between the epiphyseal plate and the HA

"Iatrogenic" Segmental Defect: How I Debride High-Energy Open Tibial Fractures.

High-energy, open tibial shaft fractures may result in significant comminution, bone loss, and soft tissue injuries. Early, thorough debridement of all nonviable tissue is of critical importance in treating these fractures as an inadequate initial debridement increases the risk of infection and nonunion. Large iatrogenic bone and soft tissue defects can result from debridement and will require subsequent reconstruction by both orthopaedic and plastic surgeons. Although a variety of approaches exist to address these reconstructions, successful management of bone defects remains a considerable c

The healing stages of an intramedullary implanted tibia: A stress strain comparative analysis of the calcification process.

AIMS: The extended usage of unreamed tibial nailing resulted in reports of an increased rate of complications, especially for the distal portion of the tibia. Unreamed nailing favours biology at the expense of the achievable mechanical stability, it is therefore of interest to define the limits of the clinical indications for this method. Extra-articular fractures of the distal tibial metaphysis, meta-diaphyseal junction, and adjacent diaphysis are distinct in their management from impaction derived ''pilon'' type fractures and mid-diaphyseal fractures. The goals of this work were to gain a th

Locking plate fixation with fibular allograft augmentation for failed peritrochanteric fractures: A technical note.

The failure cases of peritrochanteric fractures are occasionally encountered due to poor bone stock. For proximal humeral fractures, a strategy using a segment of the fibular allograft (FA) placed endosteally was introduced. Several studies have demonstrated favorable outcomes with FA. Inspired by this, a fixation strategy using FA with a locking plate was performed in patients with unfavorable bone stock conditions. Ten patients with poor bone stock and nonunion of peritrochanteric fractures were treated with FA and a locking plate at a tertiary hospital between 2016 and 2022. We analyzed the

Medial and lateral segond fractures in a skeletally immature patient: a radiographic marker for the multiply injured knee.

Marginal fractures of the medial tibial plateau have been reported in the literature as a secondary type of Segond fracture. Some reports described this entity in the setting of combined injuries such as root avulsions of the medial meniscus, complete disruption of the posterior cruciate ligament (PCL), partial tear of the anterior cruciate ligament (ACL), and tears of the medial meniscus and medial collateral ligament. It has been postulated that medial marginal fractures are secondary to compression of the medial aspect of the femoral condyle and tibial plateau with a corresponding posterola

Osteosynthesis of femoral-neck nonunion with angle blade plate and autogenous fibular graft.

PURPOSE: Revision internal fixation for femoral-neck nonunion is a challenging procedure. Treatment options are osteotomy, osteosynthesis using various implants and grafting techniques (muscle pedicle, vascularised or nonvascularised fibular graft) or arthroplasty. The objective of this article is to report the outcome of revision internal fixation using an angle blade plate and autogenous fibular graft in symptomatic aseptic femoral-neck nonunion. METHODS: Twenty-two patients who had been treated previously with cannulated screws or dynamic hip screw for femoral-neck fracture and progressed

Vacuum-assisted closure complicated by erosion and hemorrhage of the anterior tibial artery.

We report an erosion and hemorrhage of a left anterior tibial artery associated with a vacuum-assisted closure device. To our knowledge, this report represents the first arterial erosion associated with a vacuum-assisted closure device. We estimate our patient lost 6 units of blood. The hemorrhage was complicated by anticoagulation and a traumatic setting. Based on our complication, we believe great care should be taken when placing a vacuum-assisted closure device adjacent to an exposed artery.

Pediatric ankle fractures: evaluation and treatment.

Pediatric ankle fractures account for approximately 5% of pediatric fractures and 15% of physeal injuries. The biomechanical differences between mature and immature bones, as well as the differing forces applied to those bones, help explain the differences between adult and pediatric fractures. The potential complications associated with pediatric ankle fractures include those seen with adult fractures (such as posttraumatic arthritis, stiffness, and reflex sympathetic dystrophy) as well as those that result from physeal damage (including leg-length discrepancy, angular deformity, or a combina

Osteofibrous dysplasia: a narrative review.

Osteofibrous dysplasia (OFD) is a rare, benign, self-limited bone disorder with a relatively low incidence, accounting for approximately 0.2% of all primary bone tumors. It was frequently found intra-cortical of the mid-shaft of the tibia. OFD can also occur in other skeletal regions, including the fibula, ulna, radius, femur, humerus, ischium, rib, tarsus, metatarsals, vertebral, and capitate. OFD can present with asymptomatic, mass, pain, swelling, deformity, and even pathological fracture. OFD might be misdiagnosed as adamantinoma (AD) and because they are three subtypes origin from the sam

Cross-beam Non-vascular Fibular Graft Reconstruction in the Management of Long Bone Giant Aneurysmal Bone Cyst - A Case Series.

INTRODUCTION: Aneurysmal bone cyst (ABC) is a benign expansile osteolytic lesion, characterized by a blood-filled cavity in the bone. Giant ABC (GABC) is an uncommon condition due to the delayed presentation of an ABC that is difficult to handle when it occurs in long weight-bearing bones due to its aggressive nature. The common treatment relies on total resection of tumor with reconstruction of the resultant defect. CASE REPORT: We present the results of 5 cases of GABCs of long bones managed with non-vascularized fibular graft in a cross-beam fashion along with internal fixation. All patien

Invasive spindle-cell rhabdomyosarcoma with osteolysis in a dog: case report and literature review.

Rhabdomyosarcoma (RMS), a malignant mesenchymal neoplasm derived from skeletal muscle, is relatively rare in both human and veterinary medicine. Here we report an unusual case of invasive spindle-cell RMS (SCRMS) with bone infiltration and pathologic fracture in a 3.5-y-old intact female Bulldog. Radiographically, a large, predominantly osteolytic mass in the tibia and fibula of the left hindlimb had features typical of a malignant primary bone tumor. Clinically, osteosarcoma was suspected, and the leg was amputated. Histologically, the mass was composed of loosely interwoven spindle-cell fasc

Induced membrane technique for reconstruction of a 25 cm humerus diaphyseal defect secondary to chronic osteomyelitis in an adolescent.

The surgical treatment of long bone defects in septic environments remains a challenge for any orthopedic surgeon. The two-stage reconstruction technique described by Masquelet AC is a better alternative in our regions where expertise in microsurgical techniques is rare. We report our first experience with this technique through the reconstruction of the humeral diaphyseal bone defect. We presented a 12-year-old boy diagnosed with chronic osteomyelitis of the left humerus with sequestrum, a pathologic fracture with overly joint involvement. The first stage consisted of a sequestrectomy removin
