B-type natriuretic peptide levels in patients with functionally univentricular hearts after total cavopulmonary connection.

AIMS: To assess plasma B-type natriuretic peptide (BNP) levels in patients with univentricular hearts late after volume unloading by total cavopulmonary connection (TCPC). METHODS: Plasma BNP was measured by sandwich immunoassay in 67 patients after a modified Fontan procedure. BNP levels were compared with age and sex-specific normal values, clinical and echocardiographic data, and results of exercise testing. RESULTS: BNP had a wide range of 5-290 pg/ml, but was normal in 81% of patients, and median BNP was only 13 pg/ml. There was no difference between males and females, between patients …

Hemodynamic profile after the Norwood procedure with right ventricle to pulmonary artery conduit.

BACKGROUND: The balance of systemic, pulmonary, and coronary blood flow after the Norwood operation for hypoplastic left heart syndrome (HLHS) is critical to early survival. We hypothesized that a right ventricle to pulmonary artery conduit (instead of a systemic to pulmonary artery shunt) would result in hemodynamic changes consistent with a more stable balance of systemic, pulmonary, and coronary perfusion. METHODS AND RESULTS: Hemodynamic data were obtained during cardiac catheterization before the hemi-Fontan procedure from 24 patients with HLHS; the first 10 had a Norwood operation with …

Augmentation of pulmonary blood flow with an axillary arteriovenous fistula after a cavopulmonary shunt.

Of 320 children with cyanotic congenital heart malformations who had previously undergone cavopulmonary shunt operations, 11 had increasing cyanosis and exercise intolerance and were considered unsuitable for definitive repair, a Fontan procedure, or other palliation. Eight had a previous Glenn shunt and three had a previous bidirectional cavopulmonary connection. To augment pulmonary blood flow, 10 patients underwent creation of an ipsilateral axillary arteriovenous fistula. Mean oxygen saturations were 80% +/- 2% before operation, 85% +/- 2% immediately after operation, and 84% +/- 3% at a m…

Risk Factors of Exercise Intolerance in Children and Adolescents After Total Cavopulmonary Connection.

(1) Background: Exercise intolerance is a nearly ubiquitous consequence among patients who have undergone the total cavopulmonary connection (TCPC) or modified Fontan procedure; however, the specific factors influencing this condition-especially the impact of surgical timing and perioperative management-remain inadequately understood. (2) Methods: From a retrospective cohort of 255 TCPC patients, 99 who had undergone cardiopulmonary exercise testing (CPET) were included. Patients were stratified into a normal exercise tolerance group (ET group, peak VO2 ≥ 80% predicted, n = 45) and an exercise…

Neo-Aortic Valve Insufficiency and Root Dilatation During and After the Staged Palliation for Hypoplastic Left Heart Syndrome and Variants.

In hypoplastic left heart syndrome (HLHS) and variants, the pulmonary valve functions as the neo-aortic valve after the Norwood procedure. Neo-aortic valve insufficiency (Neo-AI) represents an emerging complication with implications for long-term survival. This study investigated the clinical impact and patterns of Neo-AI development following staged palliation. Neonates undergoing the Norwood procedure (2001–2023) were retrospectively reviewed. Neo-AI was evaluated using serial echocardiograms, with patients classified into two groups: early onset (during the Norwood hospitalization) and late

Serial changes of mitral and tricuspid valve diameter after the Norwood procedure in patients with aortic atresia.

BACKGROUND: In hypoplastic left heart syndrome with aortic atresia, the non-functional left ventricle may influence right ventricular performance. We evaluated serial changes in tricuspid and mitral valve diameters after the Norwood procedure and their relationship to right ventricular function. METHODS: Of 150 patients who underwent the Norwood procedure (2001-2020), 47 died before total cavopulmonary connection. Of the 103 survivors, 76 with sufficient serial echocardiographic data were analysed (1,399 examinations). Tricuspid and mitral valve diameters were measured, and right ventricular

Pulmonary artery growth in Fontan: what is the most effective strategy?

INTRODUCTION AND OBJECTIVES: To model the growth of pulmonary artery (PA) branches in children with a functionally univentricular heart between the Glenn procedure and total cavopulmonary connection (TCPC) and to assess whether anterograde pulmonary blood flow (APBF) promotes growth and influences morbidity and mortality. METHODS: Patients with a functionally univentricular heart who underwent cardiac catheterization prior to Glenn and TCPC between 2004 and 2024 were included. Growth was assessed by comparing pre-Glenn and pre-TCPC cross-sectional areas. PA growth was modeled using linear reg

A shunt decision-making protocol in the surgical palliation of hypoplastic left heart syndrome from 2004 to 2016.

OBJECTIVES: The aim of this study was to study the impact of a decision-making protocol for shunt type in the Norwood procedure for hypoplastic left heart syndrome. Our cohort extends from 2004 to 2016. In era 1 (pre-2008), there was no policy for the choice of Norwood shunt. In era 2 (post-2008), a standard protocol was implemented. The right ventricle (RV)-to-pulmonary artery conduit was utilized for low-birth weight patients (<2.5 kg). The right modified Blalock-Taussig Shunt (RBTS) was constructed for normal birth weight patients. METHODS: The records of 133 consecutive operative patients

Postnatal pulmonary artery development from transcript to tissue.

Many congenital conditions and surgical interventions perturb haemodynamics in the proximal pulmonary arteries during postnatal development, thereby altering gene expression and associated changes in vascular structure and function. Among these, pathological conditions include patent ductus arteriosus, pulmonary atresia and stenosis and hypoxemia-induced pulmonary hypertension while surgical interventions include the placement of a Blalock-Thomas-Taussig shunt and the Glenn and Fontan procedures. Despite the significant morbidity associated with these diverse conditions, little attention has b…

Cardiovascular cavities cast in silicone rubber as an adjunct to post-mortem examination of the heart.

We used silicone rubber as a material for making post-mortem casts of cavities in 21 human hearts, 16 of them congenitally malformed, 13 after surgical reconstruction (Fontan and Norwood procedures) and 4 after reconstruction had been performed posts mortem for study purposes. The organs were either fresh, or had been perfusion fixed with formalin prior to casting. When suitable silicone rubber and catalyst were used, we found that casts allowed a very clear view of chamber morphology, and, where surgery had been performed, of any vascular constriction or distortion. We made casts of the aorti

A low threshold for neonatal intervention yields a high rate of biventricular outcomes in pulmonary atresia with intact ventricular septum.

AIMS: Management strategies for pulmonary atresia with intact ventricular septum are variable and are based on right ventricular morphology and associated abnormalities. Catheter perforation of the pulmonary valve provides an alternative strategy to surgery in the neonatal period. We sought to assess the long-term outcome in terms of survival, re-intervention, and functional ventricular outcome in the setting of a 26-year single-centre experience of low threshold inclusion criteria for percutaneous valvotomy. METHODS AND RESULTS: Retrospective analysis of patients diagnosed with pulmonary atr

[Congenital heart disease in adults: residua, sequelae, and complications of cardiac defects repaired at an early age].

Nowadays, it is estimated that 85% of the infants born with congenital heart disease (CHD) will survive to adulthood, thanks mainly to surgical or therapeutic procedures performed during infancy or childhood. The clinical profile and disease pattern of adults with CHD is changing. The prevalence of certain adult CHDs, such as tetralogy of Fallot, transposition of the great arteries or univentricular heart, is rising, but these conditions have practically become new diseases as a result of therapy. Most surviving patients present residua, sequelae, or complications, which can progress during ad
