Apophyseal and epiphyseal knee injuries in the adolescent athlete.

OBJECTIVES: In the context of pediatric sports injuries, the epiphyseal and apophyseal knee fractures represent rather peculiar lesions. The most frequently involved anatomical area is the knee. The peculiar function of the physis and the need to preserve their integrity, makes choosing what treatment methods to employ very important. Objective of this study is to assess the kind and the effectiveness of the most suitable treatment in the apophyseal and epiphyseal knee lesions occurring in the adolescents. MATERIALS AND METHODS: From 2006 to 2011, were treated 41 patients (34 M-7 F) between t

Risk of concomitant traumatic brain injuries in children with traumatic dental injuries in a pediatric emergency department: A case-control study.

BACKGROUND: Children with traumatic dental injuries (TDIs) may simultaneously sustain a traumatic brain injury (TBI). The aim of this study was to investigate the risk of concomitant TBI in children with TDIs. METHODS: Children (≤ 18 years) who sought treatment at the emergency department of a major children's hospital for TDIs from 2010 through 2019 were identified. Children with a concomitant TBI were assigned as case patients (TDI and TBI). Two control patients (TDI only) were randomly age- and sex-matched with each case patient. Associations between variables of TDI and concomitant TBI we

Mesiodens as a risk factor in treatment of trauma cases.

The presence of mesiodens often results in complications including retention of primary teeth and delayed eruption of permanent teeth, closure of eruption path, rotations, retention, root resorption, pulp necrosis, and diastema as well as nasal eruption and formation of dentigerous and primordial cysts. Less common complications involving the permanent incisors include dilaceration of the developing roots and loss of tooth vitality. Therefore, early diagnosis of mesiodens has particular importance in terms of preventing such complications. However, with respect to surgical removal of mesiodens

Orthodontic fine adjustment after vertical callus distraction of an ankylosed incisor using the floating bone concept.

The outcome of vertical callus distraction of a segment of tooth-supporting alveolar process might be functionally and esthetically unsatisfactory because of the unidirectional impact of intraoral distraction devices. In this case report, we describe how, with a shortened consolidation phase and application of the floating bone effect, the tooth-supporting osteotomy segment can be successfully aligned 3 dimensionally. We applied orthodontic force systems that went beyond the unidirectional vector preset by the mechanical properties of the distraction device.

Replantation: an analysis of 29 teeth.

The results of replantation of 29 teeth are described. Maxillary incisors constituted 85% of 27 permanent teeth. The period between avulsion and replantation ranged from within an hour to 86 days, but the actual extraoral period was within 5 hours in all but one case. Nineteen of the 27 permanent teeth were functioning for 7 months to 6 years and 7 months postoperatively. Root resorption was the cause of tooth loss in five cases. The incidence of root resorption seemed to increase with the increase of the extraoral period, but it was not affected by the period between avulsion and replantation

Radiological radio-carpal and mid-carpal motion after operative treatment of dorsal radio-carpal fracture dislocations.

PURPOSE: Radio-carpal fracture dislocations are rare severe injuries typically seen following high energy trauma. This injury is characterized by complete dislocation of the radio-carpal joint with avulsion fracture of dorsal or palmar cortical rim of the distal radius combined with disruption of the radio-carpal ligament complex. The purpose of our study is to assess the radiological motion outcome of the radio-carpal joint after Moneim type 1 radio-carpal fracture dislocations following surgical treatment. PATIENTS AND METHODS: Only Moneim type 1 injuries were included in this study. Eight

Surgical Management of Tibial Plateau Fractures: Current Concepts and Strategies.

Tibial plateau fractures are considered severe injuries of the lower extremity and require a differentiated treatment approach. The goal of surgical therapy is the anatomical reconstruction of the articular surface, while simultaneously restoring axial, length, and rotational alignment, in order to prevent long-term functional impairment and the development of post-traumatic gonarthrosis. A successful treatment is based on comprehensive diagnostics, including radiography, computed tomography, CT angiography in cases with suspected dislocation, and, if necessary, additional magnetic resonance i

Management Approach for Traumatic Complex Degloving Perineal Injuries: A Retrospective Review of 6 Cases.

OBJECTIVE: Degloving perineal injury is defined as avulsion of the perineal skin and subcutaneous tissue from its surrounding fascia, bone, and organs. This injury is most commonly caused by road vehicle accidents or alleged falls from heights. This devastating injury is associated with high morbidity and mortality if mismanaged. We retrospectively evaluated our management protocol for degloving perineal injury and the clinical outcomes of our case series. MATERIAL AND METHODS: Six cases of open perineal degloving injuries were managed and retrospectively reviewed at our center from 2018 to 2

A Medial Malleolar "Fleck Sign" May Predict Ankle Instability in Ligamentous Supination External Rotation Ankle Fractures.

BACKGROUND: Ankle joint stability dictates treatment in ligamentous supination external rotation ankle injuries (LSERAI). Investigation of the medial structures that support the ankle mortise is critical, and a small avulsion fracture, or "fleck", of the medial malleolus is occasionally encountered. This study aimed to assess the utility of this medial malleolus fleck sign (MMFS) in diagnosing instability requiring surgery in LSERAI. METHODS: This retrospective observational study examined 166 LSERAI at a single level I trauma center. A standardized diagnostic and treatment protocol for ankle

[Midterm results after treatment of injuries of the distal extensor mechanism with the MPT-fixateur. A critical analysis].

Between 1994 and 2000 at our institution 37 patients (8 women, 29 men) with fractures of the lower patellar pole and a proximal avulsion fracture of the patellar ligament underwent a primary surgical repair and patello-tibial fixateur for external protection. All patients underwent immediately after the reconstruction a functional rehabilitation program without any movement limitation and under early full weight bearing. Postoperative follow-up after an average of 58.5 months (range 14 to 102 months) was possible in 26 patients. Physical examination, roentgenographic evaluation and isokinetic

Reposition of intruded permanent incisor by a combination of surgical and orthodontic approach: a case report.

This report presents a case of a completely intrusive luxation of an immature permanent central incisor in a 7 years 9 months-old girl. Because there are severe intrusive trauma and cortical alveolar bone fracture, it was impossible to reposition with orthodontic or surgical method alone. The intruded tooth was repositioned to healthy alveolar bone level by using surgical extrusion and stabilization with sutures and periodontal pack. After healing of adjacent bone, the intruded maxillary central incisor erupted orthodontically by removable orthodontic appliance. It was moved from a high positi

Risk factors for displacement after closed reduction of greater tuberosity fracture associated with shoulder dislocation.

BACKGROUND: Greater tuberosity (GT) fractures associated with shoulder dislocation carry a high risk of secondary displacement, even when initial postreduction alignment appears acceptable. However, the specific factors predicting such displacement remain unclear. The aim of the study was to investigate risk factors associated with displacement of GT fragments following anterior shoulder dislocation. METHODS: This retrospective study included 52 patients (20 males, 32 females; mean age, 71.5 years) with anterior shoulder dislocation and concomitant GT fractures. Following closed reduction, pa

MRI as a reliable and accurate method for assessment of posterior hip dislocation in children and adolescents without the risk of radiation exposure.

BACKGROUND: Posterior hip dislocation in children and adolescents may involve the non-ossified posterior acetabular wall. Plain radiographs and computed tomography (CT) have been shown to underestimate injury to the unossified acetabulum as well as associated soft-tissue structures. OBJECTIVE: The purpose of this study was to describe findings on radiographs, CT and magnetic resonance imaging (MRI) after posterior hip dislocation in a series of adolescents and to report the intraoperative findings, which are considered the gold standard.
