Atypical Hepatocellular Neoplasm With Peliosis in Cirrhotic Liver Versus Hepatocellular Carcinoma: A Diagnostic Trap.

Atypical hepatocellular neoplasm (AHN) is an adenoma-like hepatic tumor that even occurs in noncirrhotic liver of males (any age) or females ≥ 50 years old, or associates focal atypical features. In this article, 2 unusual cases diagnosed in elderly cirrhotic patients, unrelated to steroids, are presented. The first case was incidentally diagnosed in an 83-year-old female. During laparoscopic surgery for cholecystectomy, hemoperitoneum was installed and laparotomy was necessary to remove a 70-mm nodular encapsulated hepatic tumor that was microscopically composed by hepatocyte-like cells with …

Liver transplantation as a rescue surgery after failure of embolization of a giant hepatic artery pseudoaneurysm.

We present the case of a male diagnosed with a giant hepatic artery aneurysm, which first presented with pain and hemorrhage due to a partial rupture of the aneurysm. After discarding treatment with a stent or surgery due to the wide extension, we chose to embolize the hepatic artery with coils. However, the progress was unfavorable after the procedure, with the appearance of liver failure that was resolved by an urgent liver transplantation. Giant hepatic artery pseudoaneurysms are an infrequent entity and their management is a great challenge. The diagnosis is usually delayed due to non-spec

[Splenic rupture secondary to abscess: Rare cause of pneumoperitoneum. Case report].

BACKGROUND: Splenic abscesses are rare entities; reports are commonly described in immunocompromised patients (72%) as: hematologic diseases, diabetes, endocarditis, acquired immunodeficiency syndrome, transplant patients and subjects who had abdominal trauma or splenic infarction. The main and most serious complication is the abscess rupture into the peritoneal cavity or adjacent organs (stomach or colon), which determines hemodynamic instability or septic state. CLINICAL CASE: Fifty-year-old man, who was admitted at Emergency Room due eight days' progressive, oppressive, and current pain; i

Spontaneous uterine rupture: report of two cases.

BACKGROUND: Uterine rupture is a rare entity with an incidence of 0.07% and is a cause of perinatal mortality. Dehiscence of previous uterine scar is the most important cause, but other causes have been reported such as placenta percreta. Almost 80% of uterine ruptures are spontaneous and its complications lead to an elevated mortality rate for the mother and the fetus. CLINICAL CASES: Case 1. We present the case of a 28-year-old female with no previous relevant medical history. The patient was 20 weeks pregnant with a diagnosis of uterus didelphys (double uterus) with lower placental implant

[Outcome and relevant factors of tubal pregnancy treated with laparoscopic conservative surgery].

OBJECTIVE: To investigate the therapeutic outcome and its influencing factors after laparoscopic conservative surgery in treatment of tubal pregnancy. METHODS: From January 2003 to December 2008, 226 cases with tubal pregnancy were treated by laparoscopic conservative surgery. The tubal patency was evaluated in 152 cases given by hysterosalpingography (HSG) and 6 cases given by second laparoscopic exploration at 3 - 6 months after surgery. In their first laparoscopic surgeries, 209 got successful treatment and 19 underwent fail treatment. At 3 - 6 months after surgery, 89 cases with tubal pat

Mechanisms of delayed splenic rupture: a new hypothesis.

A 46-year-old man was assaulted with a club to the face, chest and abdomen. On admission there were no signs of circulatory shock. The abdominal examination was unremarkable. During his stay in hospital he had light diets. He was managed symptomatically and discharged 5 days after the incident. Two hours after discharge he was admitted again with a complain of severe abdominal pain. Immediately after a heavy diet, he experienced this abdominal pain. On admission, he was pale with features of circulatory shock. There was a marked tenderness with rigidity and guarding on abdominal palpation. Wit

[Spontaneous haemoperitoneum due to a ruptured intra-abdominal varix with cirrhosis: report of two cases].

Spontaneous haemoperitoneum due to ruptured intra abdominal varices with cirrhosis is rare and the diagnosis is generally difficult. Two cases of spontaneous intraperitoneal bleeding due to ruptures varices with cirrhosis are reported. One case is a ruptured portal cavernome and the other one is a ruptured varix of the gastrosplenic ligament. The combination of hypovolemic shock with increasing abdominal girth should impose the diagnosis. This is a surgical emergency and the mortality rate is high.

The routine use of sonography in penetrating torso injury is beneficial.

BACKGROUND: Torso sonography (focused assessment with sonography for trauma [FAST]) has been added to our protocols for the evaluation of penetrating torso injury. The purpose of this study was to evaluate our recent experience and determine whether the use of FAST is beneficial. METHODS: From January 1999 to January 2000, patients with penetrating torso injury and no clinical indication for surgery were evaluated by sonography with a selective use of other investigations. FAST consisted of sonographic views of the peritoneum and/or pericardium to determine the presence or absence of fluid.

[Abdominal injuries in children. The value of emergency abdominal ultrasonography].

UNLABELLED: Abdominal injuries are frequent in children. Their early assessment is the best guarantee for an accurate management. PATIENTS AND METHODS: Seventy children with abdominal trauma were evaluated by early ultrasonography. RESULTS: This investigation found abnormalities in 52 patients, with hematic peritoneal effusion in 26 cases, renal lesion in 20 cases, hepatic lesion in ten cases, splenic lesion in seven cases and a lesion involving liver, spleen and pancreas in one case. Based on these results, surgery was performed in six cases. The remaining cases were simply supervised with

Significance of peritoneal fluid as an isolated finding on abdominal computed tomographic scans in pediatric trauma patients.

BACKGROUND: Peritoneal fluid on abdominal computed tomographic (CT) scan in the absence of solid-organ injury suggests a bowel injury. We sought to determine the significance of peritoneal fluid as the sole finding on abdominal CT scans obtained to evaluate injured pediatric patients. METHODS: We performed a retrospective review of abdominal CT scans obtained during the initial survey of blunt trauma patients less than 19 years old during a 5-year period (1991-1995). All patients received intravenous and oral contrast agents. All CT scans were read by a staff radiologist. All CT scan results

Hemorrhagic complications of severe pancreatitis.

Massive hemorrhage associated with pancreatitis is a rare but frequently lethal complication. Fifteen patients with this complication are presented. Bleeding occurred in four patients with necrotizing pancreatitis, in three patients with pancreatic abscesses, in seven patients with pseudocysts, and in one patient with chronic relapsing pancreatitis following longitudinal pancreaticojejunostomy. The initial presentation of hemorrhage was gastrointestinal in eight patients and retroperitoneal or intraperitoneal in seven. Abdominal pain with associated nausea and vomiting was present in all patie

Detection of occult abdominal trauma in patients with severe head injuries.

Among 109 consecutive victims of severe head injury, intra-abdominal injury could not be excluded in 87. These patients were considered to be at risk from occult intra-abdominal haemorrhage. 60 patients underwent diagnostic peritoneal lavage. 10 of these had a positive lavage, correctly indicating intra-abdominal haemorrhage. In each case a lesion requiring surgical repair was identified. 49 patients had a negative lavage, and only 1 of these patients subsequently showed signs of a missed intra-abdominal lesion. 13 patients had low systolic blood-pressure (< 90 mm Hg) on admission, and 4 of th

[Strategies for paediatric spleen and liver injuries].

Traumatic injuries of the spleen and liver are typically caused by age-related falls or sports and traffic accidents. Today, the non-operative management for isolated injuries is established and evidence-based guidelines are available. The intact abdominal wall and the limited space within the peritoneum produce a compression which is the pathophysiological explanation for the limitation of the haemorrhage. Precondition for the non-operative therapy is the radiology-based classification of the injury (organ injury scale) and a haemodynamically stable patient. Haemodynamic stability is, if nece

Sonography of lower uterine segment thickness and prediction of uterine rupture.

BACKGROUND: Sonographic evaluation of the lower uterine segment was undertaken to study the degree of thinning and, thus, to predict uterine rupture. However, the best measuring technique and recommended cutoff values remain controversial. CASE: Sonographic evaluation of the lower uterine segment at 36 weeks of gestation in a 31-year-old patient with prior low transverse cesarean delivery revealed a full thickness of 3.6 mm and a myometrial layer of 1.1 mm. Nevertheless, the patient experienced a large uterine rupture during a trial of labor at term. CONCLUSION: In this case, there was a dis

Postoperative hepatic artery aneurysms development and remodeling in Ehlers-Danlos syndrome type IV. Case report.

Patients affected by Ehlers-Danlos syndrome (EDS) type IV are at risk for aneurysm formation and rupture. This case report shows the extreme vascular fragility of these patients. We studied a 31-year-old man that developed hepatic artery aneurysms 3 weeks after splenectomy. Computed tomography angiography showed the extreme vascular remodeling of the aneurysms. We conclude that remote site complications should be kept in mind by all surgeons in vascular EDS patients even after general surgery operations.
