Comparing drug-related hospital morbidity following heroin dependence treatment with methadone maintenance or naltrexone implantation.

CONTEXT: Most research on heroin dependence treatments assesses short-term changes in patients' self-reported drug use. To our knowledge, long-term sustainability of changes in patients' drug use and associated hospital morbidity posttreatment have not been studied. OBJECTIVES: To evaluate drug-related hospital morbidity in heroin users at 6 months and 3 1/2 years after receiving naltrexone implant treatment and to compare these results with outcomes from a similar cohort treated with methadone maintenance treatment. DESIGN: Retrospective longitudinal follow-up, using data prospectively coll

From A to D: A Unique Case Report of Recovery After Longitudinal Myelitis Related to Lupus.

Longitudinal myelitis secondary to an acute flare of systemic lupus erythematosus has been reported in the literature. There have been few published cases of complete functional recovery in patients with systemic lupus erythematosus-related longitudinal myelitis (systemic lupus erythematosus-related longitudinal myelitis). Of those cases, none have described in detail the rehabilitation course of treatment. In the current case, intensive rehabilitation was coupled with aggressive pharmaceutical treatment resulting in almost full functional recovery. A 23-yr-old African American woman with a hi

ICD Implantation Rates in Cardiac Arrest Survivors in Canada.

BACKGROUND: Patients resuscitated from out-of-hospital cardiac arrest (OHCA) are at high risk of recurrence, posing a substantial burden on healthcare systems. Despite the established benefit of implantable cardioverter defibrillator (ICD) therapy in many such patients, and recommendations by guidelines, few studies have described the proportion of OHCA patients who receive guideline-concordant care. METHODS: The Canadian Institute for Health Information Discharge Abstract Database dataset was used to identify OHCA patients admitted to hospitals across Canada, excluding Quebec. We analyzed al

Spectrum of Microbial Diseases and Resistance Patterns at a Private Teaching Hospital in Kenya: Implications for Clinical Practice.

BACKGROUND: Accurate local prevalence of microbial diseases and microbial resistance data are vital for optimal treatment of patients. However, there are few reports of these data from developing countries, especially from sub-Saharan Africa. The status of Aga Khan University Hospital Nairobi as an internationally accredited hospital and a laboratory with an electronic medical record system has made it possible to analyze local prevalence and antimicrobial susceptibility data and compare it with other published data. METHODS: We have analyzed the spectrum of microbial agents and resistance pa

[Neonatal morbidity and mortality in a low-resource urban district hospital of Douala, Cameroon].

INTRODUCTION: In countries with limited health-care resources, achieving the fourth Millennium Development Goal recommended by the WHO requires the reduction of neonatal mortality. Interventions at different levels of the community and the health-care system are needed, including in district hospitals. METHOD: This was a descriptive study in the neonatal unit of the District Hospital of Bonassama/Douala in Cameroon that analyzed neonatal mortality and morbidity to discover possible intervention levers. The clinical, sociodemographic, and outcome data of hospitalized newborns were recorded fro

The Copenhagen Model: Advancing multidisciplinary strategies for dysphagia and supportive care after head and neck cancer.

PURPOSE OF REVIEW: This review highlights the growing need for coordinated, multidisciplinary survivorship models for individuals treated for head and neck cancer (HNC), as modern treatments have improved survival for the group but also led to a higher prevalence of complex, multisystem late sequelae. The importance of authorities and academic societies in incorporating rehabilitation as part of treatment in national cancer plans is reviewed. As healthcare systems strive to integrate hospital-based and community-based services, the balance between centralisation of specialisation and the desir

Structured reporting of B-mode, color Doppler, and CEUS in testicular tumor assessment: a reader study with urologist ratings.

PURPOSE: Structured reporting (SR) offers standardized radiological documentation, enhancing clarity and reproducibility. However, its role in contrast-enhanced ultrasound (CEUS) for testicular tumors remains underexplored. This study evaluates urologist-perceived clarity, completeness, and clinical usefulness of SR compared to free-text reporting (FTR). METHODS AND MATERIALS: In this retrospective, single-center study, 65 male patients with suspected testicular tumors underwent CEUS at LMU University Hospital. Reports were initially documented as FTRs by an experienced radiologist and later c

Adherence to Guidelines in Orthopaedic Operation Notes: A Quality Improvement Initiative.

BACKGROUND: Orthopaedic operation notes are crucial documents that record the specifics of surgical procedures performed on the musculoskeletal system. They play a vital role in ensuring clear communication between peri-operative and post-operative periods, maintaining patient care and safety, and serving as legal documents. However, studies have consistently shown gaps in the documentation of important parameters. OBJECTIVE: This study aimed to compare current practices in documenting orthopaedic operation notes with changes in these practices after awareness training and the implementation

Phenomenological Study on the Lived Experience of Patients After Major Cardiac Surgery in Intensive Care Units.

BACKGROUND: Intensive care unit (ICU) experiences after cardiac surgery significantly influence patient outcomes, including psychological disorders, quality of life and overall comfort. The existing literature emphasises psychological impacts and transitions from ICU to general wards, but there is a paucity of qualitative research focussing specifically on post-cardiac surgery ICU experiences. AIM: This study aimed to explore the lived experiences of patients in the ICU following major cardiac surgery, providing a comprehensive understanding of their emotional and psychological challenges. Disruptions to HIV services due to the COVID pandemic in the USA: a state-level stakeholder perspective.

BACKGROUND: The United States envisions a 90% reduction in HIV infections by 2030. However, the COVID-19 pandemic disrupted the HIV continuum and disproportionately affected access to social and health services for people at the highest vulnerability. This study shows how stakeholders in the State of Michigan handled disruptions and their key recommendations. As a case study, this study adds to the literature about preparedness for future pandemics. METHODS: We interviewed 33 statewide Michigan HIV/AIDS Council members-practitioners, researchers, and community representatives, guiding service

Long-Term Clinical Outcomes Following Nonsurgical Management of Salter-Harris Type II Fractures of the Proximal Phalanx of the Small Finger: A Prospective Cohort Study.

PURPOSE: Juxta-physeal fractures at the base of the proximal phalanx (FBPP) of the small finger are one of the most common hand fractures in children. Although many of these fractures are treated nonsurgically, it is unclear which fractures benefit from surgery or the degree of acceptable angulation appropriate for nonsurgical management. Our study aimed to assess long-term, patient-reported outcomes regarding function, appearance, and pain after nonsurgical management of FBPP of the small finger in a pediatric population. METHODS: Our hospital Picture Archiving and Communication Systems data

The chaos of hospitalisation for patients with critical limb ischaemia approaching major amputation.

AIMS AND OBJECTIVES: To illuminate the hospital experience for patients and families when major amputation has been advised for critical limb ischaemia (CLI). BACKGROUND: CLI creates significant burden to the health system and the family, particularly as the person with CLI approaches amputation. Major amputation is often offered as a late intervention for CLI in response to the marked deterioration of an ischaemic limb, and functional decline from reduced mobility, intractable pain, infection and/or toxaemia. While a wealth of clinical outcome data on CLI and amputation exists internationall

The Critical Role of Early Dengue Surveillance and Limitations of Clinical Reporting - Implications for Non-Endemic Countries.

The increasing dengue burden and epidemic severity worldwide have highlighted the need to improve surveillance. In non-endemic areas such as Taiwan, where outbreaks start mostly with imported cases from Southeast Asia, a closer examination of surveillance dynamics to detect cases early is necessary. To evaluate problems with dengue surveillance and investigate the involvement of different factors at various epidemic stages, we investigated 632 laboratory-confirmed indigenous dengue cases in Kaohsiung City, Taiwan during 2009-2010. The estimated sensitivity of clinical surveillance was 82.4% (5
