Twelve patients with high-output enterocutaneous fistulae were successfully managed by interventional radiologic therapy. Control of the bowel effluent was gained by anchoring a large-bore T tube in the lumen of the bowel and placing sump drains adjacent to the bowel wall. Abscesses communicating with the fistulous tract were also catheterized and drained. This approach afforded control of sepsis and allowed the formation of a mature, fibrous external tract that subsequently underwent spontaneous closure.

Colonic diverticulitis is frequently complicated by bowel obstruction, abscess, perforation and fistula formation. We report a unique case of colonic diverticulitis complicated by colovenous fistulization with Gastrografin enema examination demonstrating thrombus in the inferior mesenteric vein.

A huge pancreatic pseudocyst which nearly filled the entire abdomen ruptured into the sigmoid colon, and caused perplexing clinical and radiographic findings. Because of the thickness of its wall, the pseudocyst did not collapse but became filled with intestinal gas. The more common features of pseudocysts that rupture are discussed.

Two patients with appendicovesical fistulae are described. Both presented with gastrointestinal and urinary symptoms. Biochemical examination showed hypokalaemic hyperchloraemic metabolic acidosis; radiographic investigations showed a vesical stone and a communication between the bladder and the bowel. The findings at operation suggested that the fistulae may have arisen as complications of previous appendicitis. The diagnosis of appendicovesical fistulae may be difficult but should be considered in the presence of hypokalaemic hyperchloraemic metabolic acidosis and vesical stones.

Two cases of gastro-colic fistula occurring in analgesic abusers are described. In both patients, the fistulous communication was diagnosed at endoscopy and subsequently verified by upper gastrointestinal tract barium studies. Gastro-colic fistula is a rare complication of benign peptic ulcer disease. Whilst salcylates and cortico-steroids have been implicated as aetiological agents, abuse of compound analgesics has not previously been reported. Since the first description of gastro-colic fistula in 1755, thirty documented cases associated with benign gastric ulcer have been reported. Most oft

A 69-year-old woman had a large gallstone eroded through the wall of the gallbladder and into the duodenum. The gallstone became fixed within the cholecystoduodenal fistula and produced a duodenal obstruction. At the time of the diagnosis of this problem, the patient had had a three-week period of epigastric pain and prolonged vomiting. She was treated by removal of the stone and cholecystectomy. The duodenal fistula was closed in two layers, and a gastrojejunostomy and a truncal vagotomy were carried out to protect the duodenal suture line. A leak from the duodenal closure developed on the fo

Despite ileal conduit diversion for urinary incontinence, an adolescent male with meningomyelocele continued to void per urethram postoperatively. Urologic investigation disclosed spontaneous fistulization from a ureteroileal anastomosis to a ligated ureteral stump, with resultant urinary drainage to the bladder. This unusual complication of ileal conduit diversion has not been reported previously. More attention should be directed to securing a watertight ureteroileal anastomosis. A careful single layer anastomosis is recommended to minimize urinary leakage without increasing the risks of ste

We report a case of gastrocolic fistula resulting from a benign gastric ulcer, diagnosed by barium meal and endoscopy. The fistula healed with conservative management and treatment with carbenoxolone sodium. To our knowledge this is the first reported case of successful conservative treatment of a benign gastrocolic fistula using carbenoxolone.

Twenty-nine anastomotic leakages of the gastrointestinal tract were treated by long-term parenteral nutrition; 20 closed spontaneously, the lethality was 18.3%. Especially, well-drained fistulas of the distal small bowel and the colon show a tendency to closure and should be treated by long-term parenteral hyperalimentation (2500-4000 KCal/24 h), if no signs of ileus or diffuse peritonitis are apparent. The technique and chance for success of hyperalimentation are discussed.
