Medial Patellofemoral Ligament Reconstruction With or Without Tibial Tubercle Osteotomy in Carefully Selected Patients Results in a 5% Revision Rate: A Preliminary Analysis.

PURPOSE: The purpose was to assess the outcomes of medial patellofemoral ligament (MPFL) allograft reconstruction with or without tibial tubercle osteotomy (TTO) in adolescents, with a focus on evaluating demographic and imaging characteristics on outcomes. METHODS: A single-institution retrospective study was performed of patients ages 12 to 19 years who underwent MPFL reconstruction +/- TTO for the treatment of lateral patellar instability. Demographic, clinical, surgical, and postoperative information was collected. All x-ray and magnetic resonance imaging measurements were completed indep

[Reconstruction of the medial patellofemoral ligament with quadricipital tendon autograft].

INTRODUCTION: Alterations of the patellofemoral joint are one of the most common causes of pain and inflammation as well as joint damage and instability of the knee. Patellofemoral instability is a common multifactorial entity, requiring realignment by proximal, distal, or a combination of techniques. Within the proximal procedures in soft tissues, is the medial patellofemoral ligament plasty (MPFL), which aims to realign the patella medially and restore the anatomy between the quadricipital tendon, patella and tubercle of the tibia. OBJECTIVE: To demonstrate that the plasty of the LPFM with

Instability in primary total knee arthroplasty.

Instability is one of the most common causes of failure of total knee arthroplasty (TKA). The presentation can vary from pain to frank dislocation with the etiologies just as varied. Instability after TKA can be classified by where the instability occurs in the knee's arc of motion as well as the chronicity of the problem. Acute instability is related to intraoperative injuries or excessive release of important coronal stabilizers such as the medial collateral ligament in extension or the posterolateral corner in flexion. Chronic instability in extension is often related to varus/valgus malali

Patellar tracking during simulated quadriceps contraction.

The current study compared patella tracking during simulated concentric and eccentric quadriceps contractions in 12 knees from cadavers using a three-dimensional electromagnetic tracking system. The patella shifted (translated) and tilted medially during approximately the initial 22 degrees tibiofemoral flexion. The patella then shifted and tilted laterally for the remaining arc of tibiofemoral flexion (90 degrees). At 90 degrees tibiofemoral flexion, the patella had an orientation of lateral patella shift and lateral patella tilt. Patella shift was significantly more lateral between 40 degree

Paediatric physeal sparing posterior cruciate ligament (PCL) reconstruction with parental donation allograft: Rationale and operative approach.

BACKGROUND: Paediatric PCL injuries are rare but constitute a significant management challenge. We describe a novel approach to the surgical management of an 11-year-old boy who presented with persisting symptomatic instability following 18months of failed conservative therapy. METHODS: PCL reconstruction was performed using a physeal sparing, all-inside technique under fluoroscopic control. This avoids the potential for iatrogenic growth injury. A parentally donated hamstrings allograft was used to ensure adequate graft size, and reinforced using a non-elastic two millimetre braided suture.

An uncommon midfoot injury, naviculocuneiform and calcaneocuboid fracture dislocation: a case report.

A 57-year-old man fell from a height of 6 m and injured his right foot. Imaging studies showed an uncommon injury; naviculocuneiform and calcaneocuboid joint fracture dislocations. He underwent a temporary fixation with Kirschner wires (K-wires), and the injured foot was immobilized with a below-knee splint. Weight-bearing was started gradually. The K-wires were removed at 8 weeks. Full weight-bearing was achieved 14 weeks after the operation. At 19 weeks, he returned to his carpentry job. At 1 year, he had no marked limitation of daily activities. Early recognition of these injuries is requir

Treatment of combined anterior cruciate ligament-posterior cruciate ligament-lateral side injuries of the knee.

ACL-PCL-posterolateral corner injuries most frequently are seen in multiple trauma patients but do occur in the athletic injury population. Acute three-ligament-injured knees may have been tibiofemoral dislocations with spontaneous reduction in the field. Careful documentation of the neurovascular status is essential in these cases to avoid the complications associated with limb ischemia. Systematic evaluation of these patients with history, physical examination, imaging studies, examination under anesthesia, and diagnostic arthroscopy will aid in the correct diagnosis and treatment plan formu

[A case of Larsen syndrome with severe cervical cord compression].

We reported a case of Larsen syndrome with cervical cord compression. She had a flattened face, bilateral joint dislocations of the elbows, hips and knees, and equinovalgus deformity on the feet. At first she had flaccidity of the upper and lower limbs, she gradually developed spastic. On laboratory examination, CSF protein was elevated. EMG showed fibrillation. In short latency somatosensory evoked potential (SSEP), N 20 was not detected. MRI showed a severe cervical cord compression. Cervical spine deformity has been often described in the previous reports on Larsen syndrome, but cervical co

[Injuries of the cruciate ligament in children caused by sports accidents].

From 1982 to 1990, 87 knee arthroscopies were performed on children aged 13 years and younger, following post-traumatic haemarthrosis. Damage to anterior and posterior cruciate ligaments was found in more than 1/3 of these cases. The injury to the actual anterior cruciate ligament occurred more often than the injury associated with avulsion fracture of the tibial eminences. It was found that almost all of the ruptures to the actual anterior and posterior cruciate ligaments occurred proximally. Hence, if haemarthrosis occurs in children, arthroscopy should be performed as a diagnostic and thera

Prospective, Long-Term Functional Outcomes of Extra-Osseous Talotarsal Stabilization (EOTTS) Using HyProCure in Adult Patients with Talotarsal Joint Instability: Assessment of Physical Activity and Patient Satisfaction.

BACKGROUND: The partial dislocation of the talus from the calcaneus and navicular bones is a primary factor leading to a prolonged overpronation during weightbearing. This study aimed to assess the possibility of returning to physical activity and long-term patient satisfaction after an extra-osseous talotarsal stabilization (EOTTS) procedure with a HyProCure sinus tarsi implant for partial talotarsal joint dislocation (TTJ). METHODS: A total of 41 adult patients (61 feet), with an average age of 46.41, were included and treated surgically with EOTTS as a stand-alone surgery. Physical activit

Recurrent patellar dislocation: reappraising our approach to surgery.

INTRODUCTION: The management of recurrent traumatic patellar dislocation includes surgical realignment. There is no clear distinction whether proximal soft tissue or distal procedures produce superior results. However, distal realignment procedures are commonly associated with greater morbidity. We advocate a distal procedure only for cases which are more severe, such as the presence of patellar maltracking. MATERIALS AND METHODS: Between January 2002 and June 2007, all patients who had a history of traumatic patellar dislocation with recurrent symptoms and failed conservative management unde

Outcomes of isolated medial tibial plateau fractures by fracture morphology.

PURPOSE: To identify a cohort of isolated medial tibial plateau fractures treated with surgical fixation and to categorize them by Moore and Wahlquist classifications in order to determine the rate of complications with each fracture morphology and the predictive value of each classification system. We hypothesized there would be high rates of neurovascular injury, compartment syndrome, and complications overall with a higher incidence of neurovascular injury in Moore type III rim avulsion fractures and Wahlquist type C fractures that enter the plateau lateral to the tibial spines. METHODS: P

Predicting patellar kinematics and contact forces after TKA: a simulation study on quadriceps malalignment.

BACKGROUND: Patellofemoral complications remain a frequent cause of dissatisfaction following total knee arthroplasty (TKA), despite continuous advances in implant design and surgical techniques. Abnormal patellar tracking may lead to increased contact stresses, instability, and anterior knee pain, particularly in patients with preoperative quadriceps malalignment. Subject-specific assessment tools are therefore needed to improve surgical planning and postoperative outcomes. METHODS: A subject-specific multibody dynamics (MBD) framework was developed to simulate patellofemoral mechanics follo

When the Going Gets Tough: A Review of Total Hip Arthroplasty in Patients with Ipsilateral Above- and Below-Knee Amputation.

Life expectancy and overall function of amputated patients have improved significantly over the last few decades; for this reason, amputees are more exposed to primary or secondary degenerative disease of the hip, requiring total hip arthroplasty (THA) surgery. However, during training, not all the surgeons acquire adequate skills to manage these patients, and only a few studies and case reports describe technical pearls and outcomes of THA in patients with ipsilateral lower limb amputation, either above or below the knee. The objective of this narrative review is to present current evidence a
