In motion, out of place: the public space(s) of Tourette Syndrome.

According to Doris Humphrey, "Nothing so clearly or inevitably reveals the inner man [sic] than movement and gesture...the moment you move you stand revealed, for good or ill, for what you are" (The dance notebook, 1984). Tourette Syndrome, characterized by motor and vocal tics, elicits stigma for just this reason: the tics and movements it causes have revealed Touretters as something beyond the 'normal.' This paper examines the ways Touretters are perceived and received in public spaces, using published illness narratives and film documentaries to address the question of why the reactions TS

Effect of selective capsulorrhaphy on the passive range of motion of the glenohumeral joint.

BACKGROUND: Capsulorrhaphy of the glenohumeral joint is a common surgical procedure for the treatment of instability caused by increased capsular laxity. The effect of capsulorrhaphy on the range of motion of the shoulder is poorly understood. METHODS: We simulated localized capsular contractures by selective capsular plications in eight human cadaveric shoulders and studied the effect of such plications on the passive range of glenohumeral abduction, flexion, and external and internal rotation in different degrees of abduction. A 0.5 or 1-N-m torque was applied to the humerus, and the range …

The relation of motion sickness to the spatial-temporal properties of velocity storage.

Tilting the head in roll to or from the upright while rotating at a constant velocity (roll while rotating, RWR) alters the position of the semicircular canals relative to the axis of rotation. This produces vertical and horizontal nystagmus, disorientation, vertigo, and nausea. With recurrent exposure, subjects habituate and can make more head movements before experiencing overpowering motion sickness. We questioned whether promethazine lessened the vertigo or delayed the habituation, whether habituation of the vertigo was related to the central vestibular time constant, i.e., to the time con…

Standardisation of the description of patellofemoral motion and comparison between different techniques.

Patellofemoral motion is significant clinically, yet in the literature many different methods and terminologies are used, thus making comparison between studies difficult. We review and explain the different methods used for the description of patellofemoral joint motion, compare these methods by experimentation, and propose a standardised method. We found three main methods for describing patellar motion: motion of the patella about femoral body fixed axes, about patellar body fixed axes, and a combination of these. Description about femoral body fixed axes does not make sense clinically. Des…

Study of marker placements in the back for opto-electronic motion analysis.

Various investigations into anatomical landmarks that could be employed in spine and back surface measurement have highlighted the usefulness of the spinous processes of the vertebra and the posterior superior iliac spines of the pelvis. Earlier studies used an opto-electronic gait analysis system to examine the motion of skin markers and compared results with similar inter-vertebral movement recorded through radiographs. Consistent patterns of movement suggested a relationship between spinal and back surface motion. Further investigations into the use of opto-electronic gait analysis systems …

Opioid receptor involvement in the adaptation to motion sickness in Suncus murinus.

The aim of the present study was to investigate an opioid receptor involvement in the adaptation response to motion sickness in Suncus murinus. Different groups of animals were treated intraperitoneally with either saline, morphine (0.1 and 1.0 mg/kg), naloxone (1.0, 10.0 and 5.0 mg/kg) or a combination of naloxone plus morphine in the absence or 30 min prior to a horizontal motion stimulus of 1 Hz and 40 mm amplitude. For the study of adaptation, different groups received saline on the first trial, and in subsequent trials (every 2 days) they received either saline, naloxone (1.0 and 10.0 mg/…

A novel constrained acetabular liner design with increased range of motion.

One treatment strategy for recurrent dislocation after total hip arthroplasty is the use of a constrained acetabular component. A major drawback of currently available constrained components is the limited range of motion (ROM). We present a novel constrained component that employs a monopolar, highly cross-linked polyethylene liner with cutouts oriented to allow increased ROM, while maintaining constraint against dislocation. ROM and lever-out tests comparing this novel design with a conventional constrained component showed that the cutaway monopolar component allowed ROM substantially great…

Using cineradiography for continuous dynamic-motion analysis of the lumbar spine.

STUDY DESIGN: Cineradiography was used to analyze continuous dynamic motion in the lumbar spine. OBJECTIVES: To identify motion patterns of the lumbar spine in asymptomatic volunteers and symptomatic patients with L4 degenerative spondylolisthesis, and to use the findings to discuss segmental instability in this disorder. SUMMARY OF BACKGROUND DATA: The use of radiographic findings to assess lumbar spine instability remains controversial. Although some studies have reported on lumbar kinematics during actual movement, the motion patterns in asymptomatic volunteers and symptomatic patients wi…

Mechanisms of simple and choice reaction to changes in direction of visual motion.

Experiments are presented in which a random dot pattern moved vertically upwards (velocity vector V(1)) and then abruptly changed its direction of motion by the angle alpha (velocity vector V(2)), either to the left or to the right, without changing the speed. Subjects performed simple reactions to the direction change, disregarding its sign. In another experiment choice reactions to the same stimuli were performed: the subjects pushed a left button when the direction change was to the left and a right button when the change was to the right. The simple reaction time decreased monotonically wi…

Postural instability and motion sickness in a fixed-based flight simulator.

We evaluated the prediction that postural instability would precede the subjective symptoms of motion sickness in a fixed-base flight simulator. Participants sat in a cockpit in a video projection dome and were exposed to optical flow that oscillated in the roll axis with exposure durations typical of flight simulation. The frequencies of oscillation were those that characterize spontaneous postural sway during stance. Head motion was measured prior to and during exposure to imposed optical flow. Of 14 participants, 6 were classified as motion sick, either during or after exposure to the optic…

Tissue motion--a disturbance in the laser-Doppler blood flow signal?

The tissue matrix in the human body is continuously in motion driven by forces generated by the circulatory and respiratory systems. The linear velocities recorded can be as high as 300 microm/s. The question raised: is the motion of tissue matrix generating an error signal in laser-Doppler (LD) recordings that is misinterpreted as solely a blood flow signal. The problem is studied by using a physical model demonstrating the contribution from a flow channel and, in addition, from a linear motion of the flow tube. Measurements of skin motion in the forearm have been performed. The corresponding…

Assessment of carotid wall motion and stiffness with tissue Doppler imaging.

In this study, the wall motion of the common carotid artery was characterized by measuring wall-motion velocity (Wv) with tissue Doppler imaging (TDI) in 78 male and female subjects (16-75 y) with no history of cardiovascular disease. The near and far arterial wall showed essentially different Wv patterns. To assess the vascular systolic distension, the Wv of the near and far arterial walls were measured and a Wv index was calculated by subtracting the far Wv from the near Wv. Aging was associated with a 2.0-2.5-fold decrease of peak Wv index. Corrected for carotid diameter and blood pressure,…

[A study on central and peripheral nerve transmitter mechanism of motion sickness].

Model of motion sickness (MS) was induced in rats by accelerating and decelerating vertical rotation for 60 min. The contents of Ach, NA, A and 5-HT and the activity of AchE in brain and blood of rats were examined after 60 min of rotation. The results showed: NA contents and AchE activity decreased significantly while A and 5-HT increased in rat's brain. Levels of Ach, NA and A in blood were increased. The results suggested that MS may be related to the imbalance of transmitters in the central nervous system resulting in the reduction of central arousal, and the imbalance between sympathetic …

Evaluating the motions of a semi-submersible platform with respect to human response.

The motions of a semi-submersible drilling platform have been evaluated so as to predict the effects on the comfort and activities of the crew. The horizontal motions at the drill floor exceeded the 'average threshold of perception' defined in International Standard 6897 (ISO 6897, 1984) by more than a factor of two; they were about half of the limit for the worst 10 min in five years for 'fixed offshore structures where work of a somewhat critical nature is carried out'. Other standards predict that the vertical motion would cause vomiting due to motion sickness in less than 5% of unadapted a…

Spectrum of presentation of paradoxical vocal cord motion in ambulatory patients.

BACKGROUND: Paradoxical vocal cord motion (PVCM) frequently masquerades as asthma. Atypical presentations of asthma or situations in which asthma does not respond to traditional therapies mandate the consideration of PVCM in the differential diagnosis. OBJECTIVE: Evaluate the demographics and spectrum of presentation of PVCM in an ambulatory outpatient population. METHODS: Retrospective survey of medical records of 164 consecutive patients (86 males, 78 females) who underwent fiberoptic rhinolaryngoscopy over a 3-year period. RESULTS: Twenty patients (16 females, 4 males) with PVCM diagnose…

In vivo mitral valve morphology and motion in mitral valve prolapse.

Mitral leaflet morphology in mitral valve prolapse (MVP) has been suggested to be prognostically important, but in vivo valvular morphology is incompletely described in patients with MVP or in normal subjects. Accordingly, the length of both mitral leaflets and their zone of apposition, the thickness of their rough and clear zones, diastolic and systolic mitral annular diameters, and indexes of abnormal leaflet motion were measured in the parasternal long-axis echocardiographic view in 100 patients with MVP and 100 age- and sex-matched normal subjects. In both groups posterior leaflet thicknes…

Effects of motion sickness and antimotion sickness drugs on gastric function.

This study examined the effects of motion sickness and antimotion sickness drugs on gastric emptying (GE). Drugs were tested in normal and motion sick subjects. To induce motion sickness, subjects performed head movements while seated in a rotating chair. Gastric emptying of liquid (300 mL) was determined by nuclear medicine techniques, whereas gastric electrical activity, the electrogastrogram (EGG), was monitored from surface (cutaneous) electrodes positioned over the abdominal area. Gastric emptying was severely inhibited at the peak of motion sickness symptoms, but returned to normal 15 mi…

Open hand fractures: an analysis of the recovery of active motion and of complications.

Seventy-five of 104 patients who underwent operative fixation of open hand fractures were reviewed between 6 months and 7 years after injury (average, 17 months). There were 140 fractures involving 125 fingers. Results, evaluated on the basis of total active range of digital motion achieved at final follow-up, correlated highly with severity of soft tissue injury. When open fractures of comparable severity were contrasted between groups that did and did not require additional extension by incision to achieve acceptable reduction and stabilization, there was some additional loss of active range…
