[Orbital blow-out fractures, a series of 12 operated cases].

BACKGROUND: The management for blow-out fractures is controversial. Some studies suggest early surgical treatment, others a conservative attitude. PATIENTS AND METHODS: We studied retrospectively the data of patients with blow-out fracture, referred to our Neuro-Ophthalmology and Strabismus Clinic from July 1993-May 2000. Out of 48 patients, 22 were operated. We evaluated all 12 patients who underwent pre- and postoperative orthoptic examinations. RESULTS: The patients' age at the time of the accident was 7.8 to 67.8 years (median 25.3). The delay between accident and operation was 2 to 91 d

Abnormal vertical optokinetic nystagmus in infants and children.

AIMS: To determine if testing vertical optokinetic nystagmus (VOKN) has a role in the clinical assessment of infants and children. METHODS: A large field projection system was developed with which optokinetic nystagmus (OKN) could be stimulated in any direction. Gross abnormalities in the response were detected simply by observation. RESULTS: VOKN was tested in 144 children using this OKN projection system. 26 of these children had abnormal VOKN; 13 had a vertical saccade initiation failure "ocular motor apraxia" (in either direction, up/down, or in both) and 13 had absent VOKN (in either di

Children with ocular motor apraxia type Cogan carry deletions in the gene (NPHP1) for juvenile nephronophthisis.

Congenital ocular motor apraxia type Cogan is characterized by impairment of horizontal voluntary eye movements, ocular attraction movements, and optokinetic nystagmus. Two patients with congenital ocular motor apraxia type Cogan exhibited a newly recognized association with nephronophthisis type 1, an autosomal recessive kidney disease. Both patients possess large deletions of the NPHP1 gene. The deletion occurred on both chromosomes 2q13 in one patient and heterozygously in combination with a point mutation of the NPHP1 gene in the other. The findings will help to elucidate the pathogenetic

[Opsoclonus-myoclonus syndrome in the adult. 4 cases].

Four cases of opsoclonus in adults are reported. In all cases, it was associated with myoclonus and cerebellar syndrome. In 2 cases, the clinical and biological picture was suggestive of an infectious origin, and the patients recovered. The last cases are paraneoplastic opsoclonus revelating a pulmonary carcinoma and a breast cancer. The two patients died from metastasis after a transient improvement of ocular disorder. The pathophysiology of opsoclonus is discussed.

Nystagmus in periventricular leucomalacia.

BACKGROUND/AIMS: Periventricular leucomalacia (PVL) is a lesion in the immature brain involving the optic radiation. Children with PVL have visual problems including crowding, visual field defects, strabismus, and visual perceptual/cognitive deficits, together with nystagmus. They often have optic nerve hypoplasia seen either as small discs or as large cupping of normal sized optic discs. This study aimed to perform eye movement recordings in a group of children with PVL in order to characterise and classify the nystagmus. METHODS: 19 children with PVL on cerebral imaging underwent eye moveme

Endoscopic repair of posttraumatic enophthalmos using medial transconjunctival approach: a case report.

A blowout fracture of the medial orbital wall should be suspected when periorbital trauma results in epistaxis, orbital hemorrhage, horizontal dysmotility or dystopia of the globe, and/or orbital emphysema. Large medial orbital wall blowout fractures are frequently complicated by posttraumatic enophthalmos. Clinicians should consider a medial transconjunctival approach for repair of these fractures when surgical repair is indicated by a comprehensive clinical and radiologic orbital evaluation. Excellent cosmetic and functional results can be achieved through the use of an extended transcaruncu

Neurological paraneoplastic syndromes.

Paraneoplastic neurological syndromes are uncommon, however, their diagnosis is of major practical importance. The identification of antibodies in the serum or cerebrospinal fluid in central nervous system paraneoplastic syndromes confirms the clinical diagnosis of a paraneoplastic syndrome and allows early identification of an underlying tumour at a stage when it is localised and more amenable to treatment. The failure to identify antibodies in patients with characteristic presentations of underlying neurological paraneoplastic syndromes does not exclude an underlying cancer. Necrotising myel

Thalamic lesions revealed by MR associated with periventricular leukomalacia and clinical profiles of subjects.

Magnetic resonance (MR) findings at the cerebral white matter and the thalamus in 44 children with spastic cerebral palsy born at preterm were analysed. Periventricular leukomalacia (PVL) was found in all of the children. Lesions of the thalamus were revealed in 22 children, 19 of which were in the anterior part of the pulvinar and 3 of which were in other areas. Gestational ages and birthweights of the children with a lesion of the pulvinar were significantly greater than those without lesions of the thalamus. Mental retardation and paroxysmal ocular downward deviation were more frequently se

Outcome of children with opsoclonus-myoclonus regardless of etiology.

Within the past 11 years, 11 patients with opsoclonus and myoclonus, with or without a history of neuroblastoma, have been admitted to Children's Memorial Hospital. Eight of the 11 children had an occult neuroblastoma. Eight children have had subsequent delayed development with motor incoordination and speech delay (7 with neuroblastoma, 1 without). Nine of 11 children initially were treated with ACTH, 1 child was treated with prednisone, and 1 was not treated. Nine of the 10 children who were treated had recurrences of symptoms during the gradual withdrawal or discontinuation of ACTH. Often t

Superior Rectus Transposition in the Management of Duane Retraction Syndrome: Current Insights.

Various surgical approaches have been described for the management of Duane retraction syndrome (DRS), a type of congenital cranial dysinnervation disorder (CCDD), the goals of which include correcting the primary position deviation and abnormal head posture (AHP), minimizing globe retraction and overshoots and improving the ocular rotations. Vertical rectus transposition (VRT) is one such technique, found more effective in improving abduction and thereby expanding the field of binocular vision, as compared to horizontal muscle surgery. VRT, however, is associated with the risk of inducing ver

Prevalence of congenital colour vision deficiency among Black school children in Durban, South Africa.

OBJECTIVES: Congenital colour vision deficiency (CCVD) is an x-linked chromosome disorder that results from abnormalities in one or all three-cone type's photoreceptors. Early assessment and diagnosis of CCVD is necessary to minimise the disability associated with the condition. Multistage sampling was used to determine the prevalence of CCVD among Black South African school children in Durban, South Africa. The examination included visual acuity measurements, ocular motility evaluation, retinoscopy, auto-refraction, and examination of the anterior segment, media and fundus. Colour vision test

The management of patients with thyroid eye disease after bilateral orbital 3 wall decompression.

INTRODUCTION: Thyroid eye disease is the most common cause of unilateral and bilateral proptosis in adults. Orbital decompression surgery may cause and/or worsen a pre-existing ocular motility disorder. METHODS: A retrospective review was carried out of all bilateral 3 wall orbital decompressions for severe thyroid eye disease performed between January 2002 and December 2004 by one surgeon. Subsequent surgeries were recorded. RESULTS: Seventy-four patients were identified, 59 (80%) females and 15 (20%) males. Mean age at the time of decompression was 46 years. Fifteen (20%) patients complain

Quantitative oculomotor and nonmotor assessments in late-onset GM2 gangliosidosis.

OBJECTIVE: A cross-sectional study was performed to evaluate whether quantitative oculomotor measures correlate with disease severity in late-onset GM2 gangliosidosis (LOGG) and assess cognition and sleep as potential early nonmotor features. METHODS: Ten patients with LOGG underwent quantitative oculomotor recordings, including measurements of the angular vestibulo-ocular reflex (VOR), with results compared to age- and sex-matched controls. Disease severity was assessed by ataxia rating scales. Cognitive/neuropsychiatric features were assessed by the cerebellar cognitive affective syndrome (

Surgical treatment of pineal cysts in non-hydrocephalic and neurologically intact patients: selection of surgical candidates and clinical outcome.

PURPOSE: Management of patients presenting for various nonspecific complaints without clear neurological abnormalities and with normal ventricular size remains highly controversial. We intended to share our rationale for surgical treatment of patients who show symptoms of transient increase of intracranial pressure owing to the presence of the cyst. MATERIALS AND METHODS: We have retrospectively analyzed 28 cases of patients who presented without Parinaud syndrome nor ventricular enlargement and underwent pineal cyst removal in our centre between 2007 and 2015. We analyzed patients' age, sex,

Saccadic Impairment Associated With Remote History of Mild Traumatic Brain Injury.

Evidence suggests that mild traumatic brain injury (TBI) is associated with long-term changes in brain function, but conventional neurocognitive tools are often insensitive to deficits after 90 days. Eye movements have been proposed as a means to identify more chronic forms of impairment. In this study, saccadic, manual, and conventional neuropsychological measures were compared between participants with remote mild TBI and well-matched control participants. Saccadic impairment was more frequent within the mild TBI group, and a history of multiple injuries or high symptom burden appeared to co
