Fifteen supracondylar fractures of the femur in patients with ipsilateral total knee arthroplasties were treated between 1975 and 1982. Three groups were identified for analyzation of treatment and end result. The average follow-up period after fracture was 18 months, with a range of ten to 48 months. Group I had four patients treated by closed reduction, cast immobilization, and early weight-bearing. At follow-up evaluation, three had a decrease in knee rating score, and one required a corrective osteotomy. Group II had eight patients treated by traction followed by cast or cast-brace immobil

Complete dislocation fractures of the forearm are rare in children. Operative treatment is indicated if it is an open fracture and if neurovascular injury is present. There is no accepted standard treatment--operative or nonoperative--for fractures that cannot be accurately reduced and stabilized. In the last 10 years we have operated on 37 fractures of the forearm in children. In 22 cases the indication for operative treatment was that the fractures could either not be reduced or could not be stabilized. Our results show that most fractures were not recognized as unstable (17 out of 22 cases)

Supracondylar fractures of the humerus are the most frequent fractures of the paediatric elbow, with a peak incidence at the ages of five to eight years.Extension-type fractures represent 97% to 99% of cases. Posteromedial displacement of the distal fragment is the most frequent; however, the radial and median nerves are equally affected. Flexion-type fractures are more commonly associated with ulnar nerve injuries.Concomitant upper-limb fractures should always be excluded. To manage the vascular status, distal pulse and hand perfusion should be monitored. Compartment syndrome should always be

INTRODUCTION AND IMPORTANCE: Simple closed lateral elbow dislocation is rare, often associated with neurovascular injury and closed reduction may be difficult. Open complex postero-lateral dislocation has not been previously reported.
CASE PRESENTATION: We present a case of open left posterior lateral elbow dislocation secondary to assault with a cutlass causing an avulsion fracture of the lateral epicondyle and common extensor origin tendon which prevented congruent reduction until the avulsed fragment was fixed with a bone screw.
CLINICAL DISCUSSION: The patient had hemorrhagic shock from

OBJECTIVE: To investigate the technique and clinical effect in the surgical reduction and screw fixation of sagittal condylar fractures.
METHODS: Open treatment was performed on 21 cases (23 condyles) with sagittal condylar fractures. The condylar fracture segment was reduced and fixed by screw (long screw or lag screw). The patients began the mandible movement exercise early after the operation, and follow-up was performed in the first half year after the operation.
RESULTS: Favorable results were obtained in all cases. Slight malocclusion was observed in 3 cases accompanied with mandible f

Fractures of the ankle in adults are very common. Like every intraarticular fracture, they require perfect reduction and stable fixation to guarantee a good result. In most cases this can only be achieved by open treatment. The classification of ankle fractures according to Weber depends upon whether the fibular fracture is below (type A) at (type B) or above (type C) the syndesmosis. This classification also defines the importance of the tibio-fibular ligaments. From 1978 to 1992 more than 1000 ankle fractures were treated in our clinic by open reduction and fixation: 72 were type C ankle fra

We reviewed 24 displaced fractures through the physis of the medial epicondyle of the distal humerus. One was a Salter-Harris type-II fracture-separation of the whole distal humeral epiphysis; the others involved only the medial epicondylar centre of ossification. Two cases had presented as pseudarthroses. One fracture had been treated closed in a plaster slab and 21 had had open reduction and internal fixation with sutures, Kirschner wires or Palmer nails. At 2 to 13 years later we found five types of deformity of the epicondyle: pseudarthrosis, an ulnar sulcus, a double-contoured epicondyle,

When both a coxarthrosis and a sub- or pertrochanteric fracture of the femur are simultaneously present, it is, as a rule, most difficult to carry out a closed reduction of the fracture on the extension table. In almost all cases, an open reduction has to be performed. When such an open reduction is necessary, this should not give reason to dispense with the advantages offered by the Ender nailing method and select another technique of osteosynthesis. An early mobilisation, which is of particular importance to old patients, can be achieved best by the aforementioned osteosynthesis. On the othe

Techniques for reduction and fixation of intra-articular distal radius fractures have improved in recent years such that fractures that were once thought doomed to a poor result may now be successfully treated operatively. This article explores open, semi-open, and arthroscopically assisted techniques for reduction and fixation of complex distal radius fractures. The reader should gain an appreciation of which fractures can benefit from operative treatment and which techniques may be most useful for specific fracture types. A number of fixation techniques are described, and familiarity with di

CASE: A Salter-Harris II fracture of the small finger middle phalanx with complete extrusion of the epiphyseal fragment in a 6-year-old boy is described. Closed reduction was unsuccessful. Open reduction was performed, the epiphysis was reduced, and a single Kirschner wire was placed obliquely across the fracture and joint.
CONCLUSION: This is an extremely rare injury. Open reduction and pinning was successful in preserving functional range of motion. Despite eventual growth arrest, there was no clinically significant angular deformity, and the joint was preserved. Surgical technique and earl

BACKGROUND: Increasing evidence suggests that open reduction and internal fixation of condylar base fractures in adults results in improved outcomes in regard to interincisal opening, jaw movement, pain, and malocclusion. However, most of the condylar fractures are managed by maxillomandibular fixation alone due to the need for specialized training and equipment. Our aim was to present an algorithm for condylar base fractures to simplify surgical management.
METHODS: A retrospective review was performed of patients (n = 22) with condylar base fractures treated from 2016 to 2020. Patients who

KEY CLINICAL MESSAGE: Lumbar facet fracture-dislocation is rare, often due to hyperflexion with rotational force. Abnormal scoliosis on radiography is a key sign, prompting further imaging. Facetectomy and laminectomy ensure safe reduction, while instrumented fusion is an effective surgical treatment.
ABSTRACT: Traumatic lumbar facet dislocation is an uncommon condition, particularly when unilateral lumbar facet dislocation is accompanied by a contralateral facet fracture. The patient had a locked facet joint at the left L4-L5, a fracture of the right superior articular process of L5 and a co

Conventional radiographic evaluation of fracture healing is not a reliable method, because it depends on the examinator's experience and the quality of the exam. Therefore, serial images differing in density, contrast and geometrical projection can lead to a misdiagnosis on the postoperative fracture healing. Even in good quality images, little changes in calcified tissues often can't be visualized, because of its little sensibility and because of the limited human sight. The use of more sensitive and objective methods could increase the accuracy of this evaluation. This study intended to comp

Endoscopic-assisted repair of subcondylar fractures is an additional tool for management; however, there is a steep learning curve. Generally, this technique allows good visualization of the fracture site for reduction through an incision with an acceptable cosmetic result. Recently, the surgical techniques and technology as well as the indications for endoscopic facial fracture repair are in development; there are few available data in the literature regarding detail complications and recovery processes following endoscopic fracture treatment. The purpose of this article was to reveal unusual

Closed tibiae plateau fractures are common injuries in the emergency room. The optimal treatment is not well defined or established. For this reason, there are several surgical management options: open reduction and internal fixation, closed reduction and percutaneous synthesis, external fixation, and even conservative treatment for this kind of fracture. The mechanism of production of this fracture is through large varus or valgus deformation to which is added a factor of axial load. The trauma may be direct or indirect. The degree of displacement, fragmentation and involvement of soft tissue

Forty-seven of 661 head-injured adults sustained 50 forearm fractures. Eight extremities exhibited elbow flexion contractures greater than 55 degrees. Traumatic heterotopic ossification at the elbow developed in ten (20%) extremities: four with Monteggia fractures, two with olecranon fractures, and four with no known trauma. Only two of 18 (11%) extremities treated by plaster immobilization achieved good or excellent results, while 17 of 32 (53%) extremities treated by open reduction and internal fixation achieved good or excellent results. Union of fractures of one or both bones occurred at t
