Subperiosteal abscess of the orbit: evolving pathogens and the therapeutic protocol.

PURPOSE: Because of widespread antibiotic use, immunization against selective species, and the emergence of hypervirulent organisms, we sought to determine changes over time in the bacteriology of sinusitis-related subperiosteal abscess (SPA) of the orbit and their impact on patient outcomes under a uniform management protocol. DESIGN: Comparative case series. SUBJECTS: Patients≤18 years of age with sinusitis-related SPA treated from 2002 to 2012; comparable cohorts from earlier time frames. METHODS: We analyzed culture results and outcomes in surgical cases, compared overall and age-specif

Periapical disease and bisphosphonates induce osteonecrosis of the jaws in mice.

Osteonecrosis of the jaw (ONJ) is a well-recognized complication of antiresorptive medications, such as bisphosphonates (BPs). Although ONJ is most common after tooth extractions in patients receiving high-dose BPs, many patients do not experience oral trauma. Animal models using tooth extractions and high BP doses recapitulate several clinical, radiographic, and histologic findings of ONJ. We and others have reported on rat models of ONJ using experimental dental disease in the absence of tooth extraction. These models emphasize the importance of dental infection/inflammation for ONJ developm

Dimension of subperiosteal orbital abscess as an indication for surgical management in children.

OBJECTIVE: Eyelid edema in children is one of the signs of orbital complications secondary to acute rhinosinusitis, and identifying abscess formation is crucial for management decision. The objective of this study is to determine whether there are different computed tomography scan abscess dimensions and volumes in children requiring medical versus surgical management for subperiosteal orbital abscess (SPOA). STUDY DESIGN: Case series with chart review. SETTING: The study was conducted at Assaf Harofeh Medical Center. SUBJECTS AND METHODS: Clinical and radiological parameters of 95 children

Bioceramic orbital implant exposure repaired by a retroauricular myoperiosteal graft.

A patient with a painful, blind eye underwent enucleation and bioceramic orbital implantation. The bio-ceramic implant was wrapped with polyglactin 910 mesh and anteriorly capped with a scleral patch graft. Because there was tension on the wound, a relaxing conjunctival incision on the fornix was performed. Implant exposure with a diameter of 9 mm occurred 4 months after surgery. The exposed bioceramic implant was successfully repaired by a retroauricular myoperiosteal graft. The graft contains myofibrovascularized tissue, provides durable and vascularized coverage of exposed implants, and onl

Spontaneous subperiosteal orbital hemorrhage following thrombolytic therapy for myocardial infarction.

A 60-year-old man experienced right orbital pain, periorbital swelling, and double vision 2 hours after treatment with streptokinase and heparin for myocardial infarction. Orbital CT revealed a right superior subperiosteal orbital hemorrhage. Conservative management in the absence of visual compromise was sufficient, as his symptoms and signs resolved completely in approximately 6 weeks with no recurrence during 6 months of follow-up. This case demonstrates that nontraumatic subperiosteal orbital hemorrhage may occur after thrombolytic therapy for myocardial infarction. Conservative treatment

Periodontal plastic surgical technique for gingival fenestration closure.

Gingival fenestration is an opening through oral keratinized tissue, usually unattached, that is observed in thin gingiva with usually thick subgingival calculus deposits. This lesion is seen infrequently but may be more common than has been reported; lack of symptoms may inhibit patient awareness. Because surgical correction usually is not required, there are very few reports in the literature concerning this lesion. The following report describes a case of gingival fenestration and surgical treatment with a connective tissue/periosteal graft.

Sinus pericranii.

Sinus pericranii is an anomalous extracranial vascular malformation that is in continuity with the intracranial dural venous sinuses. Five case reports, three congenital and two traumatic, are described. Clinical management, including evaluation, diagnosis, and treatment, is discussed. Awareness of this entity by plastic surgeons will allow for earlier diagnosis and appropriate surgical management, resulting in decreased risk of complications.

Calcifying aponeurotic fibroma: a clinicopathologic study of 22 cases arising in uncommon sites.

Calcifying aponeurotic fibroma is a rare soft tissue tumor that primarily occurs in children and adolescents and has a strong predilection f … d (n=1). The lesions were often adherent to dense fibrous connective tissue (eg, tendon, fascia, or periosteum) and ranged from 1.0 to 5.0 cm in maximum dimension. The process typically had an irregular contour and a firm, fibrous consistency. Sometimes minute foci with a calcific appearance were evident grossly. Microscopic examination showed spindled fibro …

Intravelar veloplasty: surgical modification according to anatomical defect.

The patient is often left with an oro-nasal fistula after the intravelar veloplasty procedure for the primary closure of the hard and soft palate cleft. The junction between the hard and soft palate is submitted to maximal tension during this procedure and is where the fistula most often occurs. The primary intravelar veloplasty procedure is discussed and 7 surgical modifications are introduced. The aim of these modifications is the prevention of an oro-nasal fistula and each is specifically adapted according to the existing anatomical form and defect of the cleft palate. The surgical modifica

The callotasis method of limb lengthening.

Callotasis is a lengthening technique that involves slow, controlled distraction after subperiosteal-submetaphyseal osteotomy. The technique and its advantages over other methods are described. Results of lengthenings involving 270 operated bone segments (146 femurs and 124 tibias) in 140 patients are reviewed. Ninety-five patients had limb-length inequality and 45 had hypochondroplasia or achondroplasia. On average, 6.6 cm, or 24.6% of initial length, was gained. The mean healing index was 39; the complication rate was 13.3%.

Anatomical effects of sudden middle ear pressure changes.

Acute middle ear (ME) and inner ear changes following brief unilateral phasic ME pressure changes (up to +/- 6000/mm H2O) were studied in the guinea pig. Middle ear findings included perforation of the tympanic membrane, serous and serosanguinous exudate and hemorrhage of tympanic membrane and periosteal vessels. Changes were related to magnitude of applied pressure. Perforation and hemorrhage were more commonly seen with negative rather than positive pressure. Air bubbles behind the round window were seen with positive pressures. Occasional distortion, but never perforation of the round windo

Radiologic features of Reiter disease.

The radiologic manifestations of Reiter disease in 56 patients are described. Although the findings are often nonspecific, certain features are distinctive, particularly when present in combination. These include subtle periosteal bone apposition at bony prominences, the infrequency of regional panarthritis and hip and shoulder involvement, the tendency for destructive arthritis to be confined to small joints, particularly in the feet, and a typically "discontinuous" spondylitis characterized by eccentric, laterally positioned vertebral hyperostoses which tend to bridge around the contigous di

A Retrospective Analysis of the Correlation Between Periosteal Entrapment and Growth Disturbances in Salter-Harris II Physeal Fractures of the Distal Tibia in Children.

BACKGROUND: Periosteal entrapment (PE) in displaced distal tibial Salter-Harris (S-H) II fractures is considered a risk for growth disturbances, but causality is debated. QUESTIONS/PURPOSES: To investigate whether PE is an independent risk factor for growth disturbances in distal tibial S-H II physeal fractures and to compare the clinical efficacy of different treatment methods. PATIENTS AND METHODS: Retrospective analysis of 27 patients (2015 to 2024) with distal tibial S-H II fractures and confirmed PE [magnetic resonance imaging (MRI): n=21; follow-up radiograph: n=6]. Patients received c

Mucoperiosteal Flap Healing During Vertical Bone Augmentation using Titanium Mesh: A Study in Beagle Dogs.

INTRODUCTION AND AIMS: The periosteal releasing incision (PRI) is often used in guided bone regeneration (GBR) with titanium (Ti) mesh to reduce the tension of mucoperiosteal flaps (MPF) and close the wound, but it can easily lead to keratinized gingiva (KG) narrowing and a high wound dehiscence rate. Therefore, it is necessary to explore alternative methods such as open healing. The aim of this study was to observe whether a MPF without a PRI and primary closure could heal on a titanium membrane surface in vertical bone augmentation and to measure the change in KG width. METHODS: The bilater

Pamidronate regulates the osteogenic differentiation of periosteal-derived mesenchymal stem cells in congenital tibial pseudarthrosis via OPG/RANKL.

BACKGROUND: Congenital pseudarthrosis of the tibia (CPT) is one of the most challenging conditions in orthopedics. Previous research indicates a strong association between CPT and neurofibromatosis type 1 (NF1). The clinical application of pamidronate has demonstrated efficacy in enhancing bone healing and alleviating NF1-associated bone pathology in specific CPT cases. Nevertheless, the precise mechanistic basis for these therapeutic effects remains poorly characterized, warranting further investigation. METHODS: In this study, mesenchymal stem cells (MSCs) were isolated from patients with c
