Do guidelines influence breathlessness management in advanced lung diseases? A multinational survey of respiratory medicine and palliative care physicians.

BACKGROUND: Respiratory medicine (RM) and palliative care (PC) physicians' management of chronic breathlessness in advanced chronic obstructive pulmonary disease (COPD), fibrotic interstitial lung disease (fILD) and lung cancer (LC), and the influence of practice guidelines was explored via an online survey. METHODS: A voluntary, online survey was distributed to RM and PC physicians via society newsletter mailing lists. RESULTS: 450 evaluable questionnaires (348 (77%) RM and 102 (23%) PC) were analysed. Significantly more PC physicians indicated routine use (often/always) of opioids across c

Physician's knowledge, attitudes, and practices regarding seasonal influenza, pandemic influenza, and highly pathogenic avian influenza A (H5N1) virus infections of humans in Indonesia.

Indonesia has reported highest number of fatal human cases of highly pathogenic avian influenza (HPAI) A (H5N1) virus infection worldwide since 2005. There are limited data available on seasonal and pandemic influenza in Indonesia. During 2012, we conducted a survey of clinicians in two districts in western Java, Indonesia, to assess knowledge, attitudes, and practices (KAP) of clinical diagnosis, testing, and treatment of patients with seasonal influenza, pandemic influenza, or HPAI H5N1 virus infections. Overall, a very low percentage of physician participants reported ever diagnosing hospit

Work-related sickness absence negotiations: GPs' qualitative perspectives.

BACKGROUND: GPs can find their role as issuers of sickness certification problematic, particularly in trying to maintain a balance between certifying absence and preserving the doctor-patient relationship. Little research has been published on consultations in which sickness absence has been certified. AIM: To explore negotiations between GPs and patients in sickness absence certification, including how occupational health training may affect this process. METHOD: A qualitative study was undertaken with GPs trained in occupational health who also participate in a UK wide surveillance scheme

Preventing sexually transmissible infections in Australian general practice.

The aims of the present study were to explore aspects of sexually transmissible infections (STI) prevention in general practice and to examine general practitioners' (GPs) perceived barriers to sexual-health promotion. The data from a postal survey of 409 GPs practising in New South Wales, Australia (response rate 45.4%) are analysed to explore the prevention of STI in general practice and to examine practitioners' perceived barriers to sexual-health promotion. About 49% of GPs reported having STI leaflets/pamphlets for patients in their clinic, while 21% had posters on STI displayed in their

Doctors' beliefs on the use of antithrombotic therapy in atrial fibrillation: identifying barriers to stroke prevention.

AIMS: To assess the attitudes of Australian doctors towards the use of antithrombotic drug therapy for stroke prevention in patients with non-valvular atrial fibrillation (AF), and investigate the barriers to prescribing warfarin. METHODS: A postal survey was undertaken among approximately 10% of all registered general practitioners (GPs), cardiologists and physicians in Australia. The anonymous questionnaire used case scenarios to assess doctors' knowledge of current guidelines for the therapeutic management of AF and sought opinions on potential barriers to the use of anticoagulation. Physician-level variability in adopting opportunistic salpingectomy for ovarian cancer prevention at the time of hysterectomy.

Opportunistic salpingectomy reduces ovarian cancer risk. To examine heterogeneity among physicians in their adoption of opportunistic salpingectomy in hysterectomy practice, we identified 33,401 non-pregnant patients aged 18 to 49 years in the Premier Healthcare Database who underwent inpatient hysterectomy for benign indications without oophorectomy in 2011-2021 (operated on by 1297 physicians). Opportunistic salpingectomy was measured by procedure codes reflecting bilateral complete removal of fallopian tubes. Even after adjusting for patient age, surgical route, indication, overweight/obesi

State-mandated insurance coverage is associated with the approach to hydrosalpinges before IVF.

The aim of this study was to determine whether practice in states with infertility insurance mandates is associated with physician-reported practice patterns regarding hydrosalpinx management in assisted reproduction clinics. A cross-sectional, internet-based survey of 442 members of Society for Reproductive Endocrinology and Infertility or Society of Reproductive Surgeons was performed. Physicians practising in states without infertility insurance mandates were more likely to report performing diagnostic surgery after an inconclusive hysterosalpingogram than physicians practising in states wi

An assessment of the likelihood, frequency, and content of verbal communication between radiologists and women receiving screening and diagnostic mammography.

RATIONALE AND OBJECTIVES: Research on communication between radiologists and women undergoing screening and diagnostic mammography is limited. We describe community radiologists' communication practices with patients regarding screening and diagnostic mammogram results and factors associated with frequency of communication. MATERIALS AND METHODS: We received surveys from 257 radiologists (70% of those eligible) about the extent to which they talk to women as part of their health care visit for either screening or diagnostic mammograms, whether this occurs if the exam assessment is positive or

Deflecting Before Prescribing: Negotiating About Opioids and Chronic Pain in Primary Care.

In this study, we examine opioid-related treatment negotiations during primary care visits for chronic pain. We video-recorded 86 visits involving established patients on chronic opioids seeing primary care physicians; participants completed post-visit assessments. We used conversation analysis to identify patient requests (direct and indirect) for better pain control, create a taxonomy of eight "deflection" practices physicians used to respond before making opioid-related treatment recommendations (gathering information, evaluating pain, quelling, proposing alternative treatment options, empa

"I can't take in every stray puppy": How U.S. pain specialists purify pain management through opioid-related gatekeeping.

U.S. pain specialists have always rallied against their profession being cast as "dirty work" - work that is undesirable, socially stigmatized, and morally questionable - due to both its object of study (chronic pain) and one of its primary treatment modalities (prescription opioids). This paper demonstrates that U.S. outpatient pain specialists purify pain management through opioid-related gatekeeping practices that differ depending upon their role as a physician or an advanced practice provider (APP). Drawing from abductive analysis of semi-structured interviews with 21 pain specialists and

Factors influencing physicians' recommendations for surgical treatment and mode of delivery in reproductive-age women with stress urinary incontinence: Insights from an international survey.

OBJECTIVE: To determine physicians' attitudes towards surgical treatment for stress urinary incontinence (SUI) and its consequences for women of childbearing age. METHODS: Electronic survey approved by the International Urogynecological Association (IUGA) and American Urogynecologic Society (AUGS), distributed to their members. RESULTS: A total of 191 physicians completed the survey. The majority of respondents (115, 60.2%) stated that they would perform or recommend surgical treatment for SUI for patients who have not yet completed their family planning, although most (136, 71.2%) would adv

Cultivating change door to door: Educational outreach to improve prescribing practices in rural veterans with posttraumatic stress disorder.

BACKGROUND: Clinical guidelines for the management of posttraumatic stress disorder (PTSD) recommend against the use of benzodiazepines. Benzodiazepines and PTSD are both associated with addiction-related risks. The Department of Veterans Affairs (VA) prescribing trends show continued use of benzodiazepines and polysedative use in veterans with PTSD, particularly in rural areas. The authors examine the use of an educational intervention to improve pharmacologic management of veterans with PTSD in rural clinics. METHODS: The VA Academic Detailing Service Informatics Toolset provides prescribin

Demographic and practice characteristics of pathologists who enjoy breast tissue interpretation.

Physician attributes, job satisfaction and confidence in clinical skills are associated with enhanced performance and better patient outcomes. We surveyed 252 pathologists to evaluate associations between enjoyment of breast pathology, demographic/clinical characteristics and diagnostic performance. Diagnostic performance was determined by comparing pathologist assessments of a set of 60 cases with consensus assessments of the same cases made by a panel of experienced pathologists. Eighty-three percent of study participants reported enjoying breast pathology. Pathologists who enjoy breast inte
