Ultrasound scanning was performed at three sites above the fossa supraspinata on nine healthy subjects and five patients with myofascial shoulder pain. This method produced a well-defined depiction of the soft tissue layers above the fossa supraspinata and reproducible muscle thickness measurements. However, noxious stimulation of the neck can suppress pain responsiveness both at the site of stimulation and at sites remote from the stimulated area while noxious stimulation of the tail produces analgesia only at sites remote from the stimulated area. Peliosis hepatis (PH) and sclerosing angiomatoid nodular transformation of the spleen are uncommon benign lesions. Diagnosis can be difficult in some patients. Herein, we present the case of a 28-year-old woman referred with abdominal pain who had spleen lesions. 18F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography revealed multiple non-FDG avid lesions in the liver and hypermetabolic lesions in the spleen. A 34-year-old male patient living with human immunodeficiency virus (HIV) presents to the dental clinic with a 1-week history of throbbing tooth pain. He was referred by an oral medicine specialist for evaluation and treatment. The patient presents with very low absolute neutrophil, platelet, and cluster of differentiation (CD) (also T-helper cell) 4+ cell counts, as well as a high HIV RNA/viral load. Determining factors for dental management before extraction of the offending teeth were the absolute neutrophil count and platelet counts.

A 49-year-old female patient was referred to a tertiary care hospital with a history of postmenopausal bleeding and abdominal pain that had persisted for the last two months. An abdominal examination revealed a huge mass that had been present for 12 weeks. A fibroid uterus was suggested by USG. A biopsy was done and sent to histopathology which revealed myxoid leiomyoma. Subsequently, the patient underwent total abdominal hysterectomy without bilateral salpingo-oophorectomy. Histopathological examination confirmed the diagnosis of myxoid leiomyosarcoma (MLMS) of the uterus.

Jackstones are a rare subtype of calculi, characterised by radiating spicules. We describe a case of multiple jackstones in the urinary upper tract. A 53-year-old man was referred for evaluation of left flank pain. Imaging revealed a left pelvic-ureteric junction obstruction and several left renal calculi. A left supine percutaneous nephrolithotomy was performed and two jackstone calculi were extracted. The presence of multiple jackstones in the renal pelvis should prompt for evaluation for obstruction and complete stone clearance should be achieved to avoid recurrence.

A healthy 38-year-old woman, with no significant medical or surgical history, was referred to the gynaecology emergency department with acute low quadrant abdominal pain and nausea. On examination, painful palpation of right iliac fossa, abdominal guarding and positive Blumberg's sign were noted. An abdominal-pelvic CT scan showed a hypodense tubular structure with peripheral enhancement to the right of the inferior vena cava, suggestive of thrombosis of the right ovarian vein. The only risk factor identified was the use of the combined contraceptive pill.

A 7-year-old boy with chief complaint of chronic pelvic pain was referred to our nuclear medicine department for bone scintigraphy. The images showed a focus of radiotracer activity in the right side of pelvic cavity, which is further confirmed as urinary bladder by single-photon emission computed tomography (SPECT) and delayed images. Because of high possibility of mass effect in pelvic cavity, pelvic magnetic resonance imaging (MRI) was performed, and it revealed an unusual dilatation of rectosigmoid colon with no evidence of pelvic mass.

Neurolymphomatosis represents a peripheral lymphomatous neural infiltration and is a relatively rare condition. Histopathologically, most patients are demonstrated to have non-Hodgkin lymphoma. A 27-year-old man with a history of HLA-B27-positive ankylosing spondylitis and diffuse large B-cell lymphoma was referred for the investigation of a severe neuropathic pain radiating down his right hip and posterior thigh for 1 month. This report describes the diagnosis and therapy response assessment in a relapsing aggressive non-Hodgkin lymphoma on an F-FDG PET scan.

The authors report a rare case of primary orbital melanoma (POM) combined with giant divided nevus of the eyelid. An 8-year-old Chinese girl is referred for evaluation of 2-month duration of exophthalmos with decreased vision, epiphora, and pain on her right eye. His presentation, imaging, biopsy, histopathology, and management are presented. The possible cellular origin of the POM and the relationship of POM and divided nevus are discussed. We presume that divided nevus may be one of rarely preexisting lesions of POM.

A 45-year-old female was referred to the Department of Oral and Maxillofacial Surgery with the complaint of pain in the right submandibular region and a dry mouth, which had started one week previously. A clinical examination revealed a swelling and tenderness in the right submandibular region. Panoramic radiography and computed tomography identified a sialolith in the submandibular gland. OBJECTIVES: To describe a case with the rare association of Klinefelter syndrome (47,XXY) and peripheral sensorimotor polyneuropathy.
CLINICAL PRESENTATION AND INTERVENTION: A 50-year-old man with Klinefelter syndrome was referred to our neurology clinic complaining of pain, numbness and tingles in both legs, which began 10 years prior to admission. Two years before admission, the patient had difficulty in walking with progressive weakness.
CONCLUSION: This report shows a patient with diagnosed Klinefelter syndrome, in whom symmetrical sensorimotor polyneuropathy developed in late adulthood.

Varicella-zoster virus is a common viral infection of childhood. This report concerns an 8-year-old girl who presented with a 5-day history of a typical varicella rash. She then developed red left eye 'conjunctivitis'. She had no discharge, mild pain and intense photophobia. She was referred to the acute eye clinic after 1 day and she was noted to have acute anterior uveitis. She was treated with gutt atropine 1% for 2 weeks until the inflammation resolved. She had no sequalae.

OBJECTIVE: This article presents a case of osteogenesis imperfecta and reviews the clinical and radiographic features of this condition.
CLINICAL FEATURES: A 27-year-old woman presented with pain in her left hand after a fall while dancing. Plain films revealed multiple fractures in the digits of her left hand.
INTERVENTION AND OUTCOME: The patient was referred to an orthopedic specialist for treatment.
CONCLUSION: Osteogenesis imperfecta is a heritable disorder of bone, which predisposes patients to fractures after trivial trauma, as was demonstrated in this case.

BACKGROUND: Onycholysis, which is separation of the nail plate from the nail bed at its distal and lateral attachments, can cause pain and impair function and is a well-known side effect of traditional chemotherapeutic agents.
OBJECTIVES: We present two cases of onycholysis associated with capecitabine in women with advanced breast cancer, who were initially referred for evaluation of onychomycosis and review the literature on capecitabine.
CONCLUSION: Owing to the increased frequency of use of capecitabine for different cancers, physicians should be aware of this side effect.

Syphilitic orchitis is recently a rare disease in Japan. A 75-year-old man was referred to our hospital with the complaint of persistent swelling of the left scrotal contents in spite of prior antibiotic therapy. We suspected a testicular tumor because of lack of pain, and performed high orchiectomy. The specimen showed wide-ranged necrosis with a non-specific inflammatory change of the testis on hematoxylin-eosin stain. After performing analysis using a polymerase chain reaction method, we reached the final diagnosis of syphilitic orchitis.

We report a case of acute appendicitis in situs inversus treated by laparoscopy. In our opinion, diagnostic laparoscopy is indicated if acute appendicitis is suspected in a patient with situs inversus totalis, either because the pain can be referred to the right iliac fossa in about 50% of the patients, or because the incision in open surgery cannot be sufficient to allow complete exploration of the abdomen. The operation can then be carried out laparoscopically if possible; if not, an appropriate surgical incision can be made.

Cervical disk degeneration (CDD) can give rise to segmental instability and may appear as pain referred to the arm without any evidence of root compression. CDD can give rise to root pressure either by permitting the development of a disk rupture of by provoking the development of a neurocentral osteophyte, compressing the root in the root canal. Such osteophytes may also compress the vertebral artery. Primary treatment is always conservative. When operative intervention is necessary an anterior cervical discectomy and fusion is preferable to a posterior decompression.

AIM: Knee pain is the most frequently reported type of lower extremity pain among children and adolescents. The objective of this study was to establish the features distinguishing inflammatory knee pain from non-inflammatory knee pain and to determine the specific variables to consider in suspected juvenile idiopathic arthritis (JIA).
