Following wrist trauma due to a karate accident, an 18-year-old male patient developed non-union of the scaphoid with proximal pole necrosis and significant scapholunate gap from an evident scapholunate interosseous ligament (SLIL) injury. Early diagnosis and correct treatment of acute scaphoid fractures is mandatory and of great importance in terms of the long-term prognosis for these young patients. In cases of traumatic wrist pain, emergency physicians must maintain a high index of suspicion for scaphoid fractures due to their potential for serious complications. A growing body of literature supports the use of point-of-care ultrasonography by emergency physicians in the evaluation of potential fractures. We report a case of a pediatric scaphoid fracture that was initially not visualized on x-ray and was subsequently detected using point-of-care ultrasound in the ED.

The severely displaced scaphoid fracture, when combined with a scapholunate dissociation, is a very rare condition that commonly is treated by open surgery. The authors present a case treated by arthroscopic-assisted closed reduction, using percutaneous pinning for the scapholunate dissociation and a Herbert/Whipple screw insertion for the scaphoid fracture.
