Immunocompromised patients are at high risk for invasive fungal infections (IFIs); although Aspergillus remains the most common IFI caused by molds, other fungi, such as Mucorales, dematiaceous molds, and Fusarium spp, are being seen with increasing frequency. We report the case of a 31-year-old immunosuppressed, liver transplanted man, with acute measles infection. The vaccinated patient had been exposed to measles during a known measles epidemic in public schools in Austria between January and April 2008. There is a subset of patients who have biochemical and histologic features consistent with primary biliary cirrhosis (PBC) who lack antimitochondrial antibodies (AMA). This entity is usually referred to as AMA-negative PBC or alternatively autoimmune cholangitis. Histoplasmosis is a common infection endemic in many regions of America, Asia, India and Africa, with sporadic cases also occurring throughout the world. Although excellent laboratory methods for diagnosis are available, there are deficiencies that must be met by continued research. We report a 63-year-old woman with common variable immunodeficiency (CVID) and an indolent distal sensory neuropathy. Despite having negative serologic tests for Sjögren's syndrome, evaluation of the neuropathy led to a new diagnosis of Sjögren's syndrome based on the presence of sicca symptoms, an abnormal Schirmer's test, and histologic evidence of sialadenitis. Two young adults presented with thyrotoxicosis after increased iodide ingestion in the course of backpacking trips to Central and South America, during which each of them used an iodinated preparation for water purification. Blastomycosis is a rare systemic fungal infection caused by Blastomyces dermatitidis, which is often difficult to diagnose because of its spectrum of manifestations, varying severity, and unusual occurrence in most geographic areas. Because routine serologic methods and skin tests are not reliable for this pathogen, diagnosis is made by visualization of the organism in tissue, sputum, or exudate, or by culture. Between December 1983 and June 1984 280 patients with keratoconjunctivitis had a laboratory work-up for Chlamydia trachomatis infections. This study explores the exposure of urban children to the spirochete Leptospira icterohaemorrhagiae. This organism is carried by 90 percent of the rats in Detroit. It is felt that these rats are a potential vehicle for childhood exposure to this organism. Strain-specific tests were performed comparing antibody levels in sera of inner-city (exposed) and suburban (unexposed) children. Study and control groups, numbering 124 and 113, respectively, showed significant serologic differences, with urban children having higher antibody titers. Histoplasmosis commonly infects the lower respiratory tract, causing either a sub-clinical or a nonspecific pulmonary illness. It may also affect the larynx from wide-spread dissemination or in a localized form that mimics carcinoma. Hoarseness, dysphagia, and pain on swallowing are symptoms common to many diseases of the laryngopharynx, including histoplasmosis. Laboratory confirmation of reported cases of St. Louis encephalitis (SLE) lags 2-4 weeks behind onset of illness.
