The epidemiology of shoulder dislocation in a state-hospital: a review of 106 cases.

This retrospective study was conducted in a state hospital set-up and aimed at identifying the magnitude of shoulder dislocations and their demographic data, characteristics of the injury, mechanism and predisposing factors, and the instituted treatment. Patients with radiographic evidence of shoulder dislocation admitted to the hospital from January 1999 to December 2002 were included. Data were recorded from the case notes. There were 105 shoulder dislocations with male predomination in 77% cases and age ranged between 11 and 90 years (average 30.9 years). The right shoulder was affected in

Scapulohumeral rhythm, posterior capsule stiffness, shoulder muscle strength and flexibility in young unilateral and bilateral overhead athletes.

BACKGROUND: Asymmetric musculoskeletal adaptations, resulting from the type of sports, may lead to shoulder injuries due to altered shoulder mechanics and increased pressure on tissues. This study compared the scapulohumeral rhythm (SHR) ratio, flexibility, posterior capsule stiffness (PCS), and shoulder muscle strength between volleyball players as unilateral overhead athletes and swimmers as bilateral overhead athletes. METHODS: Fifty-seven young overhead athletes, 30 volleyball players, and 27 swimmers participated in this study voluntarily. The SHR, PCS, and shoulder internal and external …

A Modified Arthroscopic Outside-in Shoulder Release Approach for Severe Shoulder Stiffness.

OBJECTIVE: Arthroscopic release is effective for patients with shoulder stiffness, but the traditional inside-out procedure cannot effectively alleviate the mobility of some severe stiff shoulder and even cause itrogenic injuries sometimes. The aim of this study is to evaluate the clinical efficacy and advantages of a modified outside-in shoulder release approach for severe shoulder stiffness. METHODS: Included in this retrospective study were 15 patients (five male and 10 female) with severe shoulder stiffness who underwent modified outside-in shoulder release surgery at our hospital between

Shoulder Instability in Contact and Collision Athletes: A Focused Review on Assessment, Management, and Treatment.

Glenohumeral instability is a common injury affecting contact and collision athletes. Male sex, younger age at time of first dislocation, and contact sports participation are risk factors for recurrent instability. MRI is the gold standard to evaluate soft tissue structures, while CT is beneficial in quantifying glenoid bone loss and identifying on-track and off-track Hill-Sachs lesions. The management of shoulder instability in contact athletes is a nuanced, individualized process, taking into consideration the athlete's sport and playing position, hand dominance, timing of the in-season inju

Superior labrum anterior to posterior (SLAP) rehabilitation in the overhead athlete.

Due to the complexity of shoulder pathomechanics in the overhead athlete, injuries located in the superior aspect of the glenoid, known as superior labral anterior to posterior (SLAP) lesions, are often a surgical and rehabilitation challenge. In an effort to determine surgical versus conservative care of SLAP lesions a thorough clinical examination and evaluation are necessary. If surgery is identified as the treatment of choice, post operative rehabilitation will vary pending surgical findings including the extent and location of the SLAP lesion, and other concomitant findings and procedures

[Acute instability of the shoulder in athletes. The role of magnetic resonance in therapy planning].

Magnetic resonance imaging (MRI) of the shoulder was performed to evaluate both the actual role of this technique in the study and staging of acute shoulder instability, and its potentials as diagnostic tool, with particular reference to treatment planning. Seventeen athletes with acute shoulder instability were examined. All MRI examinations, subsequent to plain radiographs, were performed within 48 hours from the traumatic event. After MRI examination, 14 patients underwent physiotherapy (2 cases were subsequently submitted to arthrotomy), and only 3 cases underwent surgical treatment in the

A Review of Workload-Monitoring Considerations for Baseball Pitchers.

Because of the unique demands of a pitch, baseball players have the greatest percentage of injuries resulting in surgery among high school athletes, with a majority of these injuries affecting the shoulder and elbow due to overuse from throwing. These injuries are believed to occur because of repeated microtrauma to soft tissues caused by the repetitive mechanical strain of throwing. Researchers and practitioners have suggested that baseball pitchers' workloads are a significant risk factor for injury in adolescent players, resulting in lost time and slowing of performance development. The pur

Birth injuries to growth plates: A sheep in wolves' clothing.

Growth plate injuries (Salter-Harris type 1 or physeal fractures) of the long bones in the newborn are easily misdiagnosed as joint fractures with dislocations due to their nonossified epiphyses on plain radiographs. Diagnosis with musculoskeletal ultrasound (US) is advantageous due to its ability to visualize the nonossified epiphysis. We present two cases of humeral growth plate fractures in newborns, one at the shoulder (proximal humerus) and the other at the elbow (distal humerus). These cases emphasize the importance of quick and noninvasive diagnosis with US to avoid unnecessary interven

Pathomechanics and Magnetic Resonance Imaging of the Thrower's Shoulder.

Repetitive, high-velocity overhead throwing can lead to several adaptive changes in the throwing shoulder, which over time lead to structural microtrauma and eventually overt injury. MR imaging is a useful imaging modality to evaluate these changes and to characterize their acuity and severity. Understanding the throwing motion and the effects of this motion on the structures of the shoulder can help radiologists to recognize these findings and provide useful information to referring physicians, which may affect the treatment of these athletes. This article reviews shoulder pathomechanics and

The role of subacromial decompression in patients undergoing arthroscopic repair of full-thickness tears of the rotator cuff: a systematic review and meta-analysis.

PURPOSE: The purpose of this study was to determine the efficacy of arthroscopic repair of full-thickness rotator cuff tears with and without subacromial decompression. METHODS: We searched the Cochrane Central Register of Controlled Trials (third quarter of 2011), Medline (1948 to week 1 of September 2011), and Embase (1980 to week 37 of 2011) for eligible randomized controlled trials. Two reviewers selected studies for inclusion, assessed methodologic quality, and extracted data. Pooled analyses were performed by use of a random effects and relative risk model with computation of 95% confid

[Selective paralysis of the infraspinatus muscle, caused by compression of the infraspinatus branch of the supraspinatus muscle].

This is the case of a thirty-year-old athlete suffering from a painful right shoulder while moving and working with his right arm and showing a massive atrophy of his right infraspinatus muscle. After careful examination an isolated compression of the inferior branch of the suprascapular nerve was suspected. The entrapment of the nerve at the spinoglenoid notch by a large and hypertrophied ligament was demonstrated by an open revision.

Biomechanical, Histologic, and Micro-Computed Tomography Characterization of Partial-Width Full-Thickness Supraspinatus Tendon Injury in Rats.

PURPOSE: Partial rotator cuff tears can cause shoulder pain and dysfunction and are more common than complete tears. However, few studies examine partial injuries in small animals and, therefore a robust, clinically relevant model may be lacking. This study aimed to fully characterize the established rat model of partial rotator cuff injury over time and determine if it models human partial rotator cuff tears. METHODS: We created a full-thickness, partial-width injury at the supraspinatus tendon-bone interface bilaterally in 31 Sprague-Dawley rats. Rats were euthanized immediately, and at 2-,

Shoulder dystocia: the unpreventable obstetric emergency with empiric management guidelines.

OBJECTIVE: Much of our understanding and knowledge of shoulder dystocia has been blurred by inconsistent and scientific studies that are of limited scientific quality. In an evidence-based format, we sought to answer the following questions: (1) Is shoulder dystocia predictable? (2) Can shoulder dystocia be prevented? (3) When shoulder dystocia does occur, what maneuvers should be performed? and (4) What are the sequelae of shoulder dystocia? STUDY DESIGN: Electronic databases, including PUBMED and the Cochrane Database, were searched using the key word "shoulder dystocia." We also performed

Stratification of outcome of shoulder dystocia according to maneuver used for delivery, retrospective cohort and meta-analysis.

BACKGROUND: Shoulder dystocia is an unpredictable obstetric condition with potential long-term neonatal complications. The risk of neonatal injury might be related to the condition itself as well as to the obstetrics maneuvers used for its release. OBJECTIVES: To examine the available evidence to assess current management and possible improvement of outcomes. SEARCH STRATEGY: A comprehensive search of MEDLINE, EMBASE, EMCARE, and The Cochrane Library database was performed, all studies reporting on neonatal outcomes in cases of shoulder dystocia stratified by obstetric maneuvers used for del
