Ten patients with sickle cell (SS) disease from a Jamaican family were found to have unusually high levels of haemoglobin F for this population. Each of them has inherited one sickle cell gene on a chromosome characterized by an arrangement of restriction fragment length polymorphisms (haplotype) which is very rare in the Jamaican population. Genetic analysis of the family suggests that there is a determinant linked to the beta-globin gene cluster, characterized by this haplotype, which is responsible for increased haemoglobin F production in response to anaemia. Interestingly this particular

Sickle cell (drepanocytic) anemia is a hereditary blood disease occurring very rarely in Mexico. A 13-year-old Mexican boy with sickle cell anemia eventually died of a cerebrovascular accident of the brain stem, as shown by computerized tomography (CT). A characteristic moyamoya-like angiographic pattern was demonstrated on bilateral carotid and left vertebral arteriograms. Moyamoya disease has no known etiology, but the characteristic angiographic features of moyamoya have been observed in conjunction with some other disease of known origin (including sickle cell anemia). It is therefore my b

Serum alpha-hydroxybutyrate dehydrogenase (alpha-HBDH) levels were determined in 28 children with homozygous sickle cell disease (SCD) during the steady state, crisis, and postcrisis periods. Serum alpha-HBDH levels were also studied in 20 patients with infection. In patients during the steady state, serum alpha-HBDH levels were significantly above control values. During the vaso-occlusive crisis there was a significant further increase in the alpha-HBDH levels. In patients with infection, however, there was no significant increase in serum alpha-HBDH levels. These findings clearly indicate th

Twenty patients with sickle cell disease (14 black females and 6 black males, mean age 31.5 +/- 9.3) were studied by quantitating peripheral T and B lymphocyte percentages and measuring lymphocyte blast transformation (LBT) in response to phytohemagglutinin-P, concanavalin-A and pokeweed mitogen. Compared to normal black controls (19 black females and 1 black male, mean age 32.0 +/- 9.2 years) sickle cell patients had decreased T lymphocytes (50.2 percent +/- 6.2 compare; to 66.2 percent +/- 1.7) and increased B lymphocytes (17.0 percent +/- 3.4 compared to 7.7 percent +/- 1.1). Sickle patient

The authors describe 18 cases of this syndrome and emphasize that attenuated clinical forms of homozygotic drepanocytosis exist, as well as, in a more general manner, in acute drepanocytic syndromes. The disease may first appear in adult life. The frequency of radiological signs in the spine has to be stressed, and the authors emphasize the diagnostic value of the "staircase" deformation of the vertebrae, representing the sequelae of bone ischemic lesions. The discovery of this radiological sign suggested the diagnosis of a hemoglobinopathy in two cases. This attenuated form of the disease pre

Complete closure of retinal neovascularisation due to proliferative sickle cell retinopathy was achieved in 28 'sea fans' (areas of neovascularisation) in 9 eyes of 9 patients by means of a triple freeze-thaw technique. However, 2 eyes developed subsequent rhegmatogenous retinal detachments, presumably related to vitreous traction on necrotic retina. We therefore recommend photocoagulation as the best treatment of proliferative sickle cell retinopathy. If opacities in the media prevent photocoagulation, a single freeze-thaw cycle is preferable.

Erythrocytes and progenitor-derived erythroblasts of sickle cell anemia patients from the Eastern Province of Saudi Arabia contain increased fetal hemoglobin and G gamma globin. A distinctive DNA polymorphism haplotype in the beta globin gene cluster (++- +-), tightly coupled to a C----T substitution at position -158 5' to the cap site of the G gamma globin gene, is strongly associated with sickle cell disease in this region. To determine whether the increased fetal hemoglobin production and/or elevated G gamma globin content are tightly linked to this haplotype, we studied 55 members of five

India along with Nigeria and DRC contribute to 57% of the world sickle cell anemia population. The annual number of newborns in India with SCA was estimated at 44,000 in 2010. Even with this high prevalence there is minimal information about genetic factors that influence the disease course in Indian patients. The current study was conducted on 240 patients with SCD and 60 with sickle cell trait, to determine the association of genetic variants at the BCL11A (rs1427407) and HBS1-MYB (rs6934903) loci with fetal hemoglobin levels (HbF). Both these loci have been implicated with influencing HbF l…

Research among African-Americans indicates this population perceives sickle cell (SCD) to be a serious disease and sickle cell trait (SCT) screening an important intervention. However, studies have consistently demonstrated a lower than desired uptake of SCD education, inadequate knowledge regarding personal and family trait status, and a low perceived susceptibility of giving birth to a child with the disease. We examined general attitudes and beliefs regarding genetics and genetic testing including prenatal testing and newborn screening; we used this information as the foundation to more spe…

The malarial hypothesis presupposes that carriers of sickle cell trait and beta-thalassaemia are born in accordance with Mendelian inheritance and later favoured by an environmental factor. However, an eventual distortion favouring the transmission of the mutant allele is also a plausible hypothesis. The Mendelian proportion was tested in the progeny of 201 sickle cell trait (AS) and 138 beta-thalassaemia (AT) probands married to individuals with normal haemoglobin (AA). An excess of births of heterozygotes in the offspring of the 107 AS mothers (144 AS:89 AA, chi2=12.98; p<0.001) and 95 AT mo…

Fourteen patients (21 hips) with osteonecrosis of the femoral head with collapse had the femoral head resurfaced with a cemented titanium shell. All of the femoral heads were Ficat stage III or IV. Of the 21 surgeries, 7 were failures. Treatment for all 4 patients with sickle cell disease or trait failed (100%). When the cases of 17 patients who did not have sickle cell disease or trait were reviewed separately, the success rate was 14 of 17 (82%). The follow-up periods (all > 5 years) of the 14 successful patients in this group averaged 6.2 years, and their average Harris hip score was 87 (10

Acute haematogenous osteomyelitis formed less than a quarter of the 125 consecutive cases of osteomyelitis seen by the authors and followed up for a minimum of six months and an average of one year and four months. Of the twenty-nine cases of acute osteomyelitis a third occurred in patients with normal haemoglobin genotype (AA); a third of homozygous S disease (SS disease) and sickle cell haemoglobin C disease (SC disease); a quarter in sickle cell trait (AS) and haemoglobin AC trait, while in the remaining cases the haemoglobin genotype was not known. The sex incidence was equal, and children…

1. The deformability of erythrocytes from patients with sickle-cell anaemia was measured with a St George's blood filtrometer at a range of oxygen partial pressures and at four levels of zinc loading. 2. When incubated in buffered saline containing zinc and the chelator ethyl maltol, erythrocytes rapidly accumulated zinc and thus their oxygen affinity was increased. 3. Neither the oxygen partial pressure nor zinc loading affected the filtration of normal erythrocytes. 4. Deoxygenation of sickle erythrocytes greatly impaired filtration, although the initial filtration rate declined sharply at d

beta-Thalassemia and sickle cell disease both display a great deal of phenotypic heterogeneity, despite being generally thought of as simple Mendelian diseases. The reasons for this are not well understood, although the level of fetal hemoglobin (HbF) is one well characterized ameliorating factor in both of these conditions. To better understand the genetic basis of this heterogeneity, we carried out genome-wide scans with 362,129 common SNPs on 4,305 Sardinians to look for genetic linkage and association with HbF levels, as well as other red blood cell-related traits. Among major variants aff
