Ceftriaxone (CTRX) is known to cause reversible biliary stones/sludge, which is called biliary pseudolithiasis. We report two rare cases of biliary obstruction by pseudolithiasis shortly after completing CTRX treatment. Stones and sludge, which had not been detected before CTRX administration, appeared in the gallbladder and common bile duct and led to biliary obstruction and acute cholangitis. We reviewed the records of 20 liver transplant patients who underwent 28 procedures [endoscopic retrograde cholangiopancreatography (ERCP)] to rule out biliary obstruction, treat bile leaks, dilate and/or stent strictures, or remove stones and debris. Three patients (two with abnormal T-tube cholangiograms and one with hyperbilirubinemia) underwent ERCP to rule out obstruction. We report a case of chronic alcoholic pancreatitis with obstructive jaundice due to compression of the common bile duct by pancreatic pseudo … ilia and Pseudomonas aeruginosa. The patient was treated with broad-spectrum antibiotic, endoscopic sphincterotomy, endoscopic nasopancreatic duct drainage, and transpapillary pancreatic endoprosthesis. Jaundice subsided gradually during admission and a marked reduction of pancreatic pseudocyst was found 18 months after discharge.
