[Triceps tendon rupture : Double-row repair and overview of alternative techniques].

Triceps tendon rupture is a rare injury. While partial tendon ruptures can be treated non-operatively, complete ruptures are an indication for surgical treatment in order to restore strength and a full range of motion. Although many different methods have been published, there is no consensus on the optimal surgical technique. We report the case of a patient who suffered from a complete triceps rupture after a fall. The injury was treated by open reduction and refixation of the tendon using the double-row technique. The following article describes the technique and highlights its advantages and

Anatomy and biomechanical aspects of the gastrocsoleus complex.

The complexity of its anatomy coupled with the biomechanics of the Achilles tendon may explain the frequency of injury to this structure. Its unique characteristic of the muscle crossing three joints (knee, ankle, and subtalar joints) makes it more susceptible to injury than muscles that span a single joint. A better understanding of the contributing pathologic conditions associated with functional shortening of the gastroc-soleus complex and its effects on the normal biomechanics of the foot and ankle may improve the treatment of the many and varied pathologies that occur within the tendon it

Immunoregenerative Microenvironment Modulation in Tendon-Ligament Regeneration: Evolution of Chitosan-Based Composite Biomaterials.

Tendon and ligament injuries remain challenging to treat because healing may result in fibrosis, incomplete tendon-bone integration, impaired biomechanical properties, and reinjury. This review critically synthesizes preclinical evidence on chitosan-based composite biomaterials for tendon-ligament regeneration, focusing on their evolution from structural scaffolds toward multifunctional immunoregenerative platforms. Evidence across immune regulation, inflammatory signaling, angiogenesis, progenitor-cell activity, extracellular matrix remodeling, fibrocartilage formation, enthesis regeneration,

Biglycan Reconstitutes a Neonatal ECM Signaling Microenvironment to Drive Stem Cell-Mediated Tendon Regeneration via a Scaffold-Free Cell Sheet Platform.

Unlike newborns, tendon injuries in adults usually lead to fibrotic scarring rather than functional regeneration. This difference is primarily due to the loss of neonatal extracellular matrix (nECM) signaling in adulthood. In this study, we investigated the molecular mechanisms by which a key neonatal ECM proteoglycan, biglycan (Bgn), orchestrates the behavior of tendon stem/progenitor cells (TSPCs) within a scaffold-free 3D cell sheet microenvironment that recapitulates native tendon architecture. Through immunofluorescence screening, we confirmed that Bgn is the predominant proteoglycan in n…

Open Proximal Hamstring Repair.

BACKGROUND: Proximal hamstring ruptures meeting operative criteria may be treated through endoscopic, open, or combined techniques. Open techniques allow for facilitated tendon visualization and mobilization with ease of suture passage. INDICATIONS: Proximal hamstring repairs are indicated for complete 3 tendon avulsions; partial avulsions with 2 or more tendons involved with more than 2 cm of retraction in young, active patients; and partial avulsion injuries or chronic tears that remain refractory to conservative treatment. TECHNIQUE DESCRIPTION: Through an incision along the gluteal creas

Method development and characterization of chick embryo tendon mechanical properties.

Tendons are involved in multiple disorders and injuries, ranging from birth deformities to tendinopathies to acute ruptures. The ability to characterize embryonic tendon mechanical properties will enable elucidation of mechanisms responsible for functional tendon formation. In turn, an understanding of tendon development could inform approaches for adult and embryonic tendon tissue engineering and regenerative medicine. The chick embryo is a scientifically relevant model that we have been using to study Achilles (calcaneal) tendon development. Chick embryo calcaneal tendons are challenging to

Proximal patellar tendon pathology can develop during adolescence in young ballet dancers-A 2-year longitudinal study.

Patellar tendinopathy (tendon pain and dysfunction), or jumper's knee, is prevalent in adult jumping athletes. Pathology in the proximal patellar tendon is a key risk factor for developing patellar tendinopathy. When pathology develops in the proximal patellar tendon is not known, although it is reported to exist in adolescent athletes. The aim of this study was to follow young jumping athletes (ballet dancers) through adolescence to identify whether pathology develops and its relation to the adolescent growth spurt. Fifty-seven elite ballet students between ages 11 and 18 were monitored for 2

Clinical, Radiological, and Kinematic Impact of Gluteus Maximus Tendon Release During Total Hip Arthroplasty: A Prospective Randomized Controlled Trial.

BACKGROUND: Release of the gluteus maximus tendon during a total hip arthroplasty (THA) can lead to gluteal dysfunction. Sciatic nerve injury, while rare, remains a serious complication of THA. During deep hip flexion, release of the tendon increases the distance between the femoral neck and the sciatic nerve, potentially reducing the risk of nerve injury. The functional consequences of such a release have yet to be investigated. The aim of this study was to evaluate the effects of gluteus maximus tendon release on the development of gluteal dysfunction using clinical, radiological, and kinema

Exosome-Infused DTM-MPs Hydrogel Scaffold: A Smart Platform for Tendon Repair with Sustained Bioactivity and Anti-Inflammatory Benefits.

The regeneration of tendons remains a major challenge in tissue engineering. In this study, we present an original scaffold-based approach for tendon repair that combines decellularized tendon matrix microparticles (DTM-MPs), exosomes derived from human adipose-derived stem/progenitor cells (sub-Exos), and collagen-binding domain (CBD) peptides. This hybrid scaffold (DTM-MPs/CBD@sub-Exos/Gel) was fabricated using a DTM-MPs-based hydrogel to provide sustained release of bioactive exosomes at the site of injury. Characterization of the scaffold revealed high bioactivity, biocompatibility, and en

Considerations in spontaneous quadriceps tendon rupture repair in end-stage renal disease patients: A case report.

INTRODUCTION: Spontaneous quadriceps tendon rupture (SQTR) is a rare injury to the knee extensor mechanism that is usually associated with systemic diseases such as end stage renal diseases (ESRD) and it is more prevalent in the elderly. Due to the underlying pathology, quadriceps tendon rupture warrants special considerations and management in its repair. CASE REPORT: We present two cases of quadriceps tendon rupture in end-stage renal disease (ESRD) patients. The first case; a 57 years old female who had bilateral SQTR and is undergoing hemodialysis. The second patient, a 26 years old male

[Radiological diagnosis of apophyseal injuries of the pelvic region].

Avulsion injuries of the pelvis are common sports-related findings in skeletally immature adolescent patients. They usually present as an avulsion of the unfused apophysis, resulting from forced muscular contraction during typical athletic movements or from chronic traction injuries. Acute apophyseal avulsion injuries are usually easily detected with radiography. Occasionally, advanced imaging such as ultrasound or magnetic resonance imaging is required in order to detect or fully assess the extent of injury. Chronic injuries can mimic aggressive entities, such as neoplasms and may thus cause

The Turtle Neck Sign: Identification of Severe Retracted Distal Biceps Tendon Rupture.

BACKGROUND: Chronic tendon retraction subsequent to distal biceps tendon rupture significantly increases repair difficulty and potential for tendon grafting. Biceps tendons that appear short or absent with magnetic resonance imaging (MRI) or that cannot be readily identified at surgery may erroneously be classified as irreparable. These apparent "absent" biceps tendons may actually be retracted and curled up inside the muscle, visually resembling the head-neck of a turtle retracted inside its shell (the "turtle neck sign"). When located, these tendons could be unfolded and repaired primarily.

Magnetic resonance imaging of the extensor mechanism.

Anterior knee pain is associated with many different causes. The clinical diagnosis may be unclear when the patient initially presents for evaluation. Magnetic resonance (MR) imaging is a valuable modality with high diagnostic accuracy in the evaluation of extensor mechanism pathology, which commonly presents with anterior knee pain. The most common disorders include patellar fracture, patellofemoral tracking abnormalities, tendon trauma and degeneration. These injuries manifest with characteristic patterns on MR imaging. This article discusses normal anatomy, common injuries, and diagnostic p

Extensor Mechanism Reconstruction Using Achilles Tendon Allograft With Suture Tape Augmentation.

The management of chronic extensor mechanism injuries represents a significant challenge for orthopaedic surgeons, with numerous options for graft choice and fixation construct, but no clear consensus on which technique achieves optimal outcomes. Although there is little published data regarding outcomes of different fixation methods, small case series have demonstrated modest success using Achilles tendon bone block allografts and transverse patellar screw fixation. In this technical note, we describe a surgical technique for the treatment of a chronic inferior pole patella fracture, with ext

Surgical treatment of distal biceps rupture.

Rupture of the distal biceps tendon accounts for 10% of all biceps brachii ruptures. Injuries typically occur in the dominant elbow of men aged 40 to 49 years during eccentric contraction of the biceps. Degenerative changes, decreased vascularity, and tendon impingement may precede rupture. Although nonsurgical management is an option, healthy, active persons with distal biceps tendon ruptures benefit from early surgical repair, gaining improved strength in forearm supination and, to a lesser degree, elbow flexion. Biomechanical studies have tested the strength and displacement of various repa

Triceps reconstruction using hamstring graft for triceps insufficiency or recurrent rupture.

Triceps ruptures are relatively rare injuries. When they occur, primary surgical repair of the tendon to the proximal ulna is recommended. However, some patients require reconstruction using tendon grafting due to shortening or insufficiency of the native triceps tendon. Triceps ruptures associated with biological abnormalities (such as renal insufficiency or metabolic disease) or recurrence of rupture represent situations where a stout augmented repair is desirable. Multiple allograft and autologous tendons have been described for augmentation, but the use of gracilis and semitendinosus tendo
