Open Latarjet with Metal-Free Cerclage Fixation.

Despite multiple modifications, the Latarjet is still the most popular procedure for recurrent anterior shoulder instability with glenoid bone loss. Partial or subtotal resorption of the graft is common, potentially leading to hardware prominence and risk of anterior soft-tissue impingement. To minimize the technical difficulties and morbidity associated with metallic implants, a coracoid and conjoint tendon transfer with a mini-open approach using Cerclage tape suture is described, as an alternative for the Latarjet procedure typically performed with metal screws and plates.

The treatment of iatrogenic hallux varus.

Though uncommon, iatrogenic hallux varus is most often the result of overresection of the medial eminence, overtranslation of an osteotomy, overrelease of the lateral soft tissues, or overtightening of the medial tissues. It is not always symptomatic, as the degree of deformity can be well tolerated. For soft-tissue reconstructions, releases have little role to play unless minor deformity is detected early on and the longevity of tendon transfer and tenodesis remains unknown. For bony reconstruction, arthrodesis is the recommended salvage technique.

Dislocation of the fifth metatarsal base following partial fourth and fifth ray amputation: a case report.

This case report presents a rare postoperative dislocation of the fifth metatarsal base following a healed open partial fourth and fifth ray amputation of a 62-year-old male veteran with poorly controlled diabetes mellitus. The dislocated fifth metatarsal base subsequently created a chronic ulceration and an inhibition of normal gait. The patient was taken to the operating room where the fifth metatarsal base was resected with transfer of the peroneus brevis tendon to the cuboid to maintain biomechanical stability.

The total anterior tenoarthrolysis in the treatment of the stiffness in flexion of the fingers.

The total anterior tenoarthrolysis (TATA) allows tenolysis, arthrolysis and skin repair in the same operation, avoiding combined different surgical procedures. The authors present a series of personal and original indications and propose a partial anterior tenoarthrolysis (PATA) for the treatment of isolated stiffness of the proximal interphalangeal joint or in the presence of less severe flexion contractures. The etiology was wide various, and the TATA revealed to be extremely useful also in tetraplegia or spasticity, permitting respectively the following tendon transfer and the hygienic impr…

Biceps long head tendon revisited: a case report of split tendon arising from single origin.

A 27-year-old, right-hand-dominant woman with a posttraumatic anterior shoulder dislocation 3 months earlier after traffic accident presented because of pain and limited range of motion in the right shoulder. On physical examination, the patient had negative instability tests and a sulcus sign. On arthroscopic examination, a bifurcate long biceps tendon with two limbs was observed about 1 cm distal to the origin in the supraglenoid tubercle. We found no evidence of a tear in the long biceps tendon on probing, and the margin of each limb was smooth and round. Although this anatomic variant may

Electromechanical delay of the hamstrings following semitendinosus tendon autografts in return to competition athletes.

PURPOSE: Knee flexor electromechanical delay (EMD) has been proposed as a contributing factor to non-contact anterior cruciate ligament (ACL) injury risk and the semitendinosus (ST) autograft technique has been shown to impair knee flexor torque at large angles of knee flexion. The purpose of this study was to analyse the effects of ACL reconstruction (ACLR) using the ST tendon autograft technique on knee flexor EMD across the knee flexion range of motion, in athletes who had returned to competition. METHODS: Athletes with ACLR (n = 8 females, n = 3 males, 1.7 ± 0.5 years post-surgery) and no

[Insertional Achilles tendinopathy : Differentiated diagnostics and therapy].

Achilles tendinopathy at the calcaneal insertion is classified into insertional tendinopathy, retrocalcaneal and superficial bursitis. The aim of this study was to present the current evidence on conservative and surgical treatment of insertional tendinopathy of the Achilles tendon. Conservative first-line therapy includes reduction of activity levels, administration of non-steroidal anti-inflammatory drugs (NSAID), adaptation of footwear, heel wedges and orthoses or immobilization. In addition, further conservative therapy options are also available. Eccentric stretching exercises should be i

A literature review of congenital digiti quinti varus: clinical description and treatment.

Congenital digiti quinti varus is a deformity in which the fifth toe overlaps the base of the fourth; it consists of three main components: adduction, dorsiflexion, and external rotation. Treatment for this deformity may consist of conservative or surgical techniques, depending on the severity and/or age of the patient. The surgical techniques recommended by various authors may involve skin plasties, tenotomies and capsulotomies, tendon transfers and lengthenings, and bone resections.

The modified Jones procedure for pes cavovarus with claw hallux.

The purpose of this study is to describe long-term outcomes of the modified Jones procedure for pes cavovarus with claw hallux deformity. Jones originally described an isolated transfer of the extensor hallucis longus tendon. However, this technique does not correct and stabilize the claw hallux deformity. Therefore, this operation has been modified to include arthrodesis of the interphalangeal joint of the hallux, distal stump tenodesis of extensor hallucis longus with brevis tendon, and osteotomy of the base of the first metatarsal in cases of fixed or structural deformity. Twenty-four feet

An alternative method to reconstruct grade-3 hypoplastic thumb by hemi-metacarpal transfer.

We designed a new method using hemi-longitudinal second metacarpal bone to reconstruct grade-3 hypoplastic thumbs. Seven patients were treated in two stages. In the first stage, the second metacarpal was split longitudinally and transferred to reconstruct the first metacarpal. In the second stage, opponensplasty was performed by transferring the FDS tendon. Bone union was achieved in all cases. All patients could oppose to their middle finger at least. They managed to do daily activities such as writing, eating, using smartphones and so on. This is a useful procedure to preserve a 5-digit hand

Arthroscopic reduction and repair of a locked posterior shoulder dislocation.

Locked posterior shoulder dislocations are an uncommon but difficult problem for the orthopaedic clinician. Furthermore, they are often missed on initial presentation, resulting in significant delays in treatment. Traditional treatment has involved formal open reduction, most commonly from an anterior approach, followed by transfer of the lesser tuberosity or subscapularis tendon into the anterior humeral head defect. We present the case of a patient with locked posterior shoulder dislocation, who was treated with arthroscopically assisted reduction followed by arthroscopic posterior stabiliza

Side-to-side differences in neuromuscular function and associated joint mechanics in the lower limbs and trunk during walking and running one year after acute unilateral Achilles tendon rupture.

BACKGROUND: Achilles tendon rupture can result in persistent impairments in neuromuscular function and associated joint mechanics, reflecting altered neural-mechanical coupling despite surgical or nonsurgical treatment and structured rehabilitation. However, knowledge of how these deficits affect lower limb muscle coordination and joint power distribution across the kinematic chain remains limited. AIM: This cross-sectional exploratory study investigated side-to-side differences in muscle activation, range of motion, joint power, joint moment, and support moment during walking and running one

Editorial Commentary: Lower Trapezius Transfer Delivers Durable, Meaningful Recovery at 5 Years: Thresholds Tell Only Part of the Story.

Arthroscopic-assisted lower trapezius tendon transfer has emerged as a compelling joint-preserving solution for massive irreparable posterosuperior rotator cuff tears, yet the field has lacked a robust framework for interpreting what constitutes a meaningful patient outcome after this procedure. Establishing procedure-specific minimal clinically important difference and patient acceptable symptom state thresholds in this setting is both timely and necessary. When such thresholds are derived, however, they must be understood as cohort-specific constructs that cannot be universally applied acros…

[Translated article] Cadaveric biomechanical study of the calcaneonavicular ligament in midfoot medial column collapse comparing two surgical techniques.

INTRODUCTION: The calcaneonavicular ligament (spring ligament) plays a fundamental role in calcaneonavicular static stability and medial longitudinal arch, injury which is related to flatfoot. OBJECTIVE: The primary objective was to compare the biomechanical behaviour of the spring ligament in a healthy foot and after section and repair with augmentation and transfer of the flexor digitorum longus (FDL). As secondary objectives we have the biomechanical comparison between isolated repair with augmentation associated or not with transfer. METHODS: This experimental biomechanical cadaver study
