Because the transplanted heart is denervated, classic angina as a symptom of allograft coronary vasculopathy rarely is perceived. Any cardiac transplant patients who presents with decreased exercise capacity, shortness of breath, or syncope should be assessed thoroughly. Unfortunately, the initial presenting symptom of transplant vasculopathy may be acute myocardial infarction, heart failure, or even sudden death. Patients should be evaluated on an annual basis for the presence of transplant coronary vasculopathy in addition to when clinical suspicion warrants. Coronary angiography has been th

Hepatic parenchymal cell transplantation is a promising method for managing patients with the acute liver failure and may create a bridge to whole organ grafting. Elimination or reduction of the immunogenicity of the hepatocytes would permit long-term graft survival without the need of non-specific immunosuppression. The presented experimental evidence suggests that modulation of hepatocyte immunogenicity by purification and cryopreservation reduces the alloresponse to hepatocytes both in vitro and in vivo. Identification of inoculated cells facilitates long-term monitoring of their localizati

Diabetes mellitus is the most devastating chronic disease of all time. This review discusses the current therapies for type 1 diabetes that are predicated on the restoration of insulin secretion by transplantation. Recent developments in vascularized pancreas transplantation have led to a dramatic increase in the number of these procedures performed worldwide, with over 10,000 cases reported currently to the International Pancreas Transplant Registry. Although the procedure contributes to a significant improvement in quality of life, compared with traditional insulin therapy, it still suffers

Preclinical studies have shown that hepatocytes transplanted into the liver function normally on a long term basis. The procedure is well tolerated and offers a wide range of potential clinical applications. Clinical case studies have been performed in the setting of acute and chronic liver failure as well as metabolic diseases. In particular, the case of a patient with Crigler-Najjar Syndrome Type I clearly demonstrated the long term viability of transplanted hepatocytes with stable metabolic function. Further studies are warranted to assess the full therapeutic potential of hepatocyte transp

During the last 10 years we performed 92 renal transplantations in our Skopje hospital (Macedonia), using 22 cadaver donors and 70 living donors. We also performed 15 explanations from cadavers and seven kidneys were allocated to former Yugoslavia. Standard surgical procedures were used for both living and cadaver donor explantations and transplantations. For living explantations, donors were matching in 66 cases (94.28%) and in four cases (5.7%) non-matching donors who however were relatives of the patient Explantations and transplantations took place only after all ethical- and legal-related

Recent data show that, despite a long period during which few elderly patients in end-stage renal failure received grafts, there are neither medical nor ethical grounds for avoiding kidney transplantation, at least in those aged under 70 or even 75 years of age. Units in which transplantation in older recipients is routine show a good survival of recipients, and comparable survival of grafts to those placed in younger recipients. This equality of graft survival with age arises because, although death with a functioning graft is more common in the elderly (principally from cardiovascular diseas

Early recognition and determination of the cause of renal failure in patients with ESLD can be difficult because of the potential interplay among various factors and the wide array of differential diagnoses. A systematic approach, however, assists clinicians to identify common and potentially reversible causes of ARF. It is crucial to distinguish patients with functional renal failure, such as HRS, from those with advanced irreversible renal disease. Isolated liver transplantation is the treatment of choice for the former, and CLKT may be a therapeutic option for the latter. Because of the eve

Autoimmune hepatitis (AIH), primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) represent good indications for orthotopic liver transplantation (OLT). While there is effective treatment for AIH (steroids with or without azathioprine) and PBC (ursodeoxycholic acid) no such treatment is currently established for PSC. The need of transplantation can be delayed for AIH and PBC with appropriate therapies, while treatment options for PSC are still controversially discussed. Although the time point for liver transplantation can be roughly estimated for AIH by failure of immunosup

BACKGROUND: On the basis of a 5-year experience with heart transplantation and long-term animal experimentation, a lung transplantation program was instituted in 1987. After 10 years of experience, the entire patient population was reviewed.
METHODS: Hospital records were reviewed to evaluate the underlying diagnosis, year of transplantation, type of procedure, and long-term follow-up. The changing scope of indications, procedures performed, and donor criteria, as well as survival data for various subgroups of high-risk candidates, were also examined.
RESULTS: A total of 283 heart-lung (n =

Intestinal transplantation has emerged in the last decade as a lifesaving procedure for patients with intestinal failure who are suffering from complications arising from the administration of total parenteral nutrition. Indications for transplantation include irreversible liver injury and loss of vascular access. At least 50 children in the United States may benefit from intestinal transplantation every year. In this article, indications, pre- and postoperative management, and outcomes of intestinal transplantation in children are discussed.

OBJECTIVE: This study reviews the indications, technical aspects, and experience with ex vivo and in situ split liver transplantation.
BACKGROUND: The shortage of cadaveric donor livers is the most significant factor inhibiting further application of liver transplantation for patients with end-stage liver disease. Pediatric recipients, although they represent only 15% to 20% of the liver transplant registrants, suffer the greatest from the scarcity of size-matched cadaveric organs. Split liver transplantation provides an ideal means to expand the donor pool for both children and adults.

The advent of erythropoietin (rHuEPO) has revolutionized the treatment of anemia in end-stage renal disease. While many studies indicate beneficial effects of rHuEPO in hemodialysis, peritoneal dialysis and predialysis patients, the impact of the drug in renal transplantation is less clear. Treatment with rHuEPO may reduce the degree of allosensitization produced by random blood transfusion while still allowing the possibility of the transplant survival benefit derived from deliberate transfusion. Recovery from anemia post-transplantation is hastened by treatment with rHuEPO, while patients wi

The first heart transplantation in the Nordic countries was performed at Rikshospitalet, Oslo in 1983. In this paper, we present our experience with this treatment up to 1999. 317 heart transplantations have been performed, an average of 23 transplantations per year. 82% of the recipients were males; 50% had heart failure due to coronary heart disease. Mean age of the recipients was 47 years (range 1-64). Our indications and contraindications are similar to most other transplantation centres. Triple immunosuppression with ciclosporin, prednisolone and azathioprine have been used as standard tr

Heart disease remains one of the leading causes of death in the western world. In the 35 years since the first human heart transplants, cardiac transplantation has become established as the therapeutic option of choice in the management of terminal cardiac failure. Since 1981, the introduction of cyclosporin for immunosuppression has dramatically increased cardiac transplantation. However, several obstacles limit further utilization, including limited availability of donor hearts, limited ischemic time tolerated by donor hearts, and chronic rejection. Research is underway into donor heart pres…

Transplantation is the therapy of choice for an increasing number of patients. Organ transplantation is used to treat irreversible kidney, heart, lung and liver failure. Stem cell transplantation (previously called bone marrow transplantation) is used to treat leukaemia, bone marrow failure, severe combined immunodeficiencies and various congenital metabolic disorders. After organ transplantation, the recipient's immune system will recognise and reject the transplanted organ. T-lymphocytes transferred along with the stem cells in stem cell transplantation will attack the recipient. This paper

Thoracic transplantation has been a clinical option for patients with end-stage heart and lung disease for three decades. Heart, lung, and combined heart-lung transplantations are no longer experimental procedures; they are a standard part of the treatment algorithm for selected patients with end-stage heart and lung disease. This article summarizes the current status of heart, lung, and heart-lung transplantations and provides an insight into the future of this field.
