Caloric function after endolymphatic sac surgery.

OBJECTIVE: To evaluate the effect of endolymphatic sac surgery on vestibular functions using caloric testing on electronystagmography (ENG). STUDY DESIGN: Retrospective chart review. METHODS: The medical records of 21 adult patients with unilateral Ménière's disease who underwent endolymphatic sac surgery between 1998 and 2004 were reviewed. With use of ENG, the absolute value of the caloric response of the operated ear (i.e., the cool + warm irrigation response) and the degree of reduced vestibular response (RVR) rates as indicators of caloric functions were compared before and after surger

Vestibular rehabilitation and recovery in divers with inner ear decompression sickness: a case series.

INTRODUCTION: The mechanism of injury and recovery of divers with inner ear decompression sickness (IEDCS) is not well understood and there is no consensus regarding management following recompression treatment. Given the rare occurrence, divers are not routinely offered the standard therapies that patients with other acute vestibular disorders may be offered such as vestibular rehabilitation. METHODS: This is an observational case series of 13 divers presenting acutely with IEDCS to DDRC Healthcare in Plymouth, UK between July 2021 and January 2024. Vestibular and balance tests were undertak

Vestibular compensation after vestibular schwannoma surgery: normalization of the subjective visual vertical and disability.

CONCLUSIONS: The degree of caloric weakness before surgery influences faster or slower recovery of patients undergoing vestibular schwannoma surgery. The Dizziness Handicap Inventory (DHI) is a good index to show the recovery of patients as it relates directly to an improvement or not of the subjective visual vertical (SVV). OBJECTIVE: To evaluate the process of recovery of patients as measured by the SVV and the DHI after surgical removal of vestibular schwannoma. METHODS: We studied 24 consecutive patients of the University Hospital of Salamanca who underwent vestibular schwannoma surgery.

Space motion sickness countermeasures: a pharmacological double-blind, placebo-controlled study.

INTRODUCTION: Space motion sickness (SMS), caused by a canal-otolith conflict, is currently treated with intramuscular promethazine. However, the drug has an inconsistent efficacy against SMS. We hypothesize that pharmacological depression of the semicircular canals (SCC) might relieve SMS. The aim of the present study was to identify the effects of meclizine (25 mg), dimenhydrinate (40 mg) combined with cinnarizine (25 mg), and promethazine (25 mg) combined with d-amphetamine (10 mg) on the SCCs and the otoliths. METHODS: This double-blind, placebo-controlled study was performed on 20 health

The map of dizziness in vestibular schwannoma.

INTRODUCTION: Dizziness is a frequent complaint in patients with vestibular schwannoma (VS). An abnormal vestibulo-ocular reflex (VOR) can explain this dizziness in patients with VS. The video Head impulse test (vHIT) offers a chance to describe specifically the VOR findings in such patients. STUDY DESIGN: Retrospective cases series study in a tertiary referral hospital. METHODS: Fifty consecutive patients with VS were classified in accordance with the morphology of the VOR; gain, covert saccade, and overt saccade were analyzed both in the affected side and in the healthy side. For all patie

Superior canal dehiscence: review of a new condition.

A new cause of sound and pressure induced vertigo, superior canal dehisence, is described. Auditory manifestations include hyperacusis to bone-conducted sounds and conductive hearing loss with normal acoustic reflexes. The diagnosis is reached by a directed history, documentation of upward and torsional nystagmus evoked by sound and pressure, and radiology. Acoustic reflexes and VEMP (vestibular evoked myogenic potentials) aid in the identification of patients with an apparent conductive loss with normal acoustic reflexes or have an asymptomatic dehiscense on radiology. Treatment involves avoi

Evaluation of long term tobramycin therapy in patients with cystic fibrosis and advanced pulmonary disease.

To nine cystic fibrosis patients with chronic bronchopulmonary infection of severely damaged lungs invaded by Pseudomonas aeruginosa, eleven courses of prolonged tobramycin treatment (5 mg/kg/day) for four to 16 weeks were administered. Pulmonary symptoms improved and a better quality of life was achieved in all but one patient. Objective parameters (chest X-ray, pulmonary function tests) changed to a lesser extent. In only one patient was Pseudomonas eradicated from the sputum but reappeared after discontinuation of therapy. In the rest of the patients Pseudomonas was significantly suppressed

[Audiovestibular manifestations in systemic vasculitis].

Systemic vasculitides constitute a heterogeneous group of diseases with frequent overlapping in their clinical findings and the size of the vessels involved. They may be primary or secondary to other diseases. Their common characteristic is the inflammation of blood vessels, giving rise to ischaemia. In the present article, we have reviewed the most common audiovestibular features that may be observed in patients with systemic vasculitides. Audiovestibular manifestations may be the first symptom of a systemic vasculitis. Audiological manifestations include sudden hearing loss and progressive s

Clinical and video head impulse test in the diagnosis of posterior circulation stroke presenting as acute vestibular syndrome in the emergency department.

INTRODUCTION: Head impulse test (HIT) is the critical bedside examination which differentiates vestibular neuritis (VN) from posterior circulation stroke (PCS) in acute vestibular syndrome (AVS). Video-oculography based HIT (vHIT) may have aadditional strength in making the differentiation. METHODS: Patients admitted to the emergency department of a tertiary-care medical center with AVS were studied. An emergency specialist and a neurologist performed HIT. vHIT was conducted by an neuro-otology research fellow. RESULTS: Forty patients 26 male, 14 female with a mean age of 49 years were inclu

Tuning of the ocular vestibular evoked myogenic potential (oVEMP) to air- and bone-conducted sound stimulation in superior canal dehiscence.

Recent studies have demonstrated the frequency selectivity of air-conducted (AC) and bone-conducted (BC) stimuli in eliciting ocular vestibular evoked myogenic potentials (oVEMPs). In this study, frequency tuning of the oVEMP was assessed in patients with superior canal dehiscence (SCD) and compared with responses previously reported for healthy subjects. Six (five unilateral) SCD patients were stimulated using AC sound (50-1,200 Hz) and BC transmastoid vibration (50-1,000 Hz). Stimuli were delivered at two standardized intensities: one the same as previously used for healthy controls and the

[Influence of different head-trunk positions to the upbeat-nystagmus].

The neck torsion test (NTT) can be used to evaluate the cervico-ocular reflex (COR). We think there are two main reasons why NRT has failed to gain general acceptance in clinical practice: the test does not fully exploit the somatosensory examination potential, and electronystagmography has been most commonly used but is inferior to videonystagmography for a very low-amplitude nystagmus. These considerations have led us to develop a modified neck torsion test (mNTT) with three-dimensional video-oculographic (3D-VOG) recording. Sensors on the video glasses enable three-dimensional and continuou

Digital craniocorpography and peripheral vestibular diseases.

Peripheral vestibular diseases (PVDs) often produce many symptoms but few or no measurable signs. Tests that capture and measure signs permit the comparison between normal and vertiginous populations. The aim of our research was to find movement patterns that could identify PVD patients, using a motion analysis laboratory. The results from 21 asymptomatic volunteers who underwent the Unterberger-Fukuda stepping test were compared to those of 38 PVD patients. We established diagnosis after a complete otoneurological workup prior to the stepping test. The VICON 370 (Oxford Metrics Ltd, Oxford, U

Bedside differentiation of vestibular neuritis from central "vestibular pseudoneuritis".

Acute unilateral peripheral and central vestibular lesions can cause similar signs and symptoms, but they require different diagnostics and management. We therefore correlated clinical signs to differentiate vestibular neuritis (40 patients) from central "vestibular pseudoneuritis" (43 patients) in the acute situation with the final diagnosis assessed by neuroimaging. Skew deviation was the only specific but non-sensitive (40%) sign for pseudoneuritis. None of the other isolated signs (head thrust test, saccadic pursuit, gaze evoked nystagmus, subjective visual vertical) were reliable; however

Walk-rotate-walk test identifies patients responding to Lempert's maneuver, with benign paroxysmal positional vertigo of the horizontal canal.

Two hundred and fifteen patients were diagnosed and treated for benign paroxysmal positional vertigo of the horizontal canal (BPPV-HC). All patients were tested with conventional positional nystagmus tests lying supine and rotating head for geotropic nystagmus, registered with Frenzels glasses, and in 109 cases with ENG. The walk-rotate-walk (WRW) test, developed by one of us (T.R.) and described in the text, was applied to all patients. The immediate good treatment results with Lempert's maneuver verify the correct diagnosis of BPPV-HC. The WRW test is a more sensitive test for BPPV-HC than e

Vestibular plasticity following orbital spaceflight: recovery from postflight postural instability.

Results of previous studies suggested that the vestibular mediated postural instability observed in astronauts upon return to earth from orbital spaceflight may be exacerbated by an increased weighting of visual inputs for spatial orientation and control of movement. This study was performed to better understand the roles of visual and somatosensory contributions to recovery of normal sensori-motor postural control in returning astronauts. Preflight and postflight, 23 astronaut volunteers were presented randomly with three trials of six sensory organization test (SOT) conditions in the EquiTes

Is the vestibular system affected by middle ear effusion?

The association between middle ear effusion (MEE) and vestibular pathology is controversial. To investigate this point, 50 children with MEE scheduled for myringotomy and grommet, and 20 normal hearing children without MEE scheduled for adenotonsillectomy, underwent vestibular investigations by craniocorpography and rotatory chair, preoperatively and postoperatively. Most of the correlations, preoperative vs. postoperative, and study vs. control groups, were nonsignificant. Assuming that serous labyrinthitis is responsible for vestibular involvement in MEE, the lack of significant vestibular p

Hearing and vestibular disturbances in Behçet's syndrome.

In order to evaluate the prevalence of audiovestibular disturbances in Behçet's syndrome, we submitted 20 consecutive patients and 20 control subjects to detailed audiologic and vestibular examination in the last 3 years. A sensorineural hearing loss was found in 12 patients, 2 of whom revealed sudden deafness. Two other patients with neuro-Behçet's syndrome showed a vestibular function deficit, and 3 others exhibited altered caloric stimulation test results. Two of these last patients also revealed a simultaneous bilateral auditory deficit. HLA typing showed the presence of the B51 antigen in

Does administration of an aminoglycoside in a single daily dose affect its efficacy and toxicity?

Treatment efficacy, oto- and nephrotoxicity, and aminoglycoside pharmacokinetics were evaluated in a prospective, comparative, randomized clinical study of aminoglycosides given once a day or three times a day for severe infections. Sixty patients were treated with netilmicin or gentamicin 4.5 mg/kg bodyweight/day, either once a day or divided into three doses a day. The patients were allocated randomly to the different groups. The clinical effect was difficult to compare in the different groups, because of the small numbers of patients. Therapeutic failures were seen in seven patients (three

Intrabeat relationship of postrotatory nystagmus in patients with neurological disorders.

The intrabeat relationships between velocity, amplitude and duration of the slow phase of nystagmus as well as between amplitudes of slow vs. fast phase of nystagmus were analysed during a postrotatory nystagmus response by using linear regression analysis. Three groups of patients were studied, each consisting of 10 subjects with lesions either in the peripheral vestibular system, in the frontal lobe, or in the brain stem. Irrespective of the site of the lesion, all groups exhibited the same response pattern: a reduction in amplitude control of the end-point of nystagmus and an increase in du
