From 1980-1986 46 unselected aphakic patients suffering from persisting cystoid macular edema and vitreous incarceration to the surgical wound were treated by closed vitrectomy and removal of displaced vitreous through a limbal incision. We treated an opacified membrane on the anterior surfaces of the posterior capsule of two cynomolgus monkeys that had undergone extracapsular cataract extraction and intraocular lens insertion. After development of opacification of the posterior capsule, argon laser energy was employed in an attempt to debride the capsules while maintaining structural integrity of the capsule. With this technique, small areas of destruction of the membrane on the anterior surface of the posterior capsule were created with preservation of the capsule.

One hundred patients with aphakic detachment were treated with temporary balloon buckles. The balloons were deflated and withdrawn after seven to 10 days. Permanent attachment depended on cryo- or laser-induced adhesions round the break. The procedure was initially successful in 79 patients. Seventeen eyes from patients who had had successful intraocular lens surgery were obtained after the patients died: 14 posterior chamber lenses, 2 anterior chamber lenses, and 1 iris fixation lens. None of these lenses showed significant inflammatory reaction or scarring. Despite iris tucking in the eyes with anterior chamber lenses and asymmetrical supporting loop location in the eyes with posterior chamber lenses, the intraocular lenses seemed to function well.

Corneal edema developed in seven aphakic eyes of six patients (three men and three women ranging in age from 60 to 88 years) who used daily- or extended-wear hydrophilic and silicone contact lenses. This edema, which developed suddenly, persisted despite discontinuation of contact lens wear and treatment with topical corticosteroids. The onset of the edema occurred in two patterns. One hundred and sixty-three cases of aphakic retinal detachment were reviewed and found to be divided equally between eyes which prior to cataract surgery had been myopic, highly myopic or in a low myope group. Only 30 per cent of the non-myopic eyes had not suffered some form of complication and these eyes constituted only 10 per cent of the whole study. It is concluded that non myopic eyes do not require prophylaxis but that most of the myopes do. Consideration should be given to extra-capsular cataract surgery in addition to prophylactic cryotherapy in the high risk groups.

Fifteen aphakic eyes in 15 selected patients who underwent penetrating keratoplasty were successfully fitted with extended-wear contact lenses three to 42 months postoperatively. After an average follow-up of 17 months, there was no graft edema, no microbial corneal ulcers, and no episode of graft rejection.
