Head and helmet biodynamics and tracking performance in vibration environments.

INTRODUCTION: There are potential effects of vibration on aircrew performance and safety when using helmet-mounted equipment. The objective of this study was to quantify the effects of head orientation and helmet center-of-gravity (CG) on head and helmet biodynamics and tracking performance during exposures to aircraft buffeting and quasi-random vibration. METHODS: Three head orientations, including two off-axis or off-boresight configurations [Side (40 degree elevation, 70 degree azimuth) and Up (40 degree elevation, 0 degree azimuth)], and three helmet CGs were tested. The overall head, hel

Bacterial meningitis exposure during an international flight: lessons for communicable pathogens.

Air transport of infectious patients presents challenges for screening, post-exposure follow-up of fellow passengers, and international coordination issues. This report illustrates how an index case may not receive a clear diagnosis until days after the flight of interest, complicating treatment and fellow passenger tracking. This patient was diagnosed with meningococcal meningitis after a transatlantic flight with over 200 other passengers. In such cases, prompt initiation of public health measures and rapid coordination between various agencies may be required to limit outbreaks. Similar con

Psychiatric diagnoses as a cause of medical evacuation.

The U.S. military is one of the best trained military organizations in the world. However, war-zone stressors may compromise the psychological resilience of even the most conditioned U.S. service members. The purpose of this study was to investigate the contributing causes for medical evacuation (medevac) during the present war on terrorism. We studied 5,671 cases of medevac reports during Operations Enduring and Iraqi Freedom (OEF and OIF). Our data indicated that psychiatric problems were among the leading causes of medevacs during this period of time. In fact, during March to September 2003

Optical computer recognition of facial expressions associated with stress induced by performance demands.

Application of computer vision to track changes in human facial expressions during long-duration spaceflight may be a useful way to unobtrusively detect the presence of stress during critical operations. To develop such an approach, we applied optical computer recognition (OCR) algorithms for detecting facial changes during performance while people experienced both low- and high-stressor performance demands. Workload and social feedback were used to vary performance stress in 60 healthy adults (29 men, 31 women; mean age 30 yr). High-stressor scenarios involved more difficult performance tasks

Sinus vein thrombosis following exposure to simulated high altitude.

A relation exists between high altitude exposure and a hypercoagulable state, the nature of which is not entirely clear. This has been mostly reported in mountain climbers. We report a 19-yr-old female, working as a high-altitude chamber instructor, who presented with severe frontal headaches which persisted for a month following routine high altitude chamber training. The patient was in generally good health and was using oral contraceptives for 3 yr prior to the event. Due to the unremitting nature of the symptoms, the patient was admitted to a neurology department, and computerized tomograp

Rotation direction change hastens motion sickness onset in an optokinetic drum.

BACKGROUND: Many stationary subjects who view the patterned interior of a rotating cylinder (optokinetic drum) experience motion sickness (MS) symptoms. An experiment was conducted to investigate the effects of rotation direction change on MS onset and severity. It was predicted that intermittently changing rotation direction would hasten MS onset due to an increased degree of visual/vestibular sensory conflict. METHODS: There were 12 individuals who participated in the experiment (4 men, 8 women, mean age = 24.4 yr). Subjects viewed the interior of an optokinetic drum that rotated at 5 rpm (

Effects of ethnicity and gender on motion sickness susceptibility.

INTRODUCTION: Susceptibility to motion sickness (MS) is known to be affected by gender and ethnic origin, but whether gender and ethnicity are interacting is unknown. METHODS: We investigated MS development in healthy Caucasian subjects (n = 227), and in subjects of Chinese origin (n = 82). All subjects were exposed to nausea-inducing body rotations in a rotation chair, and rotated around the yaw axis for 5 x 1 min, while they were instructed to move their heads. Prior to rotation, subjects had to fill out a motion-sickness susceptibility questionnaire (MSSQ). Total rotation tolerance time (R

Motion sickness symptoms in a ship motion simulator: effects of inside, outside, and no view.

INTRODUCTION: Vehicle motion characteristics differ between air, road, and sea environments, both vestibularly and visually. Effects of vision on motion sickness have been studied before, though less systematically in a naval setting. It is hypothesized that appropriate visual information on self-motion is beneficial in a naval setting and that task performance is likely reduced as sickness increases. METHODS: Using a within-subjects design, 24 subjects were exposed to 30 min of motion in a ship's bridge motion simulator with 3 visual conditions: an Earth-fixed outside view; an inside view th

Motion-sickness medications for aircrew: impact on psychomotor performance.

INTRODUCTION: Motion sickness remains a significant problem for aircrew both in the flying environment (airsickness) and for aircrew deployed at sea (seasickness). While some anti-motion-sickness medications provide reasonable efficacy, adverse neurocognitive effects limit their use in military personnel engaged in safety-sensitive operational roles such as flying. The purpose of this study was to assess the impact on psychomotor performance of promethazine, meclizine, and dimenhydrinate and to determine if the addition of pseudoephedrine or damphetamine to promethazine would ameliorate its ad

Measurement of force exerted on footplates by centrifuge subjects.

INTRODUCTION: Increased G-tolerance in flight is usually achieved by isometric contraction of the muscles of the abdomen and of lower extremities, along with forceful expiration against a closed or partially closed glottis. Measurement and analysis of the forces developed by a pilot's leg muscles during +Gz acceleration is one of the steps needed to develop specialized physical conditioning programs. METHODS: We developed a system for continuous measurement of forces exerted on simulated rudder pedals in a human centrifuge. RESULTS: Data from five test subjects showed that the system could m

Model of depressed myocardium shows orthostatic intolerance with or without reduced blood volume.

INTRODUCTION: The development of orthostatic hypotension (OH) is complex and multi-factorial. Previous simulation work indicates that myocardial contractility depression (MCD) may increase OH when there is a total blood volume decrease. This paper hypothesized that MCD increased OH in both humans with and without decrease in blood volume. METHODS: A model, which was previously used to reproduce cardiovascular response to lower body negative pressure (LBNP), hypovolemia, and MCD, was modified by incorporating the physiologic mechanism of plasma filtration into the interstitium during LBNP. The

Centrifuge braking effects on cardiac arrhythmias occurring at high +Gz acceleration.

Abrupt onset and offset of acceleration induces hemodynamic changes that activate a number of reflex cardiovascular responses. At high +Gz on a centrifuge, apparently healthy subjects occasionally develop a serious cardiac arrhythmia such as supraventricular tachycardia or asystole. We report on four such cases and examine the effect of rapid centrifuge braking (1 G x s(-1)) on the arrhythmia. It appears that the hemodynamic consequences and reflex responses produced by rapid deceleration may have a positive effect on paroxysmal supraventricular tachycardia, slowing the heart rate or terminati

Renal artery dissection associated with Gz acceleration.

A 55-yr-old male presented with flank pain and nausea minutes after intensive aerobatic flight maneuvers. An initial diagnosis of acute appendicitis was made. Computed axial tomography and renal arteriography showed a right kidney with two renal arteries, a right upper pole infarction, and a dissection in the upper renal artery which had a more vertical trajectory than the usual main renal artery. No signs of diseases known to be associated with renal artery dissection were present. The patient recovered without residual hypertension. Heavy positive G loads may have potential to cause renal ar

Decompression sickness in Miskito Indian lobster divers: review of 229 cases.

BACKGROUND: The Miskito Indian lobster divers of Central America employ very provocative diving profiles and experience severe neurological decompression sickness (DCS) and/or arterial gas embolism (AGE). Scientific data are scarce regarding the clinical patterns of injury, response to treatment, and functional outcomes for such cases. METHODS: A retrospective review of 229 cases of DCS and/or AGE was conducted at 2 hyperbaric units in Central America. RESULTS: The following deficits were recorded on presentation: any neurological deficit: 94%; motor: 79%; sensory: 60%; urinary: 48%; reflex:

Hearing and performance during a 70-h exposure to noise simulating the space station environment.

INTRODUCTION: Elevated hearing thresholds have been documented in some astronauts after long-term spaceflights although noise levels were lower than those normally associated with noise-induced hearing loss in ground-based operations. The present study was conducted to determine whether prolonged exposure (70 h) to levels (72 dBA) recorded on the International Space Station (ISS) service module would impact diverse measures of auditory function, as well as cognition and memory, motivation, and cardiovascular function. METHOD: Five mixed gender subgroups of five normal-hearing subjects, aged 2

Comparison of mathematical model predictions to experimental data of fatigue and performance.

As part of the "Fatigue and Performance Modeling Workshop," six modeling teams made predictions for temporal profiles of fatigue and performance in five different scenarios. One scenario was based on a laboratory study of fatigue and performance during 88 h of extended wakefulness with or without nap opportunities. Another scenario was based on a field study of alertness in freight locomotive engineers. Two scenarios were based on laboratory studies with various conditions of chronic sleep restriction and recovery. There was a theoretical scenario for future ultra-long-range flight operations

Team climate at Antarctic research stations 1996-2000: leadership matters.

INTRODUCTION: The popular assumption is that extreme environments induce a climate of hostility, incompatibility, and tension by intensifying differences and disagreements among team members. Team members' perceptions of team climate are likely to change over time in an extreme environment, and thus team climate should be considered as a dynamic outcome variable resulting from multiple factors. In order to explore team climate as a dynamic outcome, we explored whether variables at multiple levels of analysis contributed to team climate over time for teams living and working in Antarctica. Exercise effects during diving and decompression on postdive venous gas emboli.

BACKGROUND: Exercise and diving have generally been associated with an increased risk of decompression sickness (DCS), thus accounting for the lack of studies involving exercise during decompression. However, theoretical and observational evidence contrary to this association motivated the present investigation on the effects of moderate, intermittent exercise during diving and/or during decompression on venous gas emboli (VGE) activity following a dive. HYPOTHESIS: VGE observed at both the precordium and subclavian vein sites after diving should be reduced if moderate exercise is performed d
