Automatic diagnosis of strict left bundle branch block using a wavelet-based approach.

Patients with left bundle branch block (LBBB) are known to have a good clinical response to cardiac resynchronization therapy. However, the high number of false positive diagnosis obtained with the conventional LBBB criteria limits the effectiveness of this therapy, which has yielded to the definition of new stricter criteria. They require prolonged QRS duration, a QS or rS pattern in the QRS complexes at leads V1 and V2 and the presence of mid-QRS notch/slurs in 2 leads within V1, V2, V5, V6, I and aVL. The aim of this work was to develop and assess a fully-automatic algorithm for strict LBBB…

Significant mitral regurgitation as a predictor of long-term prognosis in patients receiving cardiac resynchronisation therapy.

BACKGROUND: Cardiac resynchronisation therapy (CRT) has been shown to reduce functional mitral regurgitation, although the relationship between significant mitral regurgitation (SMR) and the clinical prognosis of CRT remains uncertain. AIM: We sought to investigate the association of baseline SMR with long-term outcomes in patients undergoing CRT. METHODS: A total of 296 consecutive patients undergoing CRT were enrolled. SMR was quantified by colour Doppler in all patients at baseline and defined as level ≥ 3 on the severity scale. The primary endpoints included all-cause death, heart failur

Cardiac Sarcoidosis, Left Ventricular Impairment and Chronic Right Ventricular Pacing: Pacing or Pathology?

BACKGROUND: Cardiac sarcoidosis (CS) is an uncommon and under-recognised disease which most frequently presents with atrioventricular (AV) block and may also present with ventricular arrhythmias and left ventricular (LV) systolic dysfunction. Because of its protean clinical manifestations, confirming a diagnosis of CS is often challenging. METHODS: We report two cases where patients presented with atrioventricular (AV) block without evidence of underlying myocardial disease, underwent chronic dual-chamber pacing, and presented several years later with severe LV systolic dysfunction. RESULTS:

Cardiac resynchronisation therapy in 2015: keeping up with the pace.

Despite improved understanding of the pathophysiology of heart failure (HF) and availability of better medical therapies, HF continues to grow as a cause of morbidity and mortality in Australia and worldwide. Over the past decade, cardiac resynchronisation therapy (CRT), or biventricular pacing, has been embraced as a powerful weapon against this growing epidemic. However, much has changed in our understanding of dyssynchrony in HF, and this has led to a change in guidelines to ensure more appropriate selection of CRT candidates to improve the 'non-response' rate. More data have also emerged a

Will biventricular pacing replace right ventricular pacing for antibradycardia therapy?

Evaluation of: Curtis AB, Worley SJ, Adamson PB, et al; Biventricular versus Right Ventricular Pacing in Heart Failure Patients with Atrioventricular Block (BLOCK HF) Trial Investigators. Biventricular pacing for atrioventricular block and systolic dysfunction. N. Engl. J. Med. 368(17), 1585-1593 (2013). Recent trials have shown the benefit of cardiac resynchronization (CRT) in mild or moderate heart failure with a left bundle branch block or intraventricular conduction delay. Randomized trials (including the BLOCK-HF study) have now shown that irrespective of baseline QRS duration, long-term

[Acute and chronic heart failure].

The initial therapy of chronic heart failure is still based on diuretics, angiotensin-converting enzyme (ACE) inhibitors, beta-blockers and … stitution in chronic heart failure and on the indications for implantable converter defibrillators, cardiac resynchronization therapy and other cardiac devices. Medicinal therapy of acute heart failure is still limited. For patients who cannot be treated with medicinal therapy, mechanical circulatory support, such as extracorporeal membrane oxygenation (ECMO) should be recommended …

Left Ventricular Pacing-Associated Cardiomyopathy Revealing Myocardial Vulnerability in Congenital Complete Heart Block.

BACKGROUND: Mechanisms underlying cardiac manifestations of immune-mediated congenital complete heart block (CCHB) remain unclear. Dilated cardiomyopathy (DCM) in immune-mediated CCHB has been associated with RV pacing, whereas reports of LV pacing-associated DCM are lacking. CASE SUMMARY: A girl with immune-mediated CCHB showed endocardial fibroelastosis (EFE) at 20 weeks of gestation before heart block developed at 22 weeks. After LV basal-lateral pacing, she developed DCM with LV dyssynchrony. Cardiac resynchronization therapy (CRT) improved LV function and induced reverse remodeling. CON…

Inappropriate Defibrillator Shocks During Cervical Medial Branch Radiofrequency Ablation: A Case Report.

A 52-year-old woman with a history of cervical spondylosis, nonischemic dilated cardiomyopathy, and complete heart block with implantation of a cardiac resynchronization therapy defibrillator (Inogen XR, Boston Scientific, Natick, MA) underwent bilateral fluoroscopy-guided radiofrequency ablation of the medial branch nerves at C4, C5, and C6 levels at an ambulatory surgery center. After completion of the seemingly routine procedure, several alerts, including 1 inappropriate shock, were received via Boston Scientific's remote monitoring system. Tracings also showed that pacing inhibition occurr…

Atrial fibrillation and conduction system disease: the roles of catheter ablation and permanent pacing.

Atrial fibrillation (AF) and diseases of the cardiac conduction system frequently co-exist, and interactions between these rhythm disturbances can adversely impact patient outcomes. Concurrent AF and sinus node disease often manifests as the tachy-brady syndrome wherein the underlying sinus node dysfunction can pose a challenge to AF management. Similarly, the combination of AF and left bundle branch block increases mortality in individuals with co-existent heart failure and hampers effective delivery of cardiac resynchronization therapy. A thorough understanding of the therapeutic interventio…

Ventricular tachycardia storm after initiation of biventricular pacing: incidence, clinical characteristics, management, and outcome.

INTRODUCTION: Cardiac resynchronization therapy (CRT) or biventricular pacing (BVP) is becoming an important treatment option in patients with severe congestive heart failure (CHF) and electrical dyssynchrony. When combined with implantable cardioverter-defibrillator (ICD) therapy, cardiac resynchronization therapy with a defibrillator (CRT-D) has been shown to improve quality of life, functional class, and, most recently, survival. However, left ventricular (LV) stimulation in the form of BVP in patients at high risk of developing ventricular tachyarrhythmias has raised concerns that BVP may …

SANS FLUORO Too Big to Fail.

We present the first ever reported case of a super morbidly obese patient (BMI > 60) with drug refractory, symptomatic persistent atrial fibrillation who underwent an uncomplicated, but unsuccessful PVI ablation procedure and subsequently underwent AV node ablation and cardiac resynchronization therapy - pacemaker (CRT-P) insertion using a zero fluoroscopy technique. This case demonstrates the following two critical points: (1) difficulties in the treatment of massively obese patients with arrhythmias1; (2) increased use of fluoroless procedures2-4.

His-bundle Pacing to Left Bundle Branch Pacing: Evolution of His-Purkinje Conduction System Pacing.

His-bundle pacing (HBP) constitutes an excellent alternative to right ventricular pacing. Recently, several studies have reported on the success and efficacy of HBP in patients with left bundle branch block requiring cardiac resynchronization therapy. Nonetheless, HBP may not always be feasible due to high capture thresholds or disease in the distal His bundle. The present report discusses the utility and feasibility of pacing in the left bundle branch area located distal to the site of conduction block.
