Differential response to cognitive stimulation in moderate versus moderately severe Alzheimer's disease.

Alzheimer's disease (AD) is characterized by impairments across several neurocognitive domains, particularly memory and executive function. The study explored the effectiveness of an individual cognitive stimulation (iCS) program on cognitive outcomes in older adults with moderate to moderately severe AD. A multicentre randomized controlled trial was conducted with 80 Portuguese older adults (Mage: 83.0 ± 7.1 years) with AD. Participants were randomly assigned to either iCS (n = 39; 49%) or treatment as usual (n = 41; 51%). Alzheimer's Disease Severity (ADS) categorized two groups based on Min

A neurofeedback-guided EEG and BCI framework for personalized attention rehabilitation in ADHD.

The integration of game-based cognitive training with electroencephalography (EEG)-based brain-computer interaction (BCI) has demonstrated potential for enhancing attention among individuals with attention-deficit hyperactivity disorder (ADHD). However, existing systems often lack adaptive difficulty regulation and rely solely on single-modal assessments, thereby limiting personalization and sustained engagement. This study developed and assessed an adaptive, multi-task EEG-BCI training system that combines real-time neurofeedback with machine learning-driven customization to bolster attention…

Preliminary efficacy of cognitive multisensory rehabilitation for neuropathic pain in chronic spinal cord injury: a phase I randomized controlled trial.

Neuropathic pain affects 69% of adults with spinal cord injury (SCI) and responds poorly to current treatments. Emerging evidence suggests that impaired mental body representations (MBR) may contribute to neuropathic pain. The parietal operculum and insula, key regions for pain perception and body awareness, show disrupted connectivity after SCI. We aimed to identify changes in brain function, neuropathic pain, and sensorimotor function after Cognitive Multisensory Rehabilitation (CMR) vs. usual care. CMR is a physical therapy which improves MBR. In this Phase I randomized delayed-treatment tr

Effect of a multicomponent exercise programme (VIVIFRAIL) on functional capacity in frail community elders with cognitive decline: study protocol for a randomized multicentre control trial.

BACKGROUND: The benefit of physical exercise in ageing and particularly in frailty has been the aim of recent research. Moreover, physical activity in the elderly is associated with a decreased risk of mortality, of common chronic illnesses (i.e. cardiovascular disease or osteoarthritis) and of institutionalization as well as with a delay in functional decline. Additionally, very recent research has shown that, despite its limitations, physical exercise is associated with a reduced risk of dementia, Alzheimer disease or mild cognitive decline. Nevertheless, the effect of physical exercise as a

Brain metabolic imaging-based model identifies cognitive stability in prodromal Alzheimer's disease.

The recent approval of anti-amyloid pharmaceuticals for the treatment of Alzheimer's disease (AD) has created a pressing need for the ability to accurately identify optimal candidates for anti-amyloid therapy, specifically those with evidence for incipient cognitive decline, since patients with mild cognitive impairment (MCI) may remain stable for several years even with positive AD biomarkers. Using fluorodeoxyglucose PET and biomarker data from 594 ADNI patients, a neural network ensemble was trained to forecast cognition from MCI diagnostic baseline. Training data comprised PET studies of p

Machine learning prediction prior to onset of mild cognitive impairment using T1-weighted magnetic resonance imaging radiomic of the hippocampus.

BACKGROUND: Early identification of individuals who progress from normal cognition (NC) to mild cognitive impairment (MCI) may help prevent cognitive decline. We aimed to build predictive models using radiomic features of the bilateral hippocampus in combination with scores from neuropsychological assessments. METHODS: We utilized the Alzheimer's Disease Neuroimaging Initiative (ADNI) database to study 175 NC individuals, identifying 50 who progressed to MCI within seven years. Employing the Least Absolute Shrinkage and Selection Operator (LASSO) on T1-weighted images, we extracted and refine

The role of serotonin 5-HT2A receptors in memory and cognition.

Serotonin 5-HT2A receptors (5-HT2ARs) are widely distributed in the central nervous system, especially in brain region essential for learning and cognition. In addition to endogenous 5-HT, several hallucinogens, antipsychotics, and antidepressants function by targeting 5-HT2ARs. Preclinical studies show that 5-HT2AR antagonists have antipsychotic and antidepressant properties, whereas agonist ligands possess cognition-enhancing and hallucinogenic properties. Abnormal 5-HT2AR activity is associated with a number of psychiatric disorders and conditions, including depression, schizophrenia, and d

A primate model of anterograde and retrograde amnesia produced by convulsive treatment.

A nonhuman primate model of the key cognitive effects of convulsive treatment was developed and tested. Rhesus macaques were trained on 3 tasks: a long-term memory task that required selection of a constant target from a background of distracters, an anterograde task that involved learning a new target each day against a variable number of distracters, and a task that assessed learning and memory for new and previously trained 3-item serial lists. This battery samples a range of cognitive functions, including orientation, working memory, retrograde amnesia for temporally graded stimuli, and an

Does cognitive impairment explain behavioral and social problems of children with neurofibromatosis type 1?

Thirty NF1-patients (mean age 11.7 years, SD = 3.3) and 30 healthy controls (mean age 12.5 years, SD = 3.1) were assessed on social skills, autistic traits, hyperactivity-inattention, emotional problems, conduct problems, and peer problems. Cognitive control, information processing speed, and social information processing were measured using 5 computer tasks. GLM analyses of variance showed significant group differences, to the disadvantage of NF1-patients, on all measures of behavior, social functioning and cognition. General cognitive ability (a composite score of processing speed, social in

Behavioral treatment for migraine prophylaxis in adults: Moderator analysis of a randomized controlled trial.

BACKGROUND: While growing evidence suggests the efficacy of various behavioral approaches to the preventive treatment of migraine, it remains largely unclear which behavioral interventions are indicated for which type of patient. This exploratory study aimed to identify moderators for the outcome between migraine-specific cognitive-behavioral therapy and relaxation training. METHODS: In this secondary analysis of an open-label randomized controlled trial, the data of N  =  77 adults (completer sample) with migraine (mean age = 47.4, SD = 12.2, 88% female), who were allocated to either migrain

Preventing postoperative delirium.

BACKGROUND: Delirium is a common complication in elderly hospitalized patients. It prolongs the length of hospital stay, raises costs, increases the workload of the nursing staff, and may necessitate transfer of the patient to a nursing home. The risk of postoperative delirium is particularly high in elderly patients with pre-existing cognitive deficits. METHODS: In an open study, we systematically assessed the frequency of postoperative delirium in patients over age 70 on two surgical wards of a general hospital. In a six-month "prevalence phase," from March to August 2011, we counted the nu

Supplementary VR batting training with slow-replay augmented feedback improves bat-ball contact precision.

Fast interceptive sport actions may provide too little usable error information for performance refinement, because performance-critical events unfold too quickly to be inspected directly. We tested whether making such information visible through slow-replay augmented feedback, as part of supplementary VR batting training added to usual practice, would support improvement in bat-ball contact precision. Thirty-seven collegiate batters were randomly assigned to a Training or Control group. Over four weeks, both groups continued usual practice; the Training group also completed eight VR batting s

Training-induced brain remapping in chronic aphasia: a pilot study.

BACKGROUND: The neural correlates of training-induced improvements of cognitive functions after brain damage remain still scarcely understood. In the specific case of aphasia, although several investigations have addressed the issue of the neural substrates of functional recovery, only a few studies have attempted to assess the impact of language training on the damaged brain. AIMS: The main goal of this study was to examine the neurobiological correlates of improved picture-naming performance in 2 aphasic patients who received intensive and specific training for a chronic and severe phonolog

A debate on working memory and cognitive control: can we learn about the treatment of substance use disorders from the neural correlates of anorexia nervosa?

BACKGROUND: Anorexia Nervosa (AN) is a debilitating, sometimes fatal eating disorder (ED) whereby restraint of appetite and emotion is concomitant with an inflexible, attention-to-detail perfectionist cognitive style and obsessive-compulsive behaviour. Intriguingly, people with AN are less likely to engage in substance use, whereas those who suffer from an ED with a bingeing component are more vulnerable to substance use disorder (SUD). DISCUSSION: This insight into a beneficial consequence of appetite control in those with AN, which is shrouded by the many other unhealthy, excessive and defi

Exercise-induced irisin-BDNF signaling in aging: implications for cognitive decline, sarcopenia, and neurodegeneration. A narrative review.

Age-related cognitive decline is closely associated with diminished neuroplasticity and disrupted neurotrophic signaling. Exercise is a potent modulator of the irisin-BDNF axis, a molecular pathway with emerging relevance to healthy aging, neuroprotection, and the prevention of neurodegenerative diseases in older adults. Through this analysys the author aimed to review evidence on the effects of different exercise modalities - with emphasis on aging populations - on the PGC-1α-FNDC5-irisin-BDNF signaling pathway, and to examine implications for cognitive health and neuroplastic adaptations acr
